Neoadjuvant and intrapleural therapies for malignant pleural mesothelioma

7604 Background: Based on our experience using intracavitary chemotherapy for peritoneal mesothelioma we have given repeated outpatient courses of intrapleural (IP) chemotherapy plus IP radiation with P32 to selected patients with malignant pleural mesothelioma. Methods: Pts underwent initial videothoracoscopy (VATS) at which two IP catheters were placed, anteriorly from the ipsilateral lung apex along the pericardial border, and posteriorly into and along the diaphragmatic sulcus. Each catheter was attached to a subcutaneous mediport chamber. IP cisplatin 20–40 mg, doxorubicin 20 mg, or gamma interferon (1,000 micrograms) was injected into both ports to perfuse the entire pleural space, at 1–2 weekly intervals over 6–12 weeks. The distribution of injectate was verified by intracavitary iohexol CT imaging. Additional intravenous pemetrexed plus ciplatin chemotherapy was given to selected patients.
