TL;DR: The distribution of lymph node metastases in esophageal adenocarcinoma following neoadjuvant chemoradiation (nCRTx) is unclear, but may have consequences for radiotherapy and surgery, and nCRTx is therefore not a reason to minimize lymphadenectomy in patients with esophagesis.

TL;DR: Until a group of devoted contributors can be put together to put TLM into rigorous scientific testing, it is simply not feasible to consider a trial to test the efficacy and functional outcome of surgical techniques against other larynx-preserving therapies prospectively until such an effort is successfully completed.
