Determination of the occlusal vertical dimension (OVD) is an integral part of complete dentures fabrication Due to the lack of teeth, the clinician faces the challenge of how to accurately establish the OVD of the new denture Therefore, the purpose of this review article was to present, discuss and critique the available methods used in determining the OVD for complete dentures patients This review identified two main streams to determine the OVD: (i) pre-extraction methods and (ii) post-extraction methods For the pre-extraction methods, the OVD of the natural dentition is transferred to the new dentures mainly by intra-oral measurements, profile tracing and cephalometric analysis The post-extraction methods rely on mandibular rest position, facial aesthetic appearance, swallowing pattern, craniofacial landmarks measurements, cephalometric analysis, phonetics and existing dentures In general, all the available techniques have merits and are helpful for routine clinical use However, they are empirical in nature, controversial and lack the scientific support Further, there is no single accurate method for OVD determination To overcome the limitations of the techniques, the clinician will benefit from applying combination of techniques to approximate the OVD

Determination of occlusal vertical dimension for complete dentures patients: an updated review.

Objective. To investigate the relationship between centric relation–maximum intercuspation (CR-MI) discrepancy and temporomandibular joint dysfunction (TMD) in pre-treated orthodontic patients. Material and methods. The study involved an experimental group of 107 pre-treated orthodontic patients with signs and symptoms of TMD aged 18–32 years, and a control group of 70 students with no signs and symptoms of TMD aged 20–30 years. The psychological condition of subjects was evaluated using two standard questionnaires, and a clinical examination performed to assess masticatory musculature and temporomandibular joint (TMJ) function, and to establish the presence or absence of TMD. Helkimo indices, the anamnestic dysfunction index (Ai) and the clinical dysfunction index (Di), were determined. Correlation between centric relation–maximum intercuspation discrepancy and temporomandibular joint dysfunction

Occlusion: Understanding or misunderstanding.

A fifteen-year longitudinal study of rest position and centric occlusion was done on sixteen dentulous subjects with random occlusions. In reviewing the literature the author could find no other investigation with this number of years interim. Records consisted of lateral cephalometric roentgenograms taken when the patient was in rest position and in centric occlusion. Rest position was determined by the patient saying "m" and swallowing, and electromyographically when the anterior belly of the digastric and the temporalis muscles elicited the electrical activity. Both linear and angular measurements were made. The figures from 1958, 1959, and 1960 were averaged and compared with the mean of those taken in 1973. Rest position was found to have a small range of stability in all subjects, while the vertical dimension of centric occlusion increased in ten subjects, decreased in two, and remained the same in four.
