Bone development, casein-free diet use, supplements, and medications were assessed for 75 boys with autism or autism spectrum disorder, ages 4–8 years. Second metacarpal bone cortical thickness (BCT), measured on hand-wrist radiographs, and % deviations in BCT from reference medians were derived. BCT increased with age, but % deviations evidenced a progressive fall-off (p = .02): +3.1 ± 4.7%, −6.5 ± 4.0%, −16.6 ± 3.4%, −19.4 ± 3.7%, −24.1 ± 4.4%, at ages 4–8, respectively, adjusting for height. The 12% of the boys on casein-free diets had an overall % deviation of −18.9 ± 3.7%, nearly twice that of boys on minimally restricted or unrestricted diets (−10.5 ± 1.3%, p < .04), although even for boys on minimally restricted or unrestricted diets the % deviation was highly significant (p < .001). Our data suggest that the bone development of autistic boys should be monitored as part of routine care, especially if they are on casein-free diets.

Reduced Bone Cortical Thickness in Boys with Autism or Autism Spectrum Disorder.

The morbidity and toxicity associated with current intensive treatment protocols for acute lymphoblastic leukemia in childhood become even more important as the vast majority of children can be cured and become long-term survivors. Osteonecrosis is one of the most common therapy-related and debilitating side effects of anti-leukemic treatment and can adversely affect long-term quality of life. Incidence and risk factors vary substantially between study groups and therapeutic regimens. We therefore analyzed 22 clinical trials of childhood acute lymphoblastic leukemia in terms of osteonecrosis incidence and risk factors. Adolescent age is the most significant risk factor, with patients >10 years old at the highest risk. Uncritical modification or even significant reduction of glucocorticoid dosage cannot be recommended at this stage. A novel and innovative approach to reduce osteonecrosis-associated morbidity might be systematic early screening for osteonecrosis by serial magnetic resonance images. However, discriminating patients at risk of functional impairment and debilitating progressive joint disease from asymptomatic patients still remains challenging.

Osteonecrosis in children with acute lymphoblastic leukemia.

Many experts believe that optimizing bone mineral accrual early in life may prevent childhood fractures and possibly delay the development of osteoporosis later in life. Adequate nutrition and physical activity are environmental factors important in determining whether or not children acquire an appropriate amount of bone for their body size. Pediatric diseases, or therapeutic interventions used in their treatment, may interfere with normal bone development. Although there are specific methods available for assessing pediatric bone, there is no one method that can adequately assess bone health and identify the specific bone deficits that may be occurring. Understanding the biological basis for bone deficits and the ability of various bone assessment methods to discriminate or measure these deficits is important in understanding normal bone development and how to prevent and treat pediatric bone disease. The purpose of this review is to briefly describe changes in bone with growth, to define “bone density” in biological terms, to discuss some of the issues with pediatric bone measurements, and to review the three main methods for assessing bone parameters in pediatric populations. These methods, including dual energy X-ray absorptiometry (DXA), quantitative ultrasound (QUS) and peripheral quantitative computed tomography (pQCT) will be described, the advantages and disadvantages discussed, and the relationship between bone parameters and fracture risk presented for each of the methods.

BONE MEASUREMENTS BY PERIPHERAL QUANTITATIVE COMPUTED TOMOGRAPHY (pQCT) IN CHILDREN WITH CEREBRAL PALSY

In boys, the hormonal changes that accompany normal puberty are well defined, as are the physical signs of pubertal development and the kinetics of the growth spurt. Most androgens are derived from the testes, although adrenal androgens may also contribute; testosterone can also be aromatized to estrogen to exert important effects during puberty. Androgens, but especially their conversion to estrogens by aromatase, have a major role in the dramatic changes in linear growth, secondary sexual characteristics, and changes to bone, muscle and fat distribution that occur during puberty. Androgen therapy for delayed puberty should permit full normal pubertal development and thereby also address some of the associated psychosocial problems. Adolescent boys with conditions of permanent hypogonadism (hypogonadotropic or hypergonadotropic) or transient hypogonadotropic hypogonadism (constitutional delay of growth and puberty) can benefit from testosterone therapy. Long-term testosterone therapy should be given for hypothalamic or pituitary gonadotropin deficiency, or for primary hypogonadism such as for adolescents with Klinefelter syndrome, if endogenous testosterone levels drop or levels of luteinizing hormone rise. Intramuscular administration every few weeks is effective, but newer cutaneous forms, for example, gels or patches, also show promise in permitting adolescent males to reach adult body composition.

Male Pubertal Development and the Role of Androgen Therapy

Bone development is one of the key processes characterizing childhood and adolescence. Understanding this process is not only important for physicians treating pediatric bone disorders, but also for clinicians and researchers dealing with postmenopausal and senile osteoporosis. Bone densitometry has great potential to enhance our understanding of bone development. The usefulness of densitometry in children and adolescents would be increased if the physiological mechanisms and structural features of bone were given more consideration in the design and interpretation of densitometric studies. This review gives an overview on the most relevant techniques of quantitative noninvasive bone analysis. Furthermore it describes the relationship between bone biology, selected surrogates describing the biological processes and the possibilities of measuring these surrogates specifically and precisely by the different devices. The overall recommendation for researchers in this field is to describe firstly the biological process to be analyzed (bone growth in length, remodeling or modeling, or all together), secondly the bone parameter which describes this process, and thirdly the reason for selecting a special device.
