Metabolic surgery can cause amelioration, resolution, and possible cure of type 2 diabetes. Bariatric surgery is metabolic surgery. In the future, there will be metabolic surgery operations to treat type 2 diabetes that are not focused on weight loss. These procedures will rely on neurohormonal modulation related to the gut as well as outside the peritoneal cavity. Metabolic procedures are and will always be in flux as surgeons seek the safest and most effective operative modality; there is no enduring gold standard operation. Metabolic bariatric surgery for type 2 diabetes is more than part of the clinical armamentarium, it is an invitation to perform basic research and to achieve fundamental scientific knowledge.

46 Заболеваемость сахарным диабетом (СД) во всем мире приобрела характер эпидемии. Движущей силой этой эпидемии является высокая, постоянно увеличивающаяся распространенность, а последствием ранняя инвалидность и колоссальное экономическое бремя [1, 2]. В США, к примеру, в 2012 г. на 22,3 млн пациентов с СД было затрачено 306 млрд долларов, или 59% всех расходов на здравоохранение [3]. В Российской Федерации увеличение распространенности СД отмечается за счет СД 2-го типа (СД2): за 5-летний период добавилось 835 тыс. пациентов, что подтверждает глобальные мировые тенденции роста и увеличения доли больных среди общего количества больных СД [1, 3]. Среди причин смертности при СД2 в России ведущие позиции занимает сердечно-сосудистая патология. В общей сложности сердечно-сосудистые заболевания (ССЗ), острое нарушение мозгового кровообращения, инфаркт миокарда и гангрена нижних конечностей являются причиной смерти 51,7% пациентов с СД2 [1]. Создавшееся положение объясняет острую необходимость новой парадигмы гликемического контроля, смысл которой должен заключаться в раннем агрессивном управлении гликемией и прогрессированием факторов риска ССЗ [3]. Последние данные, приведенные И.И. Дедовым и соавт. [1], еще больше подчеркивают эту необходимость: осложнения СД2 зафиксированы у 55% пациентов (гипертония у 37,6%; нейропатия у 19%; ретинопатия у 15,3%; микроангиопатия у 8,3%, диабетическая стопа у 5,6%). Очевидные успехи фармакологии, к сожалению, не в состоянии изменить эту печальную статистику. Поэтому новую страницу в истории лечения СД2 открыли возможности и достижения лапароскопической хирургии. Созданная на ее основе метаболическая хирургия стала новой дисциплиной, включающей вмешательства на органах желудочнокишечного тракта (ЖКТ), связанных с течением и развитием осложнений СД2 [4]. В связи с тем что определяющей характеристикой СД2 является метаболическая дисрегуляция (33,7% пациентов с СД2 имеют избыточную массу тела, 57% — ту или иную степень ожирения и лишь у 9,3% устанавливается нормальная масса тела [4]), становится очевидной роль метаболической хирургии в лечении СД2. Ей предшествовали бариатрические вмешательства, многолетний клинический опыт которых, убедительно доказал высокую эффективность и право на дальнейшую жизнь. При этом, несмотря на убедительные доказательства положительного влияния бариатрической (метаболическая) хирургии на течение СД2, существующие в мировой диабетологии алгоритмы лечения СД до сих пор не включают хирургические методы [5]. Background Obesity, diabetes mellitus may be related to the health, the relationship and the physiological capacity of the production of thyroid hormones (TH), triiodothyronine (T3) and thyroxine (T4). Objectives The main aims of this review are to describe the relationship between obesity, appetite, weight management, hormonal mechanisms of diabetes mellitus and hypothyroidism post-bariatric surgery. Methodology An in-depth literature search was conducted to identify scientific studies, which analyzed the correlation between diabetes mellitus and hypothyroidism post-bariatric surgery. Results Bariatric surgery decreases hypothyroidism, reduces the need for pharmacological action (such as levothyroxine), controls the weight and body fat and increases the sensitivity to leptin and insulin. Conclusion The reduction of the stomach and intestine by bariatric surgery is an evolutionary and beneficial action, because it may lead to a drastic decrease on numbers of conditions such as diabetes, obesity, hypothyroidism, and others. Thus, new studies should also focus on patients' post-operatory conditions, such as lifetime, regulation and functioning of organs after reduced nutrition, and consumption and delivery of nutrients to health maintenance.

Relationship Between Hormonal Mechanisms of Diabetes Mellitus and Hypothyroidism Post-Bariatric Surgery.

Currently more than one third of Americans suffer from obesity. Of greater concern than one’s adiposity are obesity-related comorbidities such as diabetes, hypertension, asthma, infertility, and arthritis, among others. For nearly a century, diets, exercise, behavorial modifications, and drug therapies have proved to be an ineffective treatment. This chapter examines the evolution of bariatric surgery from its creation in the 1950s, with a focus on the effect of surgery on weight loss, comorbidity reduction, and patient safety. Bariatric surgery has been a remarkable success. For the first time, it is possible to curve severe obesity, diabetes, and hyperlipidemia in addition to the many other manifestations of obesity with remarkable safety. Equally important are the opportunities for research afforded by the measurement of outcomes. Until better treatments become available, bariatric surgery is the therapy of choice for patients with morbid obesity for weight control and comorbidity improvement.
