Objective: The golden standard for measuring erythrocyte sedimentation rate (ESR) is the Westergren method. Some other methods for measuring ESR have become available in the recent decades. They differ from a modest modification of the Westergren method to very different methodologies. Even though the methodologies of these tests are different the results of them are assessed as the same test by clinicians. Therefore the bias reasoned by different methodology has caused misinterpretation of the patients by clinicians. The comparison and harmonization of this method will provide a standardization and same interpretation for ESR. Methods: The golden standard method for measuring ESR which is called Westergren Method and two prototypes of the remaining methods were chosen for comparison. Three different group of patients were were assessed with three different methods in this study. Results: The monitorization of three samples were observed for 60 minutes. There was no correlation detected depending on the time after the monitorization had started. Hence the adjustment of the results were collected after 20 or 30 minutes for simulating 60 minutes were found significantly different than the classical Westergren method. The detected bias between the adjustment of the earlier result to one hour and the classical method is not always different than the classical method. The sample results were found significantly different in comparison to two methods for all selected patient groups. Conclusion: There is no sedimentation algorithm detected in ESR monitorization process. Therefore the observed bias is thought to be caused by the adjustment of the earlier results to one hour. The adjusted results of 30 minutes were found to be significantly similar compared to the adjusted results of 20 minutes. The results of the capillary photometric kinetic method were found unbiased with the classical Westergren method and compatible with the clinical observations.

Abstract Background and Aims The erythrocyte sedimentation rate (ESR) is an essential inflammatory marker in treating some patients, particularly children. The level of ESR can be affected by red blood cell (RBC) indices, and not considering this can complicate the interpretation of ESR and the treatment and follow‐up of patients. The study aimed to assess the association between ESR and RBC indices in children hospitalized with fever and cough in the pediatric ward of Imam Khomeini Hospital, Jiroft, in 2023. Methods A cross‐sectional study was conducted to measure the association between ESR and RBC indices in children hospitalized with fever and cough in the pediatric ward of Imam Khomeini Hospital, Jiroft, in 2023. A total of 156 patients participated in the study. SPSS software was used for statistical analysis. Results The mean age of participants was 27.26 ± 3.14 months. The results showed that there is a significant negative correlation between ESR and RBC, r = −0.282 (p < 0.001), and ESR and hematocrit (HCT), r = −0.215 (p = 0.007). Also, the results demonstrated that there is a significant positive correlation between ESR and mean corpuscular volume (MCV), r = 0.159 (p = 0.048), ESR and mean corpuscular hemoglobin (MCH), r = 0.214 (p = 0.007), and ESR and mean cell hemoglobin concentration (MCHC), r = 0.209 (p = 0.009). There was a negative correlation between ESR and hemoglobin (Hb), r = −0.98 (p = 0.225), but this correlation was insignificant. Conclusion This study showed an association between ESR and RBC indices in hospitalized children with complaints of fever and cough. So, it is necessary that physicians and treatment staff pay attention to the RBC indices while interpreting and following up the results of ESR to complete the treatment process of patients.

AMAC: Eritrosit sedimentasyon hizi (ESR) klinik uygulamada yaygin olarak kullanilan bir laboratuvar belirtecidir. ESR, temel olarak iki faktorden etkilenir: fibrinojen gibi plazma proteinleri ve hematokrit gibi eritrositler ile iliskili bazi durumlar. Hematokritin ESR uzerindeki etkisini ortadan kaldirarak ESR'yi yorumlayabilmek icin bazi formuller gelistirilmistir. Bu calismanin amaci, hematokrite gore duzeltilmis ESR formullerinin C-reaktif protein (CRP) ile daha iyi bir korelasyonunun olup olmadigini belirlemekti. GEREC VE YONTEM: Hastanemiz veri tabanindan Şubat 2018'e ait veriler alindi. Ayni anda elde edilen kan orneklerinde olculen ESR, CRP ve hematokrit degerlerine ait toplam 1206 hastanin sonuclari, bu parametreler arasinda bir korelasyon olup olmadigi acisindan analiz edildi. Hematokrite gore duzeltilmis ESR degerleri, asagidaki formullerle hesaplandi: (a) Hematokrite gore duzeltilmis sedimentasyon-1 (HA-ESR-1) = [(15)/(55-hematokrit)]X sedimentasyon, (b) Hematokrite gore duzeltilmis sedimentasyon- 2 (HA-ESR-2) = (hematokrit/45)X sedimentasyon. BULGULAR: HA-ESR-2 standart ESR olcumlerine kiyasla CRP ile daha iyi bir korelasyona sahipti, ancak HA-ESR-1, CRP ve ESR arasindaki iliskiyi iyilestiremedi. SONUC: HA-ESR-2'nin CRP ile standart ESR'den anlamli olarak daha iyi bir korelasyon gosterdigini bulduk. Bu nedenle, soz konusu formul klinik uygulamada yararli olabilir.
