Laser irradiation has numerous favorable characteristics, such as ablation or vaporization, hemostasis, biostimulation (photobiomodulation) and microbial inhibition and destruction, which induce various beneficial therapeutic effects and biological responses. Therefore, the use of lasers is considered effective and suitable for treating a variety of inflammatory and infectious oral conditions. The CO2 , neodymium-doped yttrium-aluminium-garnet (Nd:YAG) and diode lasers have mainly been used for periodontal soft-tissue management. With development of the erbium-doped yttrium-aluminium-garnet (Er:YAG) and erbium, chromium-doped yttrium-scandium-gallium-garnet (Er,Cr:YSGG) lasers, which can be applied not only on soft tissues but also on dental hard tissues, the application of lasers dramatically expanded from periodontal soft-tissue management to hard-tissue treatment. Currently, various periodontal tissues (such as gingiva, tooth roots and bone tissue), as well as titanium implant surfaces, can be treated with lasers, and a variety of dental laser systems are being employed for the management of periodontal and peri-implant diseases. In periodontics, mechanical therapy has conventionally been the mainstream of treatment; however, complete bacterial eradication and/or optimal wound healing may not be necessarily achieved with conventional mechanical therapy alone. Consequently, in addition to chemotherapy consisting of antibiotics and anti-inflammatory agents, phototherapy using lasers and light-emitting diodes has been gradually integrated with mechanical therapy to enhance subsequent wound healing by achieving thorough debridement, decontamination and tissue stimulation. With increasing evidence of benefits, therapies with low- and high-level lasers play an important role in wound healing/tissue regeneration in the treatment of periodontal and peri-implant diseases. This article discusses the outcomes of laser therapy in soft-tissue management, periodontal nonsurgical and surgical treatment, osseous surgery and peri-implant treatment, focusing on postoperative wound healing of periodontal and peri-implant tissues, based on scientific evidence from currently available basic and clinical studies, as well as on case reports.

Periodontal and peri‐implant wound healing following laser therapy

Pathogenesis of apical periodontitis and the causes of endodontic failures

The objective of this study was to examine a new blue light diode laser system (445 nm) for dental soft tissue surgery on cellular level. An in vitro cell culture model was established to evaluate the effects of the 445-nm diode laser in comparison to an established infrared diode laser (IR). Monolayer cell cultures were irradiated and wound healing was morphometrically measured. Fluorescence staining was used for proof of potential DNA double-strand breaks as well as cytoskeleton alterations. Cellular live/dead discrimination was performed and temperature development during laser irradiation was measured with a thermographic infrared camera. A characteristic zone formation was detected after irradiation with both wavelengths. Despite a larger wound area after irradiation with 445 nm, due to its higher temperature development, this laser system showed a faster wound healing in comparison to the IR laser. No increase of devitalized cells was documented with higher distances between laser tip and cell layer and thus without thermal interaction. Neither cytoskeleton alteration nor DNA double-strand breaks could be recorded after irradiation in non-contact mode. The blue diode laser system demonstrated an excellent direct thermal coupling to cells and tissues without side effects even by reduced power settings. The blue diode laser seems to be a promising technology for clinical application due to high absorption of blue light without major side effects in adjacent tissues even by reduced power settings.

Examples of apparatus, methods, and compositions for endodontic treatments are described. The apparatus can include a fluid platform configured to substantially retain fluid in a tooth chamber during treatment. The fluid platform can help maintain fluid circulation in the tooth chamber as fluid flows into and out of the tooth chamber. The apparatus can also include a pressure wave generator configured to generate acoustic waves that can be used for cleaning root canals and tooth surfaces in the tooth chamber. Examples of pressure wave generators include a liquid jet, an electromagnetic energy delivery device, and an ultrasonic device. The fluid can include antiseptic or antibacterial solutions to assist in tooth cleaning. The fluid may be degassed to have a reduced dissolved gas content (compared to non- degassed fluids used in endodontic treatments), which may improve the effectiveness of the pressure wave generation or the cleaning.

Apparatus, methods, and compositions for endodontic treatments

Radial-firing tips should allow a more homogeneous laser irradiation of root canal walls. The aim of the study was to assess the effects of erbium, chromium:yttrium-scandium-gallium-garnet (Er,Cr:YSGG) laser irradiation in conjunction with those newly designed tips. The investigation comprised bacteriology, morphological evaluations and temperature measurements. Root canals were inoculated with two test strains and laser irradiated with power settings of 0.6 W and 0.9 W and a repetition rate of 20 Hz. Subsequently, the samples were subjected to microbiological evaluation. The morphological changes of the canal walls were assessed by scanning electron microscopy. To reveal possible thermal side effects, we carried out temperature measurements. The bacteriological evaluation revealed a decisive disinfectant effect. Scanning electron microscopy showed the homogeneous removal of smear layer from the root canal walls. The temperature rise at the root surface during the irradiation was moderate, yielding 1.3°C for the 0.6 W setting and 1.6°C for the 0.9 W setting. The investigations indicated that the Er,Cr:YSGG laser, in conjunction with radial-firing tips, is a suitable tool for the elimination of bacteria in root canals and for the removal of smear layer.

The impact of an erbium, chromium:yttrium-scandium-gallium-garnet laser with radial-firing tips on endodontic treatment

The aim of this study was to investigate the effect of different working modes (pulsed and micropulsed) and power settings of a standardized 980-nm diode laser on collateral thermal soft-tissue damage. A total of 108 bovine liver samples were cut with a diode laser at various settings in pulsed and micropulsed mode and histologically assessed to determine the area and depth of carbonization, necrosis and reversible tissue damage, as well as incision depth and width. Incision depth and width and the area and depth of carbonization, necrosis and reversible damage were correlated strongly with cutting speed. The area and depth of reversible damage were correlated with average power. The micropulsed mode produced a smaller zone of carbonization and necrosis and a smaller incision width. Setting the laser parameters in accordance with the absorption characteristics of the tissue reduced collateral thermal tissue damage while maintaining an acceptable cutting ability. Reducing collateral thermal damage from diode laser incisions is clinically relevant for promoting wound healing.

Reduction of collateral thermal impact of diode laser irradiation on soft tissue due to modified application parameters.

Since the diode laser is a good compromise for the daily use in dental offices, finding usage in numerous dental indications (e.g., surgery, periodontics, and end- odontics), the minimization of the collateral damage in laser surgery is important to improve the therapeutical outcome. The aim of this study was to investigate the effect of water/air cooling on the collateral thermal soft tissue damage of 980-nm diode laser incisions. A total of 36 mechanically executed laser cuts in pork liver were made with a 980-nm diode laser in micropulsed mode with three different settings of water/air cooling and examined by histological assessment to determine the area and size of carbonization, necrosis, and reversible tissue damage as well as incision depth and width. In our study, clearly the incision depth increased significantly under water/air cooling (270.9 versus 502.3 μm—test group 3) without significant changes of incision width. In test group 2, the total area of damage was significantly smaller than in the control group (in this group, the incision depth increases by 65 %). In test group 3, the total area of damage was significantly higher (incision depth increased by 85 %), but the bigger part of it represented a reversible tissue alteration leaving the amount of irreversible damage almost the same as in the control group. This first pilot study clearly shows that water/air cooling in vitro has an effect on collateral tissue damage. Further studies will have to verify, if the reduced collateral damage we have proved in this study can lead to accelerated wound healing. Re- duction of collateral thermal damage after diode laser incisions is clinically relevant for promoted wound healing.
