Ogilvie's syndrome is a rare and potentially fatal disease that can easily be mistaken for postoperative ileus. Also known as acute colonic pseudo-obstruction, early recognition and diagnosis of the syndrome allows for treatment prior to bowel perforation and requisite abdominal surgery. The authors report a case of Ogilvie's syndrome following spinal deformity correction and tethered cord release in an adolescent who presented with acute abdominal distension, nausea, and vomiting on postoperative Day 0. The patient was initially diagnosed with adynamic ileus and treated conservatively with bowel rest, reduction in narcotic dosage, and a regimen of stool softeners, laxatives, and enemas. Despite this treatment, her clinical course failed to improve, and she demonstrated significant colonic distension radiographically. Intravenous neostigmine was administered as a bolus with a rapid and dramatic response. This case is the first reported instance of neostigmine use for Ogilvie's syndrome treatment following a pediatric neurosurgical operation.

. 12.00 Background: Based on the dramatic response of acute colonic pseudo-obstruction to neostigmine, we designed this study to evaluate the effect of neostigmine on the tolerance of enteral feeding in Intensive Care Unit (ICU) patients. Materials and Methods: A total of 60 patients hospitalized in the ICU of Alzahra Hospital, Isfahan, Iran entered the study. They were randomly assigned to one of the two groups of case (who received intravenous neostigmine infusion) and control (normal saline). They were compared with respect to incidence of constipation, diarrhea, and vomiting. Arrhythmia, bronchospasm, mean arterial blood pressure (MAP), and heart rate (HR) were also evaluated at 0, 4, 8, 12, 16, 20, and 24 h. Results: The frequency distribution of constipation, diarrhea, vomiting, and increase in gastric lavage volume in the intervention group was 20%, 33.3%, 46.7% and 43.3%, while these indices in the control group were 40%, 30%, 43.3%, and 63.3% , respectively ( P > 0.05). Arrhythmia was observed in 3% and 6% in the case and control groups, respectively. Bronchospasm was not detected in any of patients. Conclusion: There was no significance difference between neostigmine and normal saline with respect to tolerance of enteral nutrition in ICU patients. We report a case of clozapine induced acute intestinal pseudo obstruction (also known as Ogilvie’s syndrome) in a 54-year female with a history significant for multiple psychiatric disorders. The diagnosis was made after a challenging clinical presentation where collaboration between multiple medical services resulted in reaching a unified diagnosis. The diagnosis was established based on multiple radiological imaging modalities and confirmed by a documented response to colonic decompression through colonoscopy and discontinuation of the offending medication. Clozapine Induced Ogilvie’s Syndrome

Celiac disease is a common disorder not only in the young but also in the elderly. Different complications may be observed in elderly patients who have had celiac disease for a long time. We report a case of Ogilvie's syndrome (acute colonic pseudo-obstruction (ACPO)) seen as a complication of celiac disease in an elderly patient. A 69-year-old woman presented with a 1-week history of gradually increasing abdominal distension and obstipation. She had been diagnosed as celiac disease approximately 35 years ago, and had been on a gluten-free diet since then. Her serum calcium was 6.7 mg/dL. She was treated with hydration and calcium repletion. We think that her long duration of celiac disease which is approximately 35 years and the intermittent diet inconsistencies due to this long duration may have contributed to the development of ACPO. Therefore, in elderly patients who have had celiac disease for a long time, ACPO may develop with an electrolyte imbalance. An early diagnosis of ACPO will minimize the rates of mortality Ozet
