Title. Nurse-led self-management group programme for patients with congestive heart failure: randomized controlled trial. Aim. This paper reports on of the effects of the Chronic Disease Self-Management Programme on psychosocial attributes, self-care behaviour and quality of life among congestive heart failure patients who experienced slight to marked limitation of physical activity. Background. Most self-management programmes for congestive heart failure patients emphasize the medical aspects of this chronic condition, without incorporating psychosocial aspects of self-management. The programme has been used with various patient groups, but its effectiveness with congestive heart failure patients when led by pairs of cardiac nurse specialists and peer leaders is unknown. Method. A randomized controlled trial with 12 months of follow-up from start of the programme was conducted with 317 patients. Control group patients (n = 131) received usual care, consisting of regular outpatient checkups. Intervention group patients (n = 186) received usual care and participated in the six-week self-management programme. The programme teaches patients medical, social and emotional self-management skills. Twenty-one classes were conducted in six hospitals in the Netherlands, and data were collected between August 2004 and January 2007. Results. Directly after the programme, statistically significant effects were found for cognitive symptom management (P < 0AE001), self-care behaviour (P =0 AE008) and cardiacspecific quality of life (P =0 AE005). No effects were found at 6- and 12-month follow-up. Conclusion. Further research is necessary to study how long-term effectiveness of the programme with patients with congestive heart failure can be achieved, and how successful adaptations of the programme can be integrated into standard care.

Nurse‐led self‐management group programme for patients with congestive heart failure: randomized controlled trial

Streptomyces strain KX852460 having antifungal activity against Rhizoctonia solani AG-3 KX852461 that is the causal agent of target spot disease in tobacco leaf. The aim of the study was to determine the antifungal activity of Streptomyces strain KX852460 extract against R. solani AG-3 and to identify bioactive antifungal compounds produced by strain KX852460. Crude substance was produced by submerged fermentation process from Streptomyces strain KX852460. Various solvent was used to extract the culture filtrate. Among all, ethyl acetate extracted supernatant showed great potency against R. solani AG-3 KX852461. The active fractions were purified by silica gel column chromatography having 52 mm zone of inhibition against R. solani AG-3 KX852461. The purified fractions were identified by gas chromatography–mass spectrometry technique. Twenty-seven compounds were identified and most of the compounds were the derivatives of aromatic compounds. Eicosane (C20H42) and dibutyl phthalate (C16H22O4) were found antifungal compounds in this study. While morphinan, 7,8-didehydro-4,5-epoxy-17-methyl-3,6-bis[(trimethylsilyl)oxy]-, (5.Alpha. 6.Alpha)—(C23H35NO3Si2), cyclononasiloxane, octadecamethyl—(C18H54O9Si9) and benzoic acid, 2,5-bis(trimethylsiloxy) (C16H30O4Si3) were the major compounds with highest peak number. These results suggested that Streptomyces strain KX852460 had good general antifungal activity and might have potential biocontrol antagonist against R. solani AG-3 KX852461 to cure the target spot in tobacco leaf.

3 Abstract: This work was carried out in the course of a screening program for specific the bioactive substances that demonstrated inhibitory affects against prokaryotic and eukaryotic microorganisms from actinomycetes strains..Five actinomycete isolates could be isolated from water sample collected from River Nile, Egypt were screened for antimicrobial activities in starch nitrate medium. One of the actinomycete cultures (AZ-NIOFD ) 1 was found to produce a wide spectrum antimicrobial agent. The most potent of the producer strains was selected and identified. The cultural, physiological and biochemical characteristics of the isolate identified the strain as a member of the genus Streptomyces. The nucleotide sequence of the 16S rRNA gene (1.5 kb) of the most potent strain evidenced a 98% similarity with S. tritolerans and a 96% similarity with S. plicatus 16 rRNA genes and the isolated strain was ultimately identified as Streptomyces sp. MAR01. The extraction S of the fermentation broth of this strain resulted in the isolation of one major compound, which was active in vitro against Gram-positive, Gram-negative bacteria and unicellular and filamentous fungi. The physico-chemical characteristics of the purified antibiotic viz. color, melting point, solubility, elemental analysis, spectroscopic characteristics and chemical reactions have been investigated. Biochemical studies on antibiotic production from Streptomyces sp.: Taxonomy, fermentation, isolation and biological properties

