Risk indicators for peri‐implantitis. A narrative review

Publications from 2011 to 2015 were selected to evaluate effect of implant surface roughness on long-term bone loss as surrogate for peri-implantitis risk. 87 out of 2,566 papers reported the mean bone loss after at least 5 years of function. Estimation of the proportion of implants with bone loss above 1, 2, and 3 mm as well as analysis the effect of implant surface roughness, smoking, and history of periodontitis was performed. By means of the provided statistical information of bone loss (mean and standard deviation) the prevalence of implants with bone loss ranging from 1 to 3 mm was estimated. The bone loss was used as a surrogate parameter for "peri-implantitis" given the fact that "peri-implantitis" prevalence was not reported in most studies or when reported, the diagnostic criteria were unclear or of dubious quality. The outcome of this review suggests that peri-implant bone loss around minimally rough implant systems was statistically significant less in comparison to the moderately rough and rough implant systems. No statistically significant difference was observed between moderately rough and rough implant systems. The studies that compared implants with comparable design and different surface roughness, showed less average peri-implant bone loss around the less rough surfaces in the meta-analysis. However, due to the heterogeneity of the papers and the multifactorial cause for bone loss, the impact of surface roughness alone seems rather limited and of minimal clinical importance. Irrespective of surface topography or implant brand, the average weighted implant survival rate was 97.3% after 5 years or more of loading. If considering 3 mm bone loss after at least 5 years to represent the presence of "peri-implantitis," less than 5% of the implants were affected. The meta-analysis indicated that periodontal history and smoking habits yielded more bone loss.

Peri-implant Diseases: A Review of Treatment Interventions

Long-term successful treatment of chronic periodontitis requires placement of patients on post-treatment recall programs known as either periodontal maintenance therapy or supportive periodontal therapy. Selection of the recall intervals must be based on the specific needs of individual patients. A single recall interval (e.g. 6 months) is not suitable for all patients. The main purpose of these programs is to prevent the recurrence of periodontitis. The components of every periodontal maintenance therapy program include: review of medical/dental histories; complete oral examination with an emphasis on the detection of gingival inflammation; establishing whether the maintenance program is working by monitoring clinical attachment levels; evaluation of oral hygiene; and full-mouth supragingival and subgingival debridement (i.e. biofilm removal). Long-term post-insertion care for dental implants also requires a similar patient-specific recall program of supportive implant therapy. The main purposes of a supportive implant therapy program are to maintain a healthy peri-implant mucosa and thereby prevent the development of peri-implantitis. In cases in which plaque-induced peri-implant mucositis has occurred, a well-designed supportive implant therapy program can help return the mucosa to a healthy state. At the current time there is no consensus on the optimal interventions for the treatment of peri-implant mucositis. However, all effective supportive implant therapy programs emphasize meticulous oral hygiene practices, careful peri-implant examination, thoughtful analysis of risk factors and periodic removal of microbial deposits from the implants.

Aim
To evaluate long-term clinical and radiographic parameters of osseointegrated implants in non-smoker patients with a previous history of chronic periodontitis.

Materials and Methods
Fifty-four screw-type implants with a moderately roughened surface and internal hexagonal implant–abutment connection were placed according to a two-phase protocol and 40 reference teeth were analysed at baseline, and after 5 and 10 years. Pocket probing depth (PPD), clinical attachment level (CAL) and bleeding on probing were analysed 6x/tooth in all teeth, implants and reference teeth. Radiographic peri-implant bone level was measured on the mesial and distal surfaces. The prevalence of peri-implantitis and the survival rate of the implants were assessed at the patient and implant levels. Data were analysed using descriptive statistics, Mann–Whitney U-test, and Wald Z-test, at α = 5%.

Results
In implants, the CAL at 5 years was 0.3 mm higher, and at 10 years 1.2 mm higher in comparison to baseline. The corresponding data for the reference teeth were 0 mm and 0.5 mm respectively. Multilevel testing showed statistical difference for PPD between implants and teeth over time. After 10 years, the mean mesial bone loss was 0.63 ± 0.26 mm, and the mean distal bone loss was 0.56 ± 0.25. The survival rates were 100% and 92.3% for the implants in the mandible and the implants in the maxilla respectively.

Conclusions
Screw-type implants with internal hexagon placed in patients with a previous history of periodontitis attending a regular maintenance programme demonstrated stable clinical and radiographic results after 5 and 10 years.
