Background: Psoriasis is a chronic, immune-mediated inflammatory skin disease, and the inflammatory response plays an important role in its development and progression. Datura metel L. is a traditional Chinese medicine that exhibited a significant therapeutic effect on psoriasis in our previous study due to its remarkable anti-inflammatory effect. Meanwhile, the mechanism underlying its effects on psoriasis is still unclear. Methods: An imiquimod-induced psoriasis-like dermatitis mouse model was constructed to evaluate the protective effect of the effective part of Datura metel L. (EPD), which was verified by evaluations of the Psoriasis Area and Severity Index (PASI) score. Hematoxylin and eosin (H&E) staining, immunohistochemical examination, enzyme-linked immunosorbent assay (ELISA), and Western blot were used to measure the inflammatory cytokines and the protein expression associated with the Toll-like receptor 7– myeloid differentiation primary response gene 88–nuclear Factor-κB–nucleotide-binding oligomerization domain (Nod)-like receptor family pyrin domain-containing 3 (TLR7/8–MyD88–NF-κB–NLRP3) inflammasome pathway. Results: EPD significantly decreased the PASI, reduced epidermal thickness, and decreased the proliferation and differentiation of epidermal cells in psoriasis-like dermatitis C57BL/6 mice induced by imiquimod (IMQ). Furthermore, EPD reduced the infiltration of CD3+ cells to psoriatic lesions, as well as ameliorated the elevations of intercellular adhesion molecule 1 (ICAM-1) and inhibited the production of imiquimod-induced inflammatory cytokines, including IL-1β, IL-2, IL-6, IL-10, IL-12, IL-17, IL-22, IL-23, tumor necrosis factor-α (TNF-α), monocyte chemotactic protein 1 (MCP-1), and interferon-γ (IFN-γ). Besides, EPD decreased the imiquimod-induced expression levels of TLR7, TLR8, TRAF6, MyD88, p-IKKα, p-IKBα, p-NF-κB, NLRP3, apoptosis-associated speck-like protein contained a caspase recruitment domain (ASC), cysteinyl aspartate specific proteinase 1 (caspase-1), and IL-1β. Conclusion: This study demonstrated that EPD exhibited a protective effect on an imiquimod-induced psoriasis mice model by inhibiting the inflammatory response, which might be ascribed to the inhibition of the TLR7/8–MyD88–NF-κb–NLRP3 inflammasome pathway.

Datura Metel L. Ameliorates Imiquimod-Induced Psoriasis-Like Dermatitis and Inhibits Inflammatory Cytokines Production through TLR7/8-MyD88-NF-κB-NLRP3 Inflammasome Pathway.

Potential effects and mechanisms of Chinese herbal medicine in the treatment of psoriasis

Objectives : The aim of this study is to explore the types of pattern identification (PI, 辨證) and the differential points of PI used for the treatment of psoriasis in Traditional Chinese Medicine (TCM) based on the Chinese references and to provide the evidences applying PI for the treatment of psoriasis in clinical practice. Methods : This study extracted patterns of psoriasis through database CNKI (China National Knowledge Infrastructure) and analysis the patterns and classification criteria of the patterns. Those examined in the study are dermal symptoms, general symptoms, formula and herbs which are different depending on the patterns. Results : Total 60 studies were selected and 44 pattern types were extracted from them. We categorized the main pattern types on psoriasis used in TCM as ‘blood-heat syndrome(BHS, 血熱證)’, blood-stasis syndrome(BSS, 血瘀證), and ‘blood-dryness syndrome(BDS, 血燥證)’, ‘dampness-heat syndrome(DHS, 濕熱證)’ and ‘yang-deficiency syndrome(YDS, 陽虛證)’. Among these patterns, BHS was the most common. In TCM, the pattern of BHS tended to have skin symptoms and signs related to inflammatory erythema and heat. Both BSS and BDS were characterized by long disease duration and poor healing. In addition, DHS tended to have the skin symptoms and signs such as oozing and severe itching. The symptoms and signs related to coldness mainly showed in YDS. For PI criteria, ‘qi-blood-essence criteria(氣血津液辨證)’ and ‘eight-doctrine criteria(八鋼辨證) are commonly used. Conclusions : Our findings show that each PI on psoriasis in TCM has different characteristics related to dermal and general symptoms or signs. Further studies are needed to develop the diagnostic tool of PI on psoriasis reflecting on clinical practices in Korean Medicine by referring to the findings of this study about PI on psoriasis in TCM.

