Osteochondroma represents the most common bone tumor and is a developmental lesion rather than a true neoplasm. It constitutes 20%–50% of all benign bone tumors and 10%–15% of all bone tumors. Its radiologic features are often pathognomonic and identically reflect its pathologic appearance. Osteochondromas are composed of cortical and medullary bone with an overlying hyaline cartilage cap and must demonstrate continuity with the underlying parent bone cortex and medullary canal. Osteochondromas may be solitary or multiple, the latter being associated with the autosomal dominant syndrome, hereditary multiple exostoses (HME). Complications associated with osteochondromas are more frequent with HME and include deformity (cosmetic and osseous), fracture, vascular compromise, neurologic sequelae, overlying bursa formation, and malignant transformation. Imaging of osteochondroma: variants and complications with radiologic-pathologic correlation.

Heterotopic ossification is the formation of bone in the soft tissues. Soft tissue bone deposition may range from the minimal and inconsequential to massive and clinically significant. In some clinical settings it is a predictable finding with an unpredictable course and in other settings it may be diagnostically confounding. Heterotopic ossification may be encountered in clinically disparate disease processes and circumstances. We review the genetic, neurogenic, post-traumatic, post-surgical and “reactive” causes of heterotopic ossification and discuss some current concepts of its pathogenesis.

Heterotopic ossification: a review

We reviewed 845 cases of benign and 356 cases of malignant cartilaginous tumors from a total of 3067 primary bone tumors in our database. Benign cartilaginous lesions are unique because the epiphyseal plate has been implicated in the etiology of osteochondroma, enchondroma (single or multiple), periosteal chondromas and chondroblastoma. In the first part of this paper, we will review important clinical, radiologic and histologic features of intramedullary cartilaginous lesions in an attempt to support theories related to anatomic considerations and pathogenesis.

Noninvasive evaluation of the swollen extremity: experiences with 190 lymphoscintigraphic examinations.

In the mid-1500s, the techniques of vascular ligature and vascular suture were developed sporadically by several pioneers in this field. However, vascular surgery became realistic experimentally as a result of the work by Carrel and Guthrie in the early 1900s, in which they performed replantations and transplantations of several composite tissues and organs, including amputated limbs, kidneys, and others using experimental animals. In contrast, the development of heparin by Howell and Holt in 1918 accelerated the rate of these types of operations being performed with increasing success in humans. Since the first use of a monocular microscope for ear surgery by Nylen in 1921 and a binocular microscope by Holmgren in 1923, in addition to the timely developments of the Zeiss operating microscope, microsurgical instruments, and suture materials, microsurgery was born in several surgical disciplines in the ensuing 50-year period. The application of microvascular surgery and microneurosurgery in the fields of hand, plastic, and reconstructive surgery resulted in revolutionary advances in clinical replantation and transplantation of composite tissues and more allotransplantations.

The natural history of microsurgical lymphovenous anastomoses: an experimental study.

We report 35 cases of peripheral skeletal osteochondromatous tumefactions which are both histologically and radiologically distinctive. The ages of the patients ranged from 14 to 74 years, with no sex predilection. Symptoms were related to the tumefaction. None of the patients had antecedent physical trauma or radiation. All lesions occurred on proximal phalanges, metatarsals, or metacarpals. Primary treatment was usually excision, and the gross appearance was typically that of a small osteochondroma. Histologically, the lesion exhibited marked proliferative activity, irregular bony-cartilaginous interfaces, and enlarged, bizarre, and binucleate chondrocytes, mimicking chondrosarcoma. Radiologically, the proliferations lacked both central continuity of the tumor with the underlying osseous medulla and "flaring" of the adjacent cortices. In 18 cases, the lesions recurred after primary excision, and at least eight lesions recurred twice. In spite of the rate of recurrence and the disturbing histologic appearance of these proliferations, behavior as a malignant tumor is either very rare or nonexistent.
