目的 Twinblock矫正器在安氏Ⅱ类错he治疗中的应用特点。方法 21名10-12岁的以下颌后缩为主的安氏Ⅱ类错he患者应用Twinblock进行矫治。男9名，女12名。每位患者治疗前拍摄头颅侧位、曲面断层及手腕骨X线片。对Twinblock治疗前后头颅侧位X光片进行18个项目的测量，并对治疗前后测量结果进行配对t检验。结果 SNA、ANB、OJ、UI／SN、UL-ELine、LL-ELine等在治疗后减小，差异有显著性；SNB、LI／MP、U1／PP、U6/PP、L6／PP、鼻唇角及颏唇角增大，差异有显著性。下颌升支、下颌体以及下颌综合长度均有增长，差异有显著性。下颌平面角在正畸治疗后无显著变化。结论 Twinblock在生长发育期的Ⅱ类患者的治疗中可以减小覆盖、覆he、改善he关系并有效的改善患者下颌后缩的面型。对于存在拥挤的Ⅱ类患者，Twinblock及二期治疗时的拔牙治疗也可以达到良好的治疗效果。

Otitis media (OM) and its recurring (rAOM), effusive (OME), and chronic forms, represent a frequent clinical challenge. The middle ear, the mandible, and the temporomandibular joint (TMJ) share several embryological and anatomical connections. Despite that, the role of mandibular malposition and TMJ dysfunction is frequently overlooked in the management of otitis media. In this narrative review, we present current evidence supporting the etiopathogenetic role of a dysfunctional stomatognathic system in the onset of OM and the effectiveness of orthognathic treatment in preventing rAOM and OME. In particular, a focus on the influence of TMJ on Eustachian tube function is provided.

Temporomandibular Joint and Otitis Media: A Narrative Review of Implications in Etiopathogenesis and Treatment

PDTB, we think it would be premature to conclude at this time—without further studies—that this molecular study in a mouse model of cleidocranial dysplasia conclusively explains the hearing loss we observe clinically in humans with PDTB. In your Triological thesis, you state, ‘‘it is likely that each disease process (Paget’s disease, otosclerosis, osteogenesis imperfecta, etc.) will prove to involve different mechanisms of hearing loss (5).’’ Likewise, we think it is prudent to perform similar studies in Paget’s disease before validating this mechanism. Finally, given the slowly progressive change in hearing in the great majority of patients, compounded by presbycusis in the aging population, it is likely difficult to definitively analyze hearing loss after treatment compared with untreated disease without decades of follow-up. We absolutely agree that most patients with PDTB will not need a cochlear implant in their lifetime; however, we point out that in these rare cases, cochlear implantation may be effective means of auditory rehabilitation.

Aminoglycoside or Quinolone Ear Drops in the Postoperative Management of Tympanoplasty: What Choice Do We Have?

This study aims to evaluate whether there is a correlation between otitis and dental malocclusions.Electronic databases were searched for observational studies published until July 2021 without language or time restrictions.CRD42021270760. Observational studies on children with and without OM and/or malocclusion were included. After removing duplicates and excluding not-eligible articles, two reviewers screened relevant articles independently. Two reviewers independently extracted data and assessed data quality and validity through the Newcastle-Ottawa Scale (NOS) quality assessment tool for non-randomized studies.Five studies met the selection inclusion criteria and were included in the studies for a total of 499 patients. Three studies investigated the relationship between malocclusion and otitis media, while the remaining two studies analyzed the inverse relationship and one of them considered eustachian tube dysfunction as a proxy of OM. An association between malocclusion and otitis media and vice versa emerged, although with relevant limitations.There is some evidence that there is an association between otitis and malocclusion; however, it is not yet possible to establish a definitive correlation.

Future Perspectives in the Management of Otitis Media

Aim. To investigate the role of dental malocclusion treatment in the outcomes of Recurrent Acute Otitis Media (RAOM). Materials and Methods. The clinical outcome (number of acute recurrences in 12 months) of 61 consecutive children treated medically for RAOM was analysed. Children underwent an odontostomatologic evaluation, a fiberoptic endoscopy, and skin-prick tests. Results. 32 children (group A) were diagnosed with dental malocclusion and treated with a mandibular repositioning plate. Dental malocclusion was ruled out in the other 29 patients with RAOM, and they were used as controls (group B). The two groups were homogeneous in terms of sex, exposure to RAOM risk factors, skin test results, and adenoid hypertrophy, while age was significantly higher in group A. Age, sex, exposure to RAOM risk factors, adenoid hypertrophy, and skin test results were not associated with RAOM outcome. Children in group A treated for dental malocclusion were strongly associated with a lower number of acute episode recurrences at both univariate (p < 0.0001) and multivariate analysis (p = 0.001). Conclusions. RAOM showed better outcomes in children with dental malocclusion wearing a mandibular repositioning device. Dental malocclusion in children with RAOM may play a role in the pathogenesis of Eustachian tube dysfunction.
