Abstract Aims To describe women's experiences with a range of bladder self‐care practices. Design We conducted a secondary, directed content analysis of qualitative data from the Study of Habits, Attitudes, Realities and Experiences, a multisite focus group study designed to explore adolescent and adult women's experiences, perceptions, beliefs, knowledge and behaviours related to bladder health. This study was conducted by the National Institute of Diabetes and Digestive and Kidney Diseases' Prevention of Lower Urinary Tract Symptoms Research Consortium. Study methods were informed by the Consortium's conceptual framework, based on a social ecological model adapted from Glass and McAtee's Society‐Behavior‐Biology Nexus. Methods Participants were recruited at seven geographically diverse United States research centres between July 2017 and April 2018. Data for the current analysis were collected using a semi‐structured discussion group with 36 focus groups involving 316 community‐dwelling adult women aged 18–93 years. Coded text was re‐examined according to eight self‐care behavioural domains identified through literature review and expert opinion as potentially influencing bladder health. Results Participants described many self‐care practices they had adopted to prevent bladder problems or manage existing symptoms and conditions. Eight themes were identified: ‘Choosing fluids, foods and medications’; ‘Dressing for bladder health’; ‘Promoting bodily cleanliness’; ‘Managing toileting environments’; ‘Timing when to void’; ‘Exercising pelvic floor muscles for bladder control’; ‘Limiting physical activities that challenge the bladder’ and ‘Staying home and navigating when away’. Thirteen subthemes were derived from five of the eight themes. Conclusion Women use a broad array of self‐care practices related to their bladder health. Research is needed to examine the efficacy of self‐care behaviours for preventing or managing bladder symptoms and conditions, and to discern potential risks. Results have important implications for development of bladder health promotion interventions and public health messaging around women's bladder health. Implications for the Profession and/or Patient Care This study provides a comprehensive framework for understanding women's bladder self‐care practices, which can be used by clinicians and public health professionals in designing interventions to promote bladder health and function. Impact Women with and without lower urinary tract symptoms use a broad range of self‐care practices that may affect their bladder health, including some that may be harmful. Because of the high prevalence of bladder symptoms in women, this study may help patient assessment and counselling regarding self‐care practices. Exploring women's bladder self‐care practices: A qualitative secondary analysis

Using antibiotics without medical guidance (non-prescription antibiotic use) may contribute to antimicrobial resistance. Hispanic individuals are a growing demographic group in the United States (US) with a high prevalence of non-prescription antibiotic use. We investigated the effects of acculturation and subjective norms on Hispanic individuals' intentions to use antibiotics without a prescription from the following sources: (1) markets in the United States (not legal), (2) other countries (abroad), (3) leftovers from previous prescriptions, and (4) friends/relatives. We surveyed self-identified Hispanic outpatients in eight clinics from January 2020 to June 2021 using the previously validated Short Acculturation Scale for Hispanics (SASH). Of the 263 patients surveyed, 47% reported previous non-prescription use, and 54% expressed intention to use non-prescription antibiotics if feeling sick. Individuals with lower acculturation (Spanish-speaking preferences) expressed greater intentions to use antibiotics from abroad and from any source. Individuals with more friends/relatives who obtain antibiotics abroad were over 2.5 times more likely to intend to use non-prescription antibiotics from friends/relatives (p = 0.034). Other predictors of intention to use non-prescription antibiotics included high costs of doctor visits and perceived language barriers in the clinic. Antibiotic stewardship interventions in Hispanic communities in the United States should consider the sociocultural and healthcare barriers influencing non-prescription use and promote language-concordant healthcare. 

Acculturation and Subjective Norms Impact Non-Prescription Antibiotic Use among Hispanic Patients in the United States

Abstract Background: Patients’ situations can impact their intentions to use antibiotics without medical guidance (non-prescription use) in the future. This survey determines the prevalence of intended (future) use of non-prescription antibiotics for 13 predefined situations and identifies the sociodemographic characteristics associated with intended use for these types of situations. Methods: Patient surveys (N = 564) were conducted from January 2020 to June 2021 in the waiting rooms of 6 safety-net primary care clinics and 2 emergency departments in a private healthcare system. We used principal component analysis to identify 3 situational summary factors: barriers to a doctor visit, accessibility of non-prescription antibiotics, and previous symptom relief with antibiotics. Multivariate linear regression identified the sociodemographic predictors associated with each summary factor. Results: The most common situations triggering patients to use non-prescription antibiotics were a perceived high cost of doctor visits (29.8%), having leftover prescription antibiotics (50.4%), and experiencing symptom relief with prior use of antibiotics (47.5%). Multivariate regression results revealed that younger patients ( P &lt; 0.04) and patients attending the safety-net health system ( P &lt; 0.001) had more intended use of non-prescription antibiotics for all 3 summary factors. Conclusions: Future stewardship interventions should consider the types of situations that drive patients’ decisions to use antibiotics without a prescription. Interventions aimed at reducing barriers to health care (eg, high costs and long waits associated with doctor appointments) and educating individuals on medically appropriate, nonantibiotic treatment options may reduce antibiotic use and antimicrobial resistance. 

