Global Epidemiology of Injecting Drug Use and HIV Among People Who Inject Drugs: A Systematic Review

The aim of this study was to compare the drug-resistance patterns of Mycobacterium tuberculosis strains among pulmonary tuberculosis patients, according to their HIV serostatus, in Burkina Faso. Tuberculosis (TB) patients were classified in new and previously treated cases by using a structured questionnaire. Susceptibility testing to isoniazid, streptomycin, rifampicin and ethambutol was done by the proportion method. Association between HIV-serostatus and drug-resistant TB was assessed with χ2 tests, and the statistical significance was set to P (25.3%), (P=0.04). The average age of the patients was 37.24±12.76 years [11-75years]. Most of the patients infected with HIV were aged from 15 to 44 years and were females, (P=0.01). In the new cases of TB, the difference between HIV-positive and HIV-negative patients was not statistically significant neither for the MDR-TB (P=0.40), nor for the resistant-TB to any drug (P=0.26). However, the difference was significant for the resistance to isoniazid and streptomycin alone (P=0.04). Among the previously treated patients, although there was more MDR-TB and more resistance to any drug in HIV-negative patients than among HIV-positive patients, these differences also were not statistically significant (P=0.54 and P=0.63, respectively). This study found no significant difference between TB/HIV-negative and TB/HIV-positive patients according to the resistance patterns to anti-TB medications, excepted the resistance to isoniazid in new cases and to isoniazid and streptomycin in all patients took globally. These results encourage the collaboration between the programs against TB and HIV to prevent rapid diffusion of drug-resistant TB and high mortality in patients living with HIV/AIDS as recommended by the World Health Organization. HIV infection and mycobacterium tuberculosis drug-resistance among tuberculosis patients in Burkina Faso, West Africa

This purpose of this study was to determine prevalences and risk factors associated with infection by human immunodeficiency virus, hepatitis B Virus, syphilis and bacillary pulmonary tuberculosis in prisons in Burkina Faso. From February 20 to March 20, 2009, 300 prisoners over 18 years of age held in Ouagadougou were selected to take part in this descriptive and analytical cross-sectional study. Sociodemographic data, confinement information (number, motive and prison time), medical history, substance addiction (alcohol, tobacco, drug), and the other risk behaviors (sexual relations, type of partners, sharing of toiletries or razor) were compiled for each prisoner. Serological tests were performed to detect anti-HIV antibodies, Hbs antigen, and anti-treponema antibody. In prisoners presenting signs of tuberculosis, BAAR detection was performed by direct examination of sputum. Men represented 95% of the study population. Median age was of 30.1 ± 8.9 years (range, 18 and 63). The prevalences of HIV infection, Hbs antigen and positive syphilitic serology were 5%, 27.3% and 5.7% respectively. Four prisoners (1.3%) had bacillary pulmonary tuberculosis. Two prisoners reported homosexual intercourse and 44 reported drug abuse. Sharing of toiletries and razor blades was reported by 18.7% and 20% of the prisoners respectively. Immediate measures are needed reduce the spread of these infections in prisons in Africa.

Prévalences et facteurs associés aux infections à Virus de l'Immunodéficience Humaine et virus de l'hépatite B, à la syphilis et à la tuberculose pulmonaire bacillifère en milieu carcéral au Burkina Faso.

Contexte : Le recours aux soins des consommateurs de drogues injectables est tres limite au Senegal, ou le contexte social envers cette population est repressif et le contexte sanitaire limite. Le Plan Strategique National de Lutte contre le Sida 2011-2015 a inscrit les consommateurs de drogues injectables comme population cle dans ses interventions et a favorise la creation du Centre de Prise en charge Integree des Addictions a Dakar (CEPIAD) dans l’enceinte du CHNU de Fann a Dakar en decembre 2014. Objectif : L’objectif de l’etude est d’evaluer le dispositif de prise en charge des consommateurs de drogues injectables au CEPIAD a Dakar en 2015. Methodologie : Nous avons realise une etude descriptive transversale au CEPIAD, de mars a aout 2015. Des grilles d’evaluation ont ete utilisees pour mesurer la satisfaction des usagers, et evaluer le dispositif de reduction des risques et les services offerts. L’analyse des donnees a repose sur la technique des atouts et faiblesses. Resultats : Durant la periode d’etude, 180 consommateurs de drogues injectables dont 10 femmes ont accede au CEPIAD (soit 1/7e des consommateurs a Dakar). Les consommateurs ont alors pu avoir connaissance de leurs statuts serologiques VIH et VHC (75 % et 66 %), un entretien social (179) et des activites de convivialite (91). Les patients enquetes se disent satisfaits a 74 % des services du centre. En une annee sur le terrain, l’equipe mobile a pu approcher plus d’un tiers des 1324 consommateurs dakarois. Parmi les 512 consommateurs approches, les injecteurs ont recu en moyenne 187 seringues, quantite proche des 200 seringues recommandees par an et par injecteur par l’OMS. Conclusion : Combinant diverses approches et disposant d’un panel d’interventions permettant de reduire les risques lies a l’usage de drogues, le CEPIAD est le premier centre dedie en Afrique de l’Ouest. L’integration effective des services (tuberculose, VIH, VHC) et l’acces des femmes aux activites doivent etre developpes.
