Menstrual regulation in the United States: a preliminary report.

This chapter explains what menstrual regulation (MR) is. Postcoital methods of fertility control, MR, and induced abortion represent a continuum of procedures that may be used to limit fertility or terminate pregnancy. Postcoital methods and MR are employed before the diagnosis of pregnancy can be reliably established and thus are often used when the woman is not pregnant. These procedures are differentiated primarily by the time at which they are performed. Postcoital methods may be defined as procedures performed after coitus up to the time of the expected onset of a menstrual period. MR may be defined as any procedure used to terminate a suspected pregnancy no later than 14 days after the expected onset of a menstrual period. MR has also been referred to as endometrial aspiration, menstrual extraction, menstrual induction, and mini-abortion. Some amenorrheic women who undergo MR will not be pregnant at the time of the procedure. While it is desirable to eliminate unnecessary MR procedures, this is not presently feasible based on standard pregnancy tests.

BEFORE the US Supreme Court decision which legalized abortion on Jan 22, 1973, the performing of uterine evacuations before pregnancy could be diagnosed was suggested as a way of evading abortion laws.1Some advocates argued that women did not really want to know whether they had terminated a pregnancy or not. If no examination of tissue was performed, both doctor and patient could be ignorant of the diagnosis. No one seemed concerned regarding the possibility of overlooking an ectopic pregnancy, hydatidiform mole, or an endocrine dysfunction. Articles on the subject appeared in one account (Newsweek, July 24, 1972, p 69), in Johnston's account (New York Times, Nov 19, 1972), and other newspapers, evoking much interest from the public. Menstrual regulation is usually performed by aspirating the superficial endometrium and any products of conception through a small-diameter flexible cannula without anesthesia or dilation of the cervix in those women with a delayed menstrual period up to 14 days to terminate possible pregnancy. Since menstrual regulation can be effectively used only during the first 6 weeks of amenorrhea when diagnosis of pregnancy cannot be confirmed on the basis of physical signs or pregnancy tests some nonpregnant women will inevitably also undergo regulation. This is clinically acceptable if complications are less for all the women undergoing the procedure than if only the gravidas were aborted when pregnancy could be reliably diagnosed. Although the morbidity associated with regulation experienced by pregnant vs. nonpregnant women is not yet known preliminary data indicate that this is a potentially safer pregnancy termination method than traditional suction curettage.
