The 11th edition of Harrison's Principles of Internal Medicine welcomes Anthony Fauci to its editorial staff, in addition to more than 85 new contributors. While the organization of the book is similar to previous editions, major emphasis has been placed on disorders that affect multiple organ systems. Important advances in genetics, immunology, and oncology are emphasized. Many chapters of the book have been rewritten and describe major advances in internal medicine. Subjects that received only a paragraph or two of attention in previous editions are now covered in entire chapters. Among the chapters that have been extensively revised are the chapters on infections in the compromised host, on skin rashes in infections, on many of the viral infections, including cytomegalovirus and Epstein-Barr virus, on sexually transmitted diseases, on diabetes mellitus, on disorders of bone and mineral metabolism, and on lymphadenopathy and splenomegaly. Behavioral researchers have increasingly become interested in the idea that chronic, low-grade inflammation is a pathway through which social and behavioral variables exert long-term effects on health. Much research in the area employs putative inflammatory biomarkers to infer an underlying state of inflammation. Interleukin 6 (IL-6) and C-reactive protein (CRP, whose production is stimulated by IL-6) are arguably the two most commonly assayed biomarkers. Yet, in contrast with near-universal assumptions in the field, discoveries in immunology over the past two decades show that neither IL-6 nor CRP are unambiguous inflammatory markers. IL-6 operates through two distinct signaling pathways, only one of which is specifically upregulated during inflammation; both pathways have a complex range of effects and influence multiple physiological processes even in absence of inflammation. Similarly, CRP has two isoforms, one of which is produced locally in inflamed or damaged tissues. The other isoform is routinely produced in absence of inflammation and may have net anti-inflammatory effects. We propose a functional framework to account for the multiple actions of IL-6 and CRP. Specifically, we argue that both molecules participate in somatic maintenance efforts; hence elevated levels indicate that an organism is investing in protection, preservation, and/or repair of somatic tissue. Depending on the state of the organism, maintenance may be channeled into resistance against pathogens (including inflammation), pathogen tolerance and harm reduction, or tissue repair. The findings and framework we present have a range of potential implications for the interpretation of empirical findings in this area-a point we illustrate with alternative interpretations of research on socioeconomic status, stress, and depression.

Regenerative dentistry is an emerging field of medicine involving stem cell technology, tissue engineering and dental science. It exploits biological mechanisms to regenerate damaged oral tissues and restore their functions. Platelet-rich plasma (PRP) is a biological product that is defined as the portion of plasma fraction of autologous blood with a platelet concentration above that of the original whole blood. A super-mixture of key cytokines and growth factors is present in platelet granules. Thus, the application of PRP has gained unprecedented attention in regenerative medicine. The rationale underlies the utilization of PRP is that it acts as a biomaterial to deliver critical growth factors and cytokines from platelet granules to the targeted area, thus promoting regeneration in a variety of tissues. Based on enhanced understanding of cell signalling and growth factor biology, researchers have begun to use PRP treatment as a novel method to regenerate damaged tissues, including liver, bone, cartilage, tendon and dental pulp. To enable better understanding of the regenerative effects of PRP in dentistry, this review describes different methods of preparation and application of this biological product, and provides detailed explanations of the controversies and future prospects related to the use of PRP in dental regenerative medicine.

Platelet‐rich plasma and regenerative dentistry

Context: Platelet-rich-plasma (PRP) and Platelet-rich-fibrin (PRF) are extensively used autologous platelet concentrates in periodontal regeneration, and PRF has a better efficacy as compared to PRP. The rationale for this difference has often been attributed to the difference in the structure of the fibrin matrix. However, the effect of concentration of platelets on the regenerative potential of these concentrates is obscure. Aims: The study was conducted to evaluate and compare, clinically and radiographically, the efficacy of PRF and PRP in the treatment of periodontal endosseous defects and to assess the effect of platelet concentration on periodontal regeneration. Materials and Methods: Twenty intrabony defects were selected and divided into two groups randomly by the coin toss method. Group I received PRP and Group II subjects were treated with PRF. The platelet counts in PRP and PRF were analyzed. Clinical and radiological parameters were assessed at baseline and 3, 6, and 9 months postoperatively. Statistical Analysis: Kruskal–Wallis Chi-square test, Wilcoxon signed rank test, t -test, and Spearman's rank correlation were used for statistical analysis of data. Results: There was statistically significant improvement in all the parameters in the two groups except in relation to gingival recession. There was a statistically significant difference between the platelet count in Group I and Group II ( P = 0.002). Conclusion: PRP and PRF appear to have nearly comparable effects in terms of periodontal regeneration. The concentration of platelets appears to play a paradoxical role in regeneration. The regenerative potential of platelets appears to be optimal within a limited range.

