There have been many recent advances in techniques for prenatal diagnosis. The direct diagnostic procedures of amniocentesis and chorionic villus sampling, which involve analysis of foetal material, are described and indications of their risks and reliability are given. Other direct procedures are briefly mentioned.



Indirect biochemical analyses that identify groups at increased risk are also considered and their individual reported relationships with Down's syndrome are discussed. Technical problems and reported acceptability of maternal serum alpha fetoprotein screening are briefly mentioned.



Studies reporting the potential of ultrasonography in the prenatal detection of Down's syndrome are reviewed. Other indications of increased risk are briefly mentioned.

The concentration of lamellar bodies expressed as the optical density at 700 nm in amniotic fluid samples was calculated after centrifugation of the samples to obtain the lamellar bodies in a relatively pure form. The lamellar body concentrations obtained were compared with lecithin-sphingomyelin (L/S) ratios from the same samples. The advantages of the method compared with the L/S ratio are discussed as is the dependence on total volume that the method shares with all other procedures which depend on the measurement of a single concentration.

Determination of fetal lung maturity by fluorescence polarization of the amniotic fluid lamellar bodies.

406 midtrimester amniotic fluid samples were examined for alpha-fetoprotein (AFP) levels and fetal karyotyping. 44 cases with Down syndrome, 12 with Klinefelter syndrome and 14 with other chromosomal abnormalities were diagnosed. The AFP levels were below the median value for the normal pregnancies in 38 of the 44 Down syndrome-affected fetuses. The mean AFP value was significantly lower (p less than 0.0001) in this group. No such difference was found for fetuses affected by Klinefelter syndrome or other chromosomal aberrations. Almost all of the values for the Down syndrome-affected fetuses were, however, within the normal range, thereby precluding the possibility of using this measurement as an alternative to fetal karyotyping as a screening test for Down syndrome.