A Review on Patterns and Classification Criteria of Psoriasis by analyzing Chinese Theses

The Vitamin D receptor (VDR) is a crucial nuclear receptor that plays a vital role in various physiological functions. To a larger extent, the genomic effects of VDR maintain general wellbeing, and its modulation holds implications for multiple diseases. Current evidence regarding using vitamin D or its synthetic analogs to treat non-communicable diseases is insufficient, though observational studies suggest potential benefits. Traditional Chinese medicines (TCMs) and bioactive compounds derived from natural sources have garnered increasing attention. Interestingly, TCM formulae and TCM-derived bioactive compounds have shown promise in modulating VDR activities. This review explores the intriguing potential of TCM and bioactive compounds in modulating VDR activity. We first emphasize the latest information on the genetic expression, function, and structure of VDR, providing a comprehensive understanding of this crucial receptor. Following this, we review several TCM formulae and herbs known to influence VDR alongside the mechanisms underpinning their action. Similarly, we also discuss TCM-based bioactive compounds that target VDR, offering insights into their roles and modes of action.

Modulation of the vitamin D receptor by traditional Chinese medicines and bioactive compounds: potential therapeutic applications in VDR-dependent diseases

The aim of this study was to explore the anti-psoriatic effect and potential mechanism of Angelica polysaccharide (APS) on an in vitro HaCaT cell model. MTS assay was performed to determine whether APS has the ability to inhibit the proliferation of HaCaT cells. RNA-sequencing (RNA-seq) was performed to investigate the underlying mechanism of APS. Quantitative real-time PCR (qRT-PCR) was used to verify the accuracy of RNA-seq data. Our MTS assay results demonstrated that APS time- and concentration-dependently inhibits the proliferation of HaCaT cells. The anti-proliferation property of APS suggests that APS may have anti-psoriatic effect. In the RNA-seq part, comparison between the CK group (i.e., Control group) and ASP groups revealed dramatic differences [468 differentially expressed genes (DEGs) for CK group vs. ASP50 group; 563 DEGs for CK group vs. ASP100 group; 532 DEGs for CK group vs. ASP200 group]. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrich analysis performed on all DEGs failed to find any significant enriched GO terms or KEGG pathways to explain the anti-proliferative effect of APS. All DEGs were then classified into 20 expression profiles by trend analysis. Interestingly, cell proliferation-related GO terms were mostly dispersed in the profile 2 and 17. DEGs enriched in these terms were then analyzed. After literature retrieval, DEGs such as SERPINE1, SMAD6, CTGF, and TGF-β were suspected to closely relevant to the anti-proliferation effect of APS. qRT-PCR results showed similar expression trend with RNA-seq data for 8 out of 10 genes, indicating our sequence data are reliable.

Traditional Chinese medicine (TCM) has a long history in the treatment of psoriasis vulgaris We aimed to evaluate the clinical efficacy and safety of Chinese herbal Pulian ointment in treating psoriasis vulgaris of blood-heat syndrome A multicenter, randomized, double-blind, placebo-controlled trial was conducted Participants with psoriasis vulgaris of blood-heat syndrome were blinded and randomized to receive Pulian ointment or placebo ointment twice daily for 4 weeks, with follow-up 8 weeks after treatment Psoriasis Area Severity Index (PASI) scores, severity of each symptom and area of skin lesion and quality of life were assessed at baseline, 2 weeks, and 4 weeks Adverse events were recorded during the study SAS 94 software and SPSS 170 software was applied for data analysis A total of 300 participants with psoriasis vulgaris of blood-heat syndrome were assessed for eligibility, and 294 were randomly assigned to the Pulian ointment and placebo group from six study centers Full analysis set (FAS): after 4 weeks of treatment, there were significant differences between groups in PASI score and the separate score of skin lesion area, favoring Pulian ointment group (P   005) Per protocol set (PPS): There was no statistically significant difference in PASI score and separate score of each symptom and area of skin lesion between two groups (P > 005) Quality of life measured by Hamilton Anxiety Rating Scale (HAMA) and 36-Item Short Form Health Survey (SF-36) improved after treatment in both groups, but there was no significant difference between the two groups (P > 005) After being followed up for 8 weeks, the total relapse rates of the Pulian Ointment group and placebo group were 588 and 845%, respectively, and the difference was not statistically significant between the two groups (P > 005) No adverse event was observed in both groups throughout the study Pulian Ointment seems effective and well tolerated in improving the PASI score and separate score of skin lesion area for patients with psoriasis vulgaris of blood-heat syndrome Further research could build on the current study to explore whether other preparation forms and greater intervention intensity are necessary for better therapeutic effects Chictrorgcn Identifier ChiCTR-TRC-12002054
