This study examined the effect of the handball warm-up program (HWP) on dynamic balance among elite adolescent handball players. In this case, 24 handball players were randomly assigned into experimental warm-up and control groups. The HWP was performed over 8 weeks (3 times per week). Dynamic balance before and after the intervention training programs were measured by the Biodex Balance System (BBS) and Y Balance test for the dominant (DL) and non-dominant legs (non-DL). After HWP training, BBS scores significantly improved in the overall (OSI) (30.4% and 31.1%), anterior-posterior (APSI) (44.6% and 35.2%), and medial-lateral stability indexes (MLSI) (38.8% and 43%) for both DL and non-DL. Post-training, the Y Balance test exhibited significant improvements in OSI (13.2% and 10.6%), anterior (17.2% and 12.6%), posteriolateral (12.8% and 11.3%), and posteriomedial stability indexes (9.2% and 7.9%) with DL and non-DL, respectively. In conclusion, dynamic balance improved overall after eight weeks of the HWP, with this improvement significantly greater with the trained versus the control group. Optimal balance during handball is an important factor to ensure coordinated and efficient movements and it is speculated that improved balance could positively impact injury prevention. Hence, the HWP program can be used as an alternative to a conventional warm-up program.

Zusammenfassung Handball gehort zu den popularsten aber auch verletzungsreichsten Ballsportarten. Das am haufigsten verletzte Gelenk ist das Kniegelenk. Die haufigste schwere Verletzung ist die Ruptur des vorderen Kreuzbandes (VKB), die typischerweise bei einem schnellen Richtungswechsel oder der Landung nach einem Sprungwurf entsteht. Die Ubersichtsarbeit diskutiert diesen Verletzungsmechanismus aber auch andere Verletzungsursachen und -formen, Risikofaktoren, Ansatze zur Pravention von Verletzungen und Uberlastungsschaden sowie altersspezifische Verletzungen.

The largest articulation inside human body is the knee joint which is composed by hard components, soft tissues and surrounded muscles. The knee is a mobile hinge, and it permits flexion, extension, slight internal and external rotation of the leg. The knee joint is vulnerable to both sharp injury and chronic osteoarthritis. Once have been injured, the knee joint is not easily restored. This study employs separately the experimental measurement, reverse engineering and finite element analysis to investigate the dynamic characteristics of intricate knee joint. The three-dimensional geometric model of each component of knee joint includes hard tissues and soft tissues. The hard tissues have femur, tibia, fibula, patella and the soft tissues have meniscus, patellar ligament, medial and lateral collateral ligament, a pair of cruciate ligaments, etc. Then the model is imported into ANSYS software. Via modal, periodic excitation and impact analysis, the dynamic characteristics of each component and the whole knee model are received. The fundamental mode shapes, natural frequencies and stresses of all the components of knee are also obtained. These normal modes are essential when investigating the dynamic motion of the whole knee. The results show that after impact, the soft tissues have larger displacement than that of the hard tissues. Consequently, the fracture occurs when the stretch which is caused by external force excess ultimate strength of the component. It also explains why the athletes frequently injure the ligaments and tendons of the knee or ankle during the intensive exercise. Therefore, by reducing the motion of articulation, the professional player could not only reduce the generated internal stresses in the tissue but also consequently lessen the chance of injury.

Unusual injury mechanism of a tibial plateau fracture in relation with a knee brace in handball – A case study

Summary We report a case of a 47-year-old male handball player who sustained a direct trauma in competition of his right lateral knee against the hinges of a braced defensive player. The trauma resulted in a split fracture of the lateral tibial plateau. There is a high probability that the hinged brace might have caused or at least aggravated the injury and that the rules of handball should be more detailed on the topic of protective gear.
