The rationale of this works is to analyse relationship in field of poisoning and toxicology between pharmaceutical care approach in order to improve the global management of the system and improving clinical outcomes in high risk situations. Observing the roles played by clinical pharmacy in toxicology medical team we can have relevant improving in the management of the systems. Starting from the analysis of some relevant literature we submit to international organization a rethinking about the toxicological medical team organization with a stabile presence of clinical pharmacist. Poisoning therapy is a multidisciplinary bio- medical work and we have more benefit when clinical pharmacist is permanent member of toxicology team.

Today more than past we need to improve clinical outcomes in the management of infectious disease related to patient in critical conditions (septic shock, sepsis, MOF, invasive fungal infections, HIV, peritonitis, meningitis, endocarditis, osteomyelitis, pneumonia, severe UTI, proteases infections and other). The same way we can see the high efficacy of some new drugs in eradicate severe infectious (as CEPAT), saving cost for transplant. This condition needs today an active role and permanent presence of clinical pharmacist in medical team because these therapies highly involved in patient’s health. The objective of this work is to analyze the role of clinical pharmacist in infectious disease medical team observing some in our opinion relevant publication and the positive effect on clinical outcomes. Infectious disease needs today new management systems to contain the increase of antimicrobial resistance in an era of few new and innovative drug discovered by pharmaceutical industries. Clinical pharmacist can give to the physicians a great help in this field to improve clinical outcomes, reducing toxicity, containing costs but also to preserve the antimicrobial potential of the antimicrobials (as safe-life drugs). These are only some of the many reason to apply the clinical pharmacist competence in patient medical team.

Infectious Disease Pharmaceutical Care: The Role of the Clinical Pharmacists to Improve Clinical Outcomes 2017

The aim of this work was to ask more use of clinical pharmacists in medical team .The application of pharmaceutical care principle in practice settings can improve clinical outcomes, reducing therapy errors and containment cost. An opportunity to use pharmacist’s expertise in assisting physician’s drug and medical devices specialists. Isn’t time for the medical community to get aid of the clinical pharmacists to work side by side and assist the physicians, in order to give a better care, protect and safeguard patients. The rationale of this focus article was to invite pharmacists to take a much more active role and to help physicians using their expertise in order to complete the therapeutic work in a more rational way. Pharmacists need to get out of their private stores, sharing their expertise and knowledge, by making active presence in the very infrastructure of clinical centers, such as hospitals and ambulatory.Physicians alone cannot cover every aspect of the pharmacological treatment, for example in the field of drug therapy monitoring, interactions, adverse drug reaction (ADR), toxicology, novel delivery systems, immunoglobuline-based therapeutics and other innovative drugs and medical devices systems, which have their pharmaceutical specific worlds.