Situations predisposing primary care patients to use antibiotics without a prescription in the United States

Introduction: Only a few studies have assessed the relationship between deprivation and excessive antibiotic use. In Texas, antimicrobial prescription rates are particularly high compared with the rest of the US. This study analyzed the association between local area socioeconomic deprivation and providers’ fluoroquinolone claim rates among beneficiaries 65 years and older in Texas. Methods: This ecological study utilized provider- and area-level data from Medicare Part D Prescribers and the Social Deprivation Index (SDI) repositories. Negative binomial regression models were employed to evaluate the relationship between provider- and area-level characteristics (prescriber’s gender, specialty, rural-urban community area, beneficiaries’ demographics, area-level population, and SDI) and fluoroquinolone claim rates per 1,000 beneficiaries. Results: A total of 11,996 providers were included. SDI (IRR 0.98, 95% CI 0.97–0.99) and male providers (IRR 0.96, 95% CI 0.94–0.99) were inversely associated with claim rates. In contrast, several factors were associated with higher claim rates, including non-metropolitan areas (1.04, 95% CI 1.00–1.09), and practices with a high proportion of male (IRR 1.12, 95% CI 1.10–1.14), Black (IRR 1.05, 95% CI 1.03–1.07), or Medicaid beneficiaries (IRR 1.15, 95% CI 1.12–1.17). Effect modification was observed between SDI and rurality, with higher SDI in non-metropolitan areas associated with higher claim rates, whereas SDI in metropolitan areas was inversely related to claim rates. Conclusion: Lower fluoroquinolone claim rates were observed among Texas Medicare providers in metropolitan areas with higher SDI. Conversely, higher rates were observed in rural areas with higher SDI. More studies are needed to understand the underlying causes of this variation and develop effective stewardship interventions.

Analyzing the relationship between socioeconomic deprivation and outpatient Medicare Part D fluoroquinolone claim rates in Texas

ABSTRACT Taking leftover prescribed antibiotics without consulting a healthcare professional is problematic for the efficacy, safety, and antibiotic stewardship. We conducted a cross-sectional survey of adult patients in English and Spanish between January 2020 and June 2021 in six safety-net primary care clinics and two private emergency departments. We assessed the reasons for stopping prescribed antibiotics early and what was done with the leftover antibiotics. Additionally, we determined 1) prior leftover antibiotic use, 2) intention for future use of leftover antibiotics, and 3) sociodemographic factors. Of 564 survey respondents (median age of 51), 45% (251/564) reported a history of stopping antibiotics early, with 171/409 (42%) from safety net and 80/155 (52%) from the private clinics. The most common reason for stopping prescribed antibiotics early was “because you felt better” (194/251, 77%). Among survey participants, prior use of leftover antibiotics was reported by 149/564 (26%) and intention for future use of leftover antibiotics was reported by 284/564 (51%). In addition, higher education was associated with a higher likelihood of prior leftover use. Intention for future use of leftover antibiotics was more likely for those with transportation or language barriers to medical care and less likely for respondents with private insurance. Stopping prescribed antibiotics early was mostly ascribed to feeling better, and saving remaining antibiotics for future use was commonly reported. To curb nonprescription antibiotic use, all facets of the leftover antibiotic use continuum, from overprescribing to hoarding, need to be addressed. 

A survey of patient practices regarding leftover antibiotics reveals a potential source of antibiotic overuse

BACKGROUND
Use of antibiotics without a prescription is potentially unsafe and may increase the risk of antimicrobial resistance. We evaluated the effect of patient, health system, and clinical encounter factors on intention to use antibiotics without a prescription: (1) purchased in the United States (U.S.), (2) obtained from friends or relatives, (3) purchased abroad, or 4) from any of these sources.


METHODS
Survey was performed January 2020 - June 2021 in six publicly-funded primary care clinics and two private emergency departments in Texas, U.S. Participants included adult patients visiting one of the clinical settings. Non-prescription use was defined as use of antibiotics without a prescription, and intended use was professed intention for future non-prescription antibiotic use.


RESULTS
Of 564 survey respondents (33% Black and 47% Hispanic or Latino), 246 (43.6%) reported prior use of antibiotics without a prescription, and 177 (31.4%) reported intent to use antibiotics without a prescription. If feeling sick, respondents endorsed that they would take antibiotics: obtained from friends/relatives (22.3% of 564), purchased in the U.S. without a prescription (19.1%), or purchased abroad without a prescription (17.9%). Younger age, lack of health insurance and a perceived high cost of doctor visits were predictors of intended use of non-prescription antibiotics from any of the source. Other predictors of intended use were lack of transportation for medical appointments, language barrier to medical care, Hispanic or Latino ethnicity and being interviewed in Spanish.