Platelet concentration in platelet concentrates and periodontal regeneration-unscrambling the ambiguity

Abstract The production of bioproducts from microorganisms is a common practice in many industries for a long time now. In recent years, studies have proved that co-culturing microorganisms increase the yield of products by synergistically degrading the solid substrate in comparison with individual cultures. The review highlights the benefits of co-culturing microorganisms using solid state fermentation (SSF) to achieve higher productivity. Filamentous fungi of genus Trichoderma, Penicillium, and Aspergillus are extensively studied and used for co-culturing and mixed culturing under SSF. Co-cultured microorganisms are beneficial because of the synergistic expression of metabolic pathways of all the microorganisms. Co-culture enables combined metabolic activity at optimal process conditions for better utilization of substrates. Depending on the nature of the process and microorganism, bioreactors are designed and operated. This review mentions various purification methods that are used to improve the purity of the products obtained. The strengths and weaknesses of various bioreactors and their effect on the microorganisms used are explained in detail. This review also identifies the challenges of co-culturing microorganisms and analyses the diverse set of fields in which SSF finds its applications. Graphical Abstract

A Critical Look at Bioproducts Co-cultured Under Solid State Fermentation and Their Challenges and Industrial Applications

Dental calculus is a hard deposit that is formed by calcification of dental plaque primarily composed of calcium phosphate mineral salts which is deposited on natural teeth and restorations and is covered by a layer of unmineralized plaque. These hard deposits may form coronal to or apical to the gingival margin, hence named accordingly as supragingival and subgingival calculus respectively. The distribution of calculus is very versatile and it differs from individual to individual, from tooth to tooth, and from surface to surface. So, a thorough knowledge on prevalence of calculus is important for the clinician in outlining the treatment plan. It is a well-known fact that calculus is itself not an inducing agent for pathological changes that occur in gingival tissues; instead it is covered by a layer of unmineralized plaque which is proven to be the key etiological agent involved in these pathogenic mechanisms. But, attributing to the porosity of calculus and its ability to retain bacterial antigens makes it an important contributing factor in initiating and accentuating periodontal disease progression. In this review, we made an attempt to discuss various aspects of calculus composition, its formation, and its etiological significance in periodontal disease progression.

A Comprehensive Review on Dental Calculus

Context: Periodontal disease and diabetes mellitus(DM) share a two - way relationship. It can be hypothesized that successful management of periodontal infection in diabetes will lead not only to reduction of local signs and symptoms of the disease, but also to better control of glucose metabolism.

Aims: To monitor the effect of Scaling and Root planing (SRP) on glycaemic control in patients with type 2 diabetes mellitus by estimating the HbA1c and GCF TNF-α levels.

Settings and Design: This Interventional clinicobiochemical study was carried out over a period of 6 months from December 2010-May 2011 in Bengaluru, Karnataka, India.

Materials and Methods: Fifteen well-controlled, 15 moderately controlled and 15 poorly controlled diabetic subjects were enrolled in this study. All participants were subjected to non-surgical periodontal (SRP) therapy. GCF sampling and clinical periodontal parameters assessment were done at baseline and 3 months post-therapy. TNF-α levels in GCF were analyzed by enzyme-linked immunosorbent assay (ELISA) at baseline and 3 months post therapy. The improvement in glycaemic control was assessed using HbA1c levels at 3 months reevaluation.

Statistical analysis: The data obtained were statistically analysed using Kruskal-Wallis test, Mann-Whitney test and Wilcoxon Signed Rank test.

Results: Following periodontal treatment, all patients demon- strated a significant improvement in periodontal status. A reduction in TNF-α level and the HbA1c values were also observed.

Conclusion: The result indicates that SRP is effective in improving metabolic control in Type 2 Diabetes Mellitus patients possibly through the reduction of TNF-α which in turn might improve the insulin resistance.

Gingival pigmentation occurs in all races of man and it varies from one race to another. Gingival hyperpigmentation usually occurs due to the abnormal accumulation of melanin in the gingival tissue and confers a dark appearance to the gingiva. Gingival hyper-pigmentation is removed by using different procedures of gingival depigmentation. Though the initial results of de-pigmentation procedures are highly encouraging, repigmentation is one of the common issue associated with them. This article aims at reviewing briefly about gingival depigmentation procedures and repigmentation.
