Genetic research is advancing rapidly. One important area for the application of the results from this work is personalized health. These are treatments and preventive interventions tailored to the genetic profile of specific groups or individuals. The inclusion of personalized health in existing health systems is a challenge for policymakers. In this article, we present the results of a thematic scoping review of the literature dealing with governance and policy of personalized health. Our analysis points to four governance challenges that decisionmakers face against the background of personalized health. First, researchers have highlighted the need to further extend and harmonize existing research infrastructures in order to combine different types of genetic data. Second, decisionmakers face the challenge to create trust in personalized health applications, such as genetic tests. Third, scholars have pointed to the importance of the regulation of data production and sharing to avoid discrimination of disadvantaged groups and to facilitate collaboration. Fourth, researchers have discussed the challenge to integrate personalized health into regulatory-, financing-, and service provision structures of existing health systems. Our findings summarize existing research and help to guide further policymaking and research in the field of personalized health governance.

No panorama atual as doenças crônicas não transmissíveis têm se destacado como responsáveis pelo aumento da morbimortalidade mundial, com relevância para as doenças cardiovasculares. Identificar na literatura científica tecnologias educacionais utilizadas no processo de educação em saúde direcionadas aos pacientes portadores de doenças cardiovasculares que contribuem para impactar na qualidade de vida. Revisão integrativa da literatura utilizando a questão de pesquisa e a combinação mnemônica PICo. Foram selecionados 16 artigos.  O recorte temporal foi de cinco anos. Se observou abordagens multidisciplinares para o gerenciamento de práticas educativas e preventivas nos grandes polos mundiais para doenças cardiovasculares, que podem também serem aproveitadas para as outras doenças crônicas não transmissíveis. As estratégias educativas são observáveis em diversas intervenções dentro das unidades de saúde. É necessário se atentar para os critérios na construção e desenvolvimento de materiais educacionais na área de saúde.

Educational technologies in the care of patients with cardiovascular diseases 
Abstract: In the current scenario, non-communicable chronic diseases have been highlighted as responsible for the increase in morbidity and mortality worldwide, with relevance to cardiovascular diseases. To identify, in the scientific literature, educational technologies used in the health education process aimed at patients with cardiovascular diseases that contribute to an impact on quality of life. Integrative literature review using the research question and the mnemonic combination PICo. 16 articles were selected. The time frame was five years. Multidisciplinary approaches were observed for the management of educational and preventive practices in the world's major centers for cardiovascular diseases, which can also be used for other non-communicable chronic diseases. Educational strategies are observable in several interventions within health units. It is necessary to pay attention to the criteria in the construction and development of educational materials in the health area.

Resumen: En el escenario actual, las enfermedades crónicas no transmisibles se han destacado como responsables del aumento de la morbilidad y mortalidad a nivel mundial, con relevancia para las enfermedades cardiovasculares. Identificar, en la literatura científica, tecnologías educativas utilizadas en el proceso de educación en salud dirigido a pacientes con enfermedades cardiovasculares que contribuyan a un impacto en la calidad de vida. Revisión integrativa de la literatura utilizando la pregunta de investigación y la combinación mnemotécnica PICo. Se seleccionaron 16 artículos. El plazo era de cinco años. Se observaron enfoques multidisciplinarios para la gestión de prácticas educativas y preventivas en los principales centros de enfermedades cardiovasculares del mundo, que también pueden ser utilizados para otras enfermedades crónicas no transmisibles. Las estrategias educativas son observables en varias intervenciones dentro de las unidades de salud. Es necesario prestar atención a los criterios en la construcción y desarrollo de materiales educativos en el área de la salud.

Tecnologias educacionais no cuidado à pacientes com doenças cardiovasculares

Sufficient exercise and sleep, a balanced diet, moderate alcohol consumption and a good approach to handle stress have been known as lifestyles that protect health and longevity since the Middle Age. This traditional prevention quintet, turned into a sextet by smoking cessation, has been the basis of the "preventive personality" that formed in the twentieth century. Recent analyses of big data sets including genomic and physiological measurements have unleashed novel opportunities to estimate individual health risks with unprecedented accuracy, allowing to target preventive interventions to persons at high risk and at the same time to spare those in whom preventive measures may not be needed or even be harmful. To fully grasp these opportunities for modern preventive medicine, the established healthy life styles require supplementation by stratified prevention. The opportunities of these developments for life and health contrast with justified concerns: A "surveillance society", able to predict individual behaviour based on big data, threatens individual freedom and jeopardises equality. Social insurance law and the new German Disease Prevention Act (Praventionsgesetz) rightly stress the need for research to underpin stratified prevention which is accessible to all, ethical, effective, and evidence based. An ethical and acceptable development of stratified prevention needs to start with autonomous individuals who control and understand all information pertaining to their health. This creates a mandate for lifelong health education, enabled in an individualised form by digital technology. Stratified prevention furthermore requires the evidence-based development of a new taxonomy of cardiovascular diseases that reflects disease mechanisms. Such interdisciplinary research needs broad support from society and a better use of biosamples and data sets within an updated research governance framework.