This work was carried out in the course of a screening program for specific the bioactive substances that demonstrated inhibitory affects against pathogenic fungi from actinomycetes strains. Forty five Streptomyces strains were isolated from soil sample collected from El-Sharkia governorate, Egypt. All these strains were screened for their antifungal activity against pathogenic fungi. Among the forty five Streptomyces, the broad spectrum strain Streptomyces sp. AZ-SH514 was selected for compound characterization and purification. The nucleotide sequence of the 16s RNA gene (1.5 Kb) of the most potent strain evidenced an 89% similarity with Streptomyces mutabilis, Streptomyces tendae and Streptomyces olivaceiscleroticus. From the taxonomic features, the actinomycetes isolate AZ-SH514 matches with Streptomyces olivaceiscleroticus in the morphological, physiological and biochemical characters. Thus, it was given the suggested name Streptomyces olivaceiscleroticus, AZ-SH514. The active metabolite was extracted using n-Butanol (1:1, v/v) at pH 7.0. The separation of the active ingredient and its purification was performed using both thin layer chromatography (TLC) and column chromatography (CC) techniques. The physico-chemical characteristics of the purified antifungal agent viz. color, melting point, solubility, elemental analysis, spectroscopic characteristics and chemical reactions have been investigated. This analysis indicates a suggested empirical formula of C H NO. 24 19 The minimum inhibition concentrations "MICs" of the purified antifungal agent were also determined. In conclusion, the collected data emphasized the fact that the purified antifungal agent was suggestive of being belonging to 4' phenyl-1-napthyl-phenyl acetamide antibiotic produced by Streptomyces olivaceiscleroticus, AZ-SH514.

This work was carried out in the course of a screening program for specification of the bioactive substances that demonstrated inhibitory affects against bacterial pathogens and application of oral administration of probiotic Lactobacillus plantrum NRRL B-227 in vivo using mices. Forty lactobacillus sp. selected from American agriculture culture collection. All these strains were screened for their antibacterial activity against pathogenic bacteria. Among the forty Lactobacillus, the broad spectrum strain Lactobacillus sp NRRL B-227 was active against bacterial pathogens, Gram positive bacteria viz Staphylococcus aureus ATCC 29213, Micrococcus luteus ATCC 9341, Bacillus subtilis NRRL B-543, Bacillus subtilis NCTC 10400, Bacillus pumilus NCTC 8214 and Gram negative bacteria viz Escherichia coli ATCC 25922, Escherichia coli NCTC 10416, Salmonella typhi NCIMB 9331, Enterobacter cloacae ATCC 13047, Pseudomonas aeruginosa ATCC 10145, Klebsiella pneumonia NCTC 9111, Klebsiella pneumonia ATCC 13883. From the taxonomic features, the Lactobacillus sp NRRL B-227 matches with Lactobacillus plantarum in the morphological, physiological and biochemical characters. Thus, it was given the suggested name Lactobacillus plantarum NRRL B-227. The nucleotide sequence of the 16S RNA gene of the most potent strain evidenced a 100% similarity with Lactobacillus plantarum. The active metabolite was precipitation using ammonium sulfate and its purification was performed using reverse-phase high performance liquid chromatography (RP-HPLC) on ODS Hypersil C18 column. The physico-chemical characteristics of the purified peptide antibiotic viz. heat & pH resistance and spectroscopic characteristics (UV, IR, Mass spectrum and amino acid analyzer) have been investigated. In conclusion, the collected data emphasized the fact that the purified peptide antibiotic may be belonging to bacteriocin antibiotic produced by Lactobacillus plantarum NRRL B-227. The application of oral administration of probiotic Lactobacillus plantrum NRRL B-227 in vivo using mice's had been investigated.
