Summary Background Since December, 2019, Wuhan, China, has experienced an outbreak of coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Epidemiological and clinical characteristics of patients with COVID-19 have been reported but risk factors for mortality and a detailed clinical course of illness, including viral shedding, have not been well described. Methods In this retrospective, multicentre cohort study, we included all adult inpatients (≥18 years old) with laboratory-confirmed COVID-19 from Jinyintan Hospital and Wuhan Pulmonary Hospital (Wuhan, China) who had been discharged or had died by Jan 31, 2020. Demographic, clinical, treatment, and laboratory data, including serial samples for viral RNA detection, were extracted from electronic medical records and compared between survivors and non-survivors. We used univariable and multivariable logistic regression methods to explore the risk factors associated with in-hospital death. Findings 191 patients (135 from Jinyintan Hospital and 56 from Wuhan Pulmonary Hospital) were included in this study, of whom 137 were discharged and 54 died in hospital. 91 (48%) patients had a comorbidity, with hypertension being the most common (58 [30%] patients), followed by diabetes (36 [19%] patients) and coronary heart disease (15 [8%] patients). Multivariable regression showed increasing odds of in-hospital death associated with older age (odds ratio 1·10, 95% CI 1·03–1·17, per year increase; p=0·0043), higher Sequential Organ Failure Assessment (SOFA) score (5·65, 2·61–12·23; p Interpretation The potential risk factors of older age, high SOFA score, and d-dimer greater than 1 μg/mL could help clinicians to identify patients with poor prognosis at an early stage. Prolonged viral shedding provides the rationale for a strategy of isolation of infected patients and optimal antiviral interventions in the future. Dans les pays en voie de developpement, notamment en Afrique subsaharienne, la stimulation cardiaque restait tres peu repandue Nous avons initie un programme specifique multinational (Africa-Pace) visant a initier et developper la stimulation cardiaque dans 14 pays africains de 1996 a 2015, avec pour objectif de donner a chaque pays les moyens de developper leur propre programme de stimulation Cet enseignement pratique etait dispense sur place aux medecins prometteurs identifies ainsi qu’aux equipes paramedicales, avec une evaluation prospective et continue du programme Au total, 502 patients ont ete operes au cours des 61 missions Plus de la moitie des patients initialement listes ne se presentaient pas a l’hopital au cours de la mission, principalement du fait de la survenue d’un deces subit au cours des semaines ou mois d’attente Au total, nous avons denombre une incidence de complications de 6 % lors des missions Ce programme a permis une augmentation significative du taux d’implantation au cours des 20 dernieres annees du fait du demarrage d’une activite propre de la majorite des pays participants Ces resultats illustrent la faisabilite et l’efficacite d’une formation personnalisee sur place pour developper la stimulation cardiaque en Afrique Cependant, malgre l’augmentation d’activite, le taux d’implantation reste pres de 500 fois plus bas que celui des pays europeens, soulignant l’importance d’un investissement local plus important

BACKGROUND AND AIMS
The incidence of atrial tachyarrhythmias is high in patients with atrioventricular septal defect (AVSD). No specific data on catheter ablation have been reported so far in this population. We aimed to describe the main mechanisms of atrial tachyarrhythmias in patients AVSD and to analyze outcomes after catheter ablation.


METHODS
This observational multicentric cohort study enrolled all patients with AVSD referred for catheter ablation of an atrial tachyarrhythmia at 6 tertiary centers from 2004 to 2022. The mechanisms of the different tachyarrhythmias targeted were described and outcomes were analyzed.


RESULTS
Overall, 56 patients (38.1±17.4 years, 55.4% females) were included. A total of 87 atrial tachyarrhythmias were targeted (mean number of 1.6 per patient). Regarding main circuits involved, a cavo-annular isthmus-dependent IART was observed in 41 (73.2%) patients and an IART involving the right lateral atriotomy in 10 (17.9%) patients. Other tachyarrhythmias with heterogeneous circuits were observed in 13 (23.2%) patients including 11 left-sided and 4 right-sided tachyarrhythmias. Overall, an acute success was achieved in 54 (96.4%) patients and no complication was reported. During a mean follow-up of 2.8±3.8 years, 22 (39.3%) patients had at least one recurrence. Freedom from atrial tachyarrhythmia recurrences was 77.5% at one year. Among 15 (26.8%) patients who underwent repeated ablation procedures, heterogeneous circuits including biatrial and left-sided tachyarrhythmias were more frequent.


CONCLUSION
In patients with AVSD, most circuits involve the cavo-annular isthmus, but complex mechanisms are frequently encountered in patients with repeated procedures. The acute success rate is excellent, although recurrences remain common during follow-up.

Catheter ablation of atrial tachyarrhythmias in patients with atrioventricular septal defect.

Major advances in pediatric cardiology in recent decades, especially surgical techniques, have resulted in an increasing number of patients with congenital heart disease (CHD) surviving to adulthood. This has generated new challenges, particularly with regards to the late onset of complex arrhythmias. Abnormal anatomy, surgical scarring, chronic hypoxemia, hemodynamic compromise, neuro-hormonal abnormalities, and genetic factors can all contribute to creating a unique substrate for arrhythmia development. This review attempts to synthesize the current state of knowledge spanning the spectrum from underlying mechanisms of arrhythmias in patients with congenital heart disease to current ablative strategies. We discuss existing knowledge gaps and highlight important areas for future research.
