This study determined the prevalence of Group B Streptococcus (GBS) in late pregnancy and the antimicrobial susceptibility of isolated GBS as well as the impact of GBS infections on pregnancy related clinical outcome with a view of providing an epidemiological baseline data for policy formulation in the teaching hospital. It is an observational and cross-sectional hospital based study. One hundred and fifty pregnant women from 35-40 weeks of gestation were purposively selected and included in the study from May to December 2010. Vaginal swab samples were aseptically collected from the subjects after informed consent. The samples were assayed for presence of GBS. The susceptibility pattern of the isolated GBS to different antibiotics were assessed using disc diffusion and agar dilution techniques based on the Clinical and Laboratory Standards institute(CLSI) standards. The result showed prevalence of 11.3% GBS vaginal colonization which increased with age. There was no significant association between GBS colonization status and age (p >0.05)), gestational age (p >0.05)), gravidity (p >0.05) and obstetric risk factors (p >0.05)). There was no incidence of GBS infection observed. Although, all (17) the GBS isolates were 100% resistance to penicillin, ampicillin, cefoxitin and clindamycin. Resistance to cefotaxime (11.8%), erythromycin (64.7%) and vancomycin 70.6% were observed. Group B Streptococcus colonization in vagina in late pregnancy has been established in the antenatal clinic of the teaching hospital with the attendant risk to the fetus in the population of those affected. There were high and multiple resistance patterns of the GBS isolates to different antibiotics in this study. This calls for a review of the present hospital policy to include the routine screening of GBS during antenatal visits and surveillance.

Group B Streptococcus Carriage during Late Pregnancy in Ile-Ife, Nigeria

Background and Objectives: Group B Streptococci (GBS) is a major cause of neonatal and maternal infections. The aim of this study was to determine the serotype distribution and antibiotic resistance profile of GBS strains isolated from pregnant women in Ardabil. Materials and Methods: Antibiotic resistance of 56 GBS isolates was investigated using E-test strips and disk-diffusion method. Serotyping was performed using capsular antiserum. Results: The results of MIC tests showed all isolates were susceptible to ampicillin, vancomycin and penicillin. One isolate (1.7%) showed reduced susceptibility pattern to penicillin (MIC; 0.25 µg/ml). There were 3 (5.3%) isolates semi-sensitive (0.25-1 µg/ml) to erythromycin (2; 0.5 µg/ml and 1; 0.38 µg/ml) and 2 (3.5%) isolates to clindamycin (1; 0.5 µg/ml, 1; 0.38 µg/ml). Additionally, 2 (3.5%) isolates were resistant to clindamycin (1; 16 µg/ml, 1; 2 µg/ml). According to the disk diffusion test, 47 (83.9%), 8 (14.2%) and 7 (12.5%) isolates were resistant to Co-trimoxazole, ciprofloxacin and ceftriaxone respectively. Serotypes V (19.6%), II (12.5%) and IV (12.5%) were the most frequent followed by serotypes III (10.7%) and VI (10.7%), Ib (8.9%), Ia (7/1%), VII (5/3%) and VIII (5/3%); 7.1% of strains were nontypeable. Conclusions: In this study, most isolates were sensitive to common antibiotics, but increased resistance to other antibiotics indicates the importance of monitoring of antibiotic resistance in group B streptococci over time.

Capsular serotype and antibiotic resistance of group B streptococci isolated from pregnant women in Ardabil, Iran

Forty-one Streptococcus agalactiae isolates collected from pregnant women at 35–37 weeks of gestation were analysed for their capsular types, antimicrobial resistance determinants, distribution of virulence factors and genetic relatedness using PCR and multiplex PCR. Capsular type III was predominant (65.8%), followed by capsular type II (14.6%), Ib (7.3%), and V(4.9%). All isolates were susceptible to penicillin, vancomycin, linezolid and quinupristin-dalfopristin. Resistance to tetracycline, erythromycin and clindamycin were found in 97.6%, 24.4%, and 14.6% of isolates, respectively. The most common antimicrobial resistance gene was tetM found in 97.6% of the isolates followed by ermTR and ermB found in 12% and 7.3% of isolates, respectively. Pregnant women colonized by Streptococcus agalactiae (group B streptococci [GBS]) may transfer this microorganism to their newborns. S. agalactiae is an important cause of pneumonia, sepsis, and meningitis in newborns. Fluorescent in situ hybridization (FISH) is considered as a method of identification in the field of diagnostic microbiology. In this paper, we have designed a study to compare the DNA FISH after 7 h Lim broth enrichment and culturing for the identification of S. agalactiae and to determine the prevalence of vaginal colonization by S. agalactiae among pregnant women in Bushehr, Iran. Vaginal swab specimens were obtained from 285 pregnant women at 35 weeks or more than 35 weeks of gestation. The specimens were inoculated into Lim broth. In order to evaluate the sensitivity and specificity of GBS DNA FISH after 7 h Lim broth enrichment, the specimens were tested using both FISH and conventional culture methods. In addition, the prevalence of GBS colonization was determined. Based on the results of this study, both the sensitivity and specificity of FISH were 100%. S. agalactiae was detected by both culture and FISH in 27 of the 285 pregnant women. Thus, the prevalence of GBS colonization was 9.5%. Since short-term (7 h) Lim broth enrichment followed by FISH using oligonucleotide probes showed a high sensitivity and specificity, this protocol is therefore a highly accurate and relatively rapid method for the detection of S. agalactiae. Our analysis suggests that the use of DNA FISH to screen for S. agalactiae colonization in pregnant women may be considered in the absence of GBS culture availability.
