Fine-needle aspiration cytology

Twenty adults were treated for intussusception in two large hospitals from 1969 to 1988. Fourteen intussusceptions originated in the small intestine and 6 in the large intestine. Diagnosis was reached preoperatively in only 10 patients, probably due to the atypical clinical picture. In addition to a high degree of suspicion, careful examination of plain abdominal radiograph and ultrasonography are helpful in diagnosing adult intussusception. In 18 of 20 patients, an organic lesion causing intussusception was found. In six patients the cause was a malignancy. In such cases surgical treatment is necessary. In jejunojejunal and ileoileal intussusceptions, an attempt at primary reduction followed by resection or enterotomy is justified. In most cases of ileocolic, ileocecocolic, and colocolic intussusception, primary resection is the treatment of choice, especially in patients over 60 years old because of the high incidence of malignancy.

Diagnosis and treatment of adult intussusception

We describe an accurate and simple method of diagnosing thyroid nodules by the modified technique of ultrasoundguided fine needle aspiration biopsy (UG-FNAB). An Aloka SSD 2000 focus type scanner (Aloka Co., Tokyo, Japan) equipped with a 10-MHz mechanical sector probe and a 7.5-MHz convex probe was used. First, a clear picture of the thyroid gland was taken with the use of the 10-MHz probe to determine the abnormalities. Then, a 7.5-MHz probe (1.5 ± 0.8 cm of surface area) was used to guide the needle perpendicularly to the neck. This method enabled us to obtain samples from nodules greater than 5 mm. We applied this technique to 1000 patients who had uncertain diagnosis by the conventional method of FNAB. Ultrasound-guided fine-needle biopsy of focal liver lesions: techniques, diagnostic accuracy and complications: A retrospective study on 2091 biopsies

Ultrasonically-guided fine-needle aspiration biopsy of 52 suspected parathyroid tumors was performed in 42 patients with primary or secondary hyperparathyroidism. Evidence of a parathyroid tumor was detected cytologically in 31 cases; in 14 the lesion could be attributed to the thyroid gland, while in 7 the aspirated material was inadequate. This technique proved to be extremely helpful in differentiating parathyroid tumors from thyroid nodules and in identifying parathyroid lesions having an atypical location or echo pattern.

The ultrasonic findings in a case of small bowel intussusception in an adult patient are presented. The sonographic features vary with the scanning plane. Scans along the longitudinal axis of the involved segment show a diagnostic pattern, not previously described: three parallel stripes of low echogenicity delineating two reflective areas.

Intussusception of the bowel: a new sonographic pattern

Ultrasonically guided fine-needle biopsy of focal liver lesions was performed in 126 patients to obtain a definitive diagnosis. Percutaneous target puncture and tissue aspiration were guided by a real-time linear-array probe with a central canal for needle insertion. The procedure is simple and rapidly performed; a correct cytological diagnosis was established in 94% of the punctured masses. No adverse reactions of clinical significance were observed. Fine-needle biopsy with ultrasound guidance by real-time scanning may be the procedure of choice to obtain a tissue diagnosis in patients with hepatic masses.

Fine-needle aspiration biopsy of bowel lesions under ultrasound guidance: indications and results.

Forty cases of acute cholecystitis, surgically and pathologically confirmed, were investigated with ultrasound. Echographic diagnosis was possible in 34 patients (85%): special emphasis was laid on wall thickening (90% of cases, with a mean wall thickness of 8.08 mm), evidence of an echofree layer within the wall (67.5% of patients) and a lumen greater than 4 cm with thick walls. Associated cholelithiasis was demonstrated in 28 of these patients. In five more patients specific wall signs were absent and cholelithiasis was the only detectable sign of the disease. In only one patient was there no echographic sign of acute cholecystitis.
