Person-Centered Rehabilitation Model: Framing the Concept and Practice of Person-Centered Adult Physical Rehabilitation Based on a Scoping Review and Thematic Analysis of the Literature

Purpose: Boredom may impede engagement in inpatient rehabilitation following an acquired brain injury. This review aimed to: (1) describe the experience and (2) quantify the incidence of boredom; (3) identify measurement tools used to quantify boredom; (4) summarize factors contributing to boredom, and (5) outline evidence-based interventions shown to reduce boredom during inpatient rehabilitation.Methods: Two researchers independently screened publications retrieved from electronic database searches. Publications presenting patient, carer or staff data relating to boredom in inpatients with acquired brain injuries were included.Results: Two thousand four hundred and ninety-nine references were retrieved, 88 full texts were reviewed, with 24 studies included. Purpose: To develop and examine the psychometric properties of the Daily Living Self-Efficacy Scale (DLSES) designed to assess stroke survivors’ self-efficacy in daily functioning. Method: Two groups of participants (N = 424) were recruited, a stroke survivor group (n = 259) who were recruited through two stroke associations in Australia and a non-stroke group (n = 165) who were the partners/carers of the stroke survivors (n = 93) and members of the community in Queensland, Australia (n = 72). Principal Component Analyses (PCA) were used to assess the factor structure of the scale and investigations of internal consistency, test--retest reliability, convergent and discriminant validity were conducted. Results: The final measure is a 12-item scale comprising two subscales: self-efficacy for psychosocial functioning and self-efficacy for activities of daily living. The daily living self-efficacy scale: a new measure for assessing self-efficacy in stroke survivors.

Objectives: Little research has been conducted that investigates the beneﬁts of reading for people with neurological conditions despite its age old use to improve well-being. The aim of this study was to identify and review the evidence of the effect of ‘lone’ reading, reading aloud and shared reading groups on the health and well-being of people with neurological conditions in clinical and long-term care settings. Methods: A literature search was conducted incorporating a systematic search of electronic databases, internet searching, ‘snowballing’ technique from references of relevant studies and consultation with clinicians and academics in the ﬁeld. Results: Twelve studies (ﬁve quantitative, three qualitative and four mixed methods) met the criteria for inclusion in the review. No randomised controlled trials were identiﬁed. Signiﬁcant heterogeneity in the results of the quantitative studies precluded statistical data synthesis. Thematic analysis and synthesis was applied to the three qualitative studies and the qualitative data of the mixed-method studies. All but one of the quantitative studies reported that the reading interventions had a positive effect. The evidence from the qualitative studies demonstrated multiple positive effects of shared reading groups. Conclusions: The effect of ‘lone’ reading, reading aloud and shared reading groups on the health and well-being of people with neurological conditions is currently an under-researched area. Although this review reports encouraging results of positive effects, the results should be viewed with caution due to the lack of randomisation, the small numbers of participants involved, and the limited and heterogeneous evidence base.

The role of reading on the health and well-being of people with neurological conditions: a systematic review.

Purpose: To explore stroke survivors’ and artists’ beliefs about participatory visual arts programme participation during in-patient rehabilitation to identify benefits and potential mechanisms of action. Method: Qualitative design using semi-structured in-depth interviews with stroke survivors (n = 11) and artists (n = 3). Analysis: Data were audio-recorded and transcribed. Framework approach was used to identify themes and develop conceptual schemes. Results: The non-medical, social context of art facilitated social interaction, provided enjoyment and distraction from stroke and re-established social identity thereby improving mood. The processes of art making generated confidence and self-efficacy, setting and achievement of creative, communication and physical recovery goals that provided control over survivors’ situation and hope for recovery. Social context, art making processes and creative output: a qualitative study exploring how psychosocial benefits of art participation during stroke rehabilitation occur.

Purpose: The importance of addressing mental well-being while stroke patients are in hospital has been recognized by policy makers but there is a gap between rhetoric and reality. This study explored the potential for using a person-centred, artist facilitated, one-to-one arts programme to improve the emotional and mental well-being of patients, which may be adversely affected as a result of the negative experiences arising from stroke and hospitalization. Method: The study focused on those patients remaining in hospital for above average durations and included patients with functional, cognitive and speech or language impairments. After participation in the arts programme and before discharge, semi-structured interviews were undertaken with sixteen patients, and in the case of two patients with severe cognitive loss, a relative, as a proxy, was interviewed. Interviews explored the experience of stroke and hospital stay and the meaning and value of taking part in Time Being Stroke and the data was analysed thematically. To underpin and contextualize the interview material, clinical status and treatment information routinely collected by the multi-disciplinary stroke team was obtained and analysed; in addition material on the content of each session was recorded by artists and analysed. Results: Patients’ accounts suggest that participation in a personcentred arts programme contributed to their mental well-being. As might be expected from a person-centred intervention, benefits varied across the sample, but the most commonly mentioned positive aspects of participation included the experiences of: pleasure and enjoyment, a sense of connection with the artists, mental stimulation, learning and creativity, engagement in purposeful occupation and relief from boredom, and reconnection with valued aspects of the self.