CONCLUSIONS
Patients without health insurance who report a financial barrier to care are likely to pursue more dangerous non-prescription antimicrobials. This is a harm of the fragmented, expensive healthcare system that may drive increasing antimicrobial resistance and patient harm.

 The use of antibiotics obtained without a prescription (non-prescription use) in the United States (U.S.) may contribute to antimicrobial resistance. We hypothesized that among Hispanic/Latino patients, the intention to use non-prescription antibiotics would be associated with: [1] lower acculturation (preference to socialize with Hispanic/Latinos) and [2] having friends or relatives who use antibiotics without contacting a doctor (social norms).
 
 
 
 We conducted a survey on the factors that impact Hispanic/Latino patients’ decisions to use non-prescription antibiotics in safety-net primary care clinics and private emergency departments in the greater Houston, Texas area between January 2020 and June 2021. Surveys were read to patients in English or Spanish and queried patient, health system, and clinical encounter factors.
 
 
 
 Of 263 Hispanic/Latino patients, 59% completed the survey in Spanish and 41% in English (Table 1). Overall, 47% reported previous non-prescription antibiotic use, and 36% professed intention to hypothetically obtain and use non-prescription antibiotics, if feeling sick, from one or more of three sources (bought without a prescription in the U. S., from another country, and from friends or relatives).
 Lack of health insurance, being surveyed in Spanish, having friends or relatives who use antibiotics without consulting a doctor, and high cost of doctor visits were predictors of intent to use antibiotics bought without a prescription in the U.S. (Table 2). Predictors of intent to use non-prescription antibiotics from other countries included lower acculturation scores (preferring to socialize with Hispanics/Latinos), younger age, having friends or relatives who use antibiotics without consulting a doctor, and reported language barriers to medical appointments. Predictors of intent to use antibiotics from friends and relatives included having friends or relatives who use antibiotics without consulting a doctor, and high cost of doctor visits.
 
 
 
 Interventions aimed at reducing non-prescription use in the Hispanic/Latino community should focus on ensuring affordable language concordant healthcare and promoting antibiotic stewardship as a social norm.

1770. Effects of Acculturation and Social Norms on Intention to use Antibiotics without a Prescription among Hispanic or Latino individuals

Background: Antibiotic use without a prescription (nonprescription use) leads to antibiotic overuse, with negative consequences for patient and public health. We studied whether screening patients for prior nonprescription antibiotic use in the past 12 months predicted their intentions to use them in the future. Methods: A survey asking respondents about prior and intended nonprescription antibiotic use was performed between January 2020 and June 2021 among patients in waiting rooms of 6 public clinics and 2 private emergency departments in economically and socially diverse urban and suburban areas. Respondents were classified as prior nonprescription users if they reported previously taking oral antibiotics without contacting a doctor, dentist, or nurse. Intended use was defined as answering “yes” or “maybe” to the question, “Would you use antibiotics without contacting a doctor, nurse, or dentist?” We calculated the sensitivity, specificity, and positive and negative predictive value (PPV and NPV) of prior nonprescription antibiotic use in the past 12 months for future intended nonprescription use. Bayes PPV and NPV were also calculated, considering the prevalence of nonprescription antibiotic use (24.8%) in our study. Results: Of the 564 patients surveyed, the median age was 51 years (SD, 19–92), with 72% of patients identifying as female. Most were from the public healthcare system (72.5%). Most respondents identified as Hispanic or Latino(a) (47%) or African American (33%), and 57% received Medicaid or the county financial assistance program. Prior nonprescription use was reported by 246 (43%) of 564 individuals, with 91 (16%) reporting nonprescription use within the previous 12 months. Intention to use nonprescription antibiotics was reported by 140 participants (25%). The sensitivity and specificity of prior nonprescription use in the past 12 months to predict the intention to use nonprescription antibiotics in the future were 75.9% (95% CI, 65.3–84.6) and 91.4% (95% CI, 87.8–94.2), respectively. After the Bayes’ adjustment, the PPV and NPV of prior use to predict future intention were 74.5% (95% CI, 66.7–80.9) and 92.0% (95% CI, 88.7–94.4) (Table 1). Conclusions: These results show that prior nonprescription antibiotic use in the past 12 months predicted the intention to use nonprescription antibiotics in the future (PPV of 75%). As a stewardship effort, we suggest clinicians use a simple question about prior nonprescription antibiotic use in primary-care settings as a screening question for patients at high risk for future nonprescription antibiotic use.
