Treating the Trauma of Rape: Cognitive-Behavioral Therapy for PTSD

Research concerning the treatment of sexual offenders has generally focused on the treatment of adult offenders. The effectiveness of treatments for male adolescent sexual offenders (N = 644) was assessed in a meta-analysis of 10 studies. Overall, the results were surprisingly encouraging, suggesting that treatments for male adolescent sexual offenders appear generally effective (r = .37). Studies which used self-report measures of outcome obtained a 6% higher effect size than studies which used measures of arousal in response to deviant stimuli, and a 22% higher effect size than studies using actual recidivism rates. A descriptive review of the set of 10 studies indicates that studies utilizing cognitive-behavioral therapy approaches were the most effective.

Treatment Effectiveness for Male Adolescent Sexual Offenders: A Meta-Analysis and Review

This article discusses the importance of a systems approach to working with adolescents who have offended sexually. Literature concerning family backgrounds of these youths is reviewed, highlighting histories of family violence, parental depression and child maltreatment, problematic communication patterns, parental psychological inaccessibility, inconsistent discipline, and failure of the family to acknowledge the youth's offending behavior. Such issues and other special challenges presented by parents of these adolescents, particularly as the issues impact caregiver support and supervision, can be significant barriers to a youth's successful treatment. However, when these barriers are addressed through a systems approach that actively involves parents in assessment and therapeutic phases of intervention, treatment of an adolescent who sexually offended is likely to be enhanced. The importance of offering parents multiple intervention strategies at various stages in the therapeutic process is emphasized; interventions might include individual, family, and multi-family group therapy, and psychoeducation and support groups.

Juvenile sexual offending is increasingly being recognized as a serious crime among youth. The prevalence of sexual offending and sexual reoffending suggests that many juvenile sex offenders (JSOs) may repeat their offending behaviors if not treated. However, clinical trials evaluating specific interventions are virtually nonexistent. Instead, the literature on the treatment of JSOs is marked by discussions of strategies that are hypothesized to be beneficial, as well as descriptions of treatment programs that exist across the country. Further, while existing literature suggests that treatment for JSOs may deter future sexual offending behaviors, it is unclear which, if any, aspects of these treatments promote the development of positive behaviors. A discussion of existing treatment approaches, effectiveness, and treatment considerations follows.

SUMMARY This article describes an outpatient treatment program for adolescent sexual abusers that was established by a mental health agency in collaboration with a specialized probation program in the juvenile court. Individualized treatment is based on a comprehensive clinical assessment with the youth and guardian, for which examples are provided. Given the heterogeneity of this population, we describe several treatment strategies directed to various individual or family clinical targets, including psychiatric disorders, sexual deviance and sexuality, normal adolescent development and adaptive skills, and parent and family relationships. Ongoing collaborative and coordination issues are also reviewed. The integration of mental health and probationary services provides a balanced approach to the community management and treatment of the low-risk, primarily first-time, adolescent sexual offender.
