Summary Background Negative-pressure wound therapy is a ubiquitous wound management resource The influence of NPWT on the bacterial bioburden of the subjacent wound remains unclear We sought to examine the evidence Datasources MEDLINE, Embase, PubMed, the Cochrane Database of Systematic Reviews and the Cochrane Controlled Trials Register were searched for articles quantitatively evaluating bacterial load under NPWT Results Twenty-four studies met the inclusion criteria including 4 randomised controlled trials, 8 clinical series and 12 experimental studies Twenty studies evaluated conventional NPWT, while 4 evaluated infiltration-based NPWT While 8 studies using conventional NPWT failed to demonstrate an observable effect on bacterial load, 7 studies reported that NPWT was inherently bacteriostatic and 5 others reported species selectivity with suppression of non-fermentative gram-negative bacilli (NFGNB), including Pseudomonas spp Simultaneously, there was some evidence of enhanced proliferation of gram-positive cocci where the niche was cleared of NFGNB Two of the 4 studies using infiltration-based NPWT also reported selectively impaired proliferation of Pseudomonas spp Conclusion The assumption that NPWT suppresses bacterial proliferation is oversimplified There is evidence that NPWT exhibits species selectivity, suppressing the proliferation of NFGNB However, this may depopulate the niche for exploitation by gram-positive cocci This, in turn, has implications for the use of NPWT where highly virulent strains of gram-positive cocci have been isolated and the duration of NPWT therapy and frequency of dressing changes

Does negative-pressure wound therapy influence subjacent bacterial growth? A systematic review

Background Large randomized controlled trials that evaluate the effects of negative-pressure wound therapy with instillation of a topical solution and dwell time (NPWTi-d) are lacking. There is a need to synthesize existing data across multiple studies to provide a more precise estimate of the clinical effects of NPWTi-d. Methods A systematic literature review and a meta-analysis of comparative studies were performed to determine the effects of NPWTi-d versus control therapy in the adjunctive management of complex wounds. Weighted standardized mean difference or odds ratios and 95% confidence intervals were calculated to pool study and control group results in each publication for analysis. Results Thirteen studies comprising 720 patients were included in the analysis. Significantly fewer surgical debridements were performed in NPWTi-d patients versus control patients (P = 0.01). Wounds in the NPWTi-d group were ready for closure faster than control wounds (P = 0.03). The odds of reducing bacterial count from baseline in the NPWTi-d group was 4.4 times greater than control group wounds (P = 0.003), and percent reduction of bacterial count in NPWTi-d wounds was evident in all studies that captured that endpoint. There was a significantly shorter length of therapy in NPWTi-d patients versus control patients (P = 0.03). Wounds in NPWTi-d group were 2.39 times more likely to close than control group wounds (P = 0.01). Length of hospital stay was not significantly reduced for NPWTi-d patients compared with that for control patients (P = 0.06). Conclusion Results of this meta-analysis show a positive effect with use of NPWTi-d in various wound types.

Effects of Negative-Pressure Wound Therapy With Instillation versus Standard of Care in Multiple Wound Types: Systematic Literature Review and Meta-Analysis.

BACKGROUND The aim of this study was to assess the effects of a new treatment strategy for envenomation that consists of multiple small incisions and negative-pressure wound therapy (NPWT) on injured limb swelling and systemic inflammatory reaction. MATERIAL AND METHODS This was a prospective randomized controlled trial on snakebite envenomation. The enrolled patients were randomly divided into 2 groups: an observation group and a control group. The traditional comprehensive treatment was administered in both groups, but the observation group also received combined treatment with multiple small incisions and NPWT. Reduction in limb swelling, mean admission duration, complication rate, and changes in the levels of relevant cytokines were recorded and compared between the 2 groups. RESULTS The mean duration of hospital stay was significantly lower in the observation group than in the control group (5.44±0.89 days vs. 7.71±1.70 days). The complication rate and IL-6 concentration were significantly lower in the observation group than in the control group. CONCLUSIONS Multiple small incisions combined with NPWT proved effective for controlling the release of inflammatory cytokines and accelerating the relief of systemic inflammatory reaction. As a consequence, the complication rate decreased. Therefore, our new treatment strategy is safe and effective.

Small Incisions Combined with Negative-Pressure Wound Therapy for Treatment of Protobothrops Mucrosquamatus Bite Envenomation: A New Treatment Strategy

Chronic venous insufficiency (CVI) is a common disorder worldwide. Related pathophysiological mechanisms reportedly involve venous pooling and reduced venous return, leading to heaviness, aching, itchiness, tiredness, varicosities, pigmentation, and even lower limb ulceration. Approaches adopted to manage CVI at various stages of clinical-etiology-anatomy-pathophysiology include compression therapy, pharmacological treatment, ultrasound treatment, surgery, electrical or wireless microcurrent stimulation, and pulsed electromagnetic treatment. Among these, polymer-based therapeutic compression materials and wound dressings play increasingly key roles in treating all stages of CVI because of their unique physical, mechanical, chemical, and biological functions. However, the characteristics, working mechanisms, and effectiveness of these CVI treatment materials are not comprehensively understood. The present systematic review examines the structures, properties, types, and applications of various polymer-based compression materials and wound dressings used in prophylaxis and treatment of CVI. Existing problems, limitations, and future trends of CVI treatment materials are also discussed. This review could contribute to the design and application of new functional polymer materials and dressings to enhance the efficiency of CVI treatments, thereby facilitating patients' self-care ability and long-term health improvement.

Therapeutic compression materials and wound dressings for chronic venous insufficiency: A comprehensive review.

Abstract Background Venomous snakebites induce tissue destruction and secondary infection; however, the optimal timing of surgical intervention for these complications remains unknown. This study assessed the clinical predictors of early surgical intervention in patients with snakebites. Methods This retrospective study included 63 patients (45 men and 18 women) with venomous snakebites. In addition to the snake species, the demographics, affected body parts, clinical characteristics, and ultrasound findings of the patients in the surgical (32 patients) and nonsurgical (31 patients) groups were analyzed and compared. Results A higher incidence of acute compartment syndrome, local ecchymosis, skin necrosis, bullae, blisters, and fever was found in the surgical group than in the nonsurgical group, and ultrasound findings of the absence of Doppler flow were more frequently noted in the surgical group than in the nonsurgical group. After adjustment using a multivariate logistic regression model, only advanced age, Naja atra bite, local ecchymosis, and bulla or blister formation remained significant factors for surgical intervention. Furthermore, comparison of the outcomes of patients who received early (≤ 24 h) and late (&gt; 24 h) surgical intervention revealed that the duration of continuous negative pressure wound therapy (6 vs. 15 days; P = 0.006), duration of hospital stay (13 vs. 26 days; P = 0.002), and duration of outpatient follow-up (15 vs. 36 days; P &lt; 0.001) were significantly lower in patients who received early surgical intervention. The final reconstructive surgery was simple among the patients who received surgical intervention within 24 h of being bitten ( P = 0 . 028). Conclusion In patients with snakebites, advanced age, high-risk clinical manifestations (e.g., local ecchymosis and bulla or blister formation), and Naja atra envenomation are predictors of surgical intervention within 24 h.
