The pharmacology of dobutamine.

Dobutamine* is a new, synthetic, intravenously administered catecholamine that acts directly to increase myocardial contractility without inducing marked tachycardia or greatly changing peripheral arterial resistance.1 These features make it useful for treating acute cardiac failure characterized by low cardiac output and elevated diastolic filling pressures, and in July, 1978, it was approved for clinical use in this country. Norepinephrine is the endogenous catecholamine that is synthesized and stored in granules in adrenergic nerve endings in the myocardium. When sympathetic nerves to the heart are activated, norepinephrine is released from its stores and stimulates specific sites on the myocardial cell surface, . . .

Dobutamine: a new synthetic cardioactive sympathetic amine.

Dobutamine is a synthetic catecholamine developed as a relatively selective positive inotropic drug for short-term parenteral administration. Dobutamine's effects are mediated by strong beta 1 adrenergic receptor stimulation and mild stimulation of beta 2 and alpha 1 receptors. Dobutamine should be used to improve ventricular function and cardiac performance in patients in whom ventricular dysfunction has caused a reduced stroke volume and cardiac output, a mild to moderate drop in systemic blood pressure, diminished organ and tissue perfusion, and elevated ventricular filling pressures. When guidelines for patient selection and dosing are adhered to, ventricular dysfunction and cardiac decompensation secondary to atherosclerotic occlusive coronary artery disease can be improved without adversely affecting the myocardial oxygen supply and demand balance. Dobutamine has less vasopressor activity than norepinephrine and dopamine, and should not be the primary treatment in conditions characterized by marked hypotension and shock.

Dobutamine is a newly developed catecholamine reported to have minimal direct vascular effects relative to its inotropic activity and to have less chronotropic and arrhythmogenic properties than other catecholamines used in the treatment of low output states. In this study, the acute hemodynamic effects of dobutamine were compared to those of dopamine in 13 patients with chronic low output cardiac failure. At dosages adjusted to achieve similar increments in cardiac output, dobutamine reduced left ventricular filling pressure (LVEP) from 24 +/- 2 mm Hg (SEM) to 17+/- 2 mm Hg, while dopamine increased LVEP to 30 +/- 3 mm Hg and in six patients caused arterial O2 saturation to fall below 90%. This poor response to dopamine was probably the result of its vasoconstrictive effects and illustrates the potential advantages of using a cardioselective agent such as dobutamine when the desired goal of therapy is to improve ventricular function by direct inotropic stimulation.
