Sunlight plays an etiological role in the formation of skin cancers [Phys. Med. Biol. 24 (1979) 931]. Non-melanoma skin cancers commonly arise in sun-exposed parts of the body, especially on the head and neck regions [Int. J. Dermatol. 34 (6) (1995) 398] although the amount of sun exposure that is required for the formation of skin cancers is still unknown. It is known that the larger the dose of UVR, and in particular the erythemal action spectrum, the more likely it is to form the non-melanoma skin cancers, basal cell carcinomas (BCCs) and squamous cell carcinomas (SCCs). A number of regions on the face exhibit a seemingly high rate of occurrence of BCCs in relation to apparent direct exposure. One of these regions is the inner canthus located next to the eye on the bridge of the nose which has an occurrence rate of 7.1% of all BCCs that occur on the head and neck [Gen. Surg. 51 (6) (1981) 576, Aust. NZ J. Surg. 60 (1990) 855, Malignant Skin Tumours, Longman Singapore Publishers, Singapore, 1991]. The inner canthus seems to be well protected from large direct doses of ultraviolet radiation (UVR) and to explain the higher incidence of BCCs on the inner canthus it is proposed that a significant proportion of the incident UVR on the eye and surrounding areas is reflected onto the inner canthus. This paper presents a preliminary investigation of the contribution of UVR reflected to the inner canthus from the tear film covering the eye using a two-dimensional theoretical model on the horizontal plane (0° elevation angle). Calculations show that up to 30% of the total radiation that is received on the inner canthus on a cellular level in this plane is reflected from the eye. A three-dimensional computer-generated ray tracing model of the eye, surrounding facial features and the inner canthus is being created to investigate the effect that these reflections have on the total dose of UVR.

UVR Reflections at the Surface of the Eye

Exposure to ultraviolet light is associated with several ocular pathologies. Understanding exposure levels and factors is therefore important from a medical and prevention perspective. A review of the current literature on ocular exposure to ultraviolet light is conducted in this study. It has been shown that ambient irradiance is not a good indicator of effective exposure and current tools for estimating dermal exposure have limitations for the ocular region. To address this, three methods have been developed: the use of anthropomorphic manikins, measurements through wearable sensors and numerical simulations. The specific objective, limitations, and results obtained for the three different methods are discussed.

Assessing Human Eye Exposure to UV Light: A Narrative Review

Background/purpose Solar ultraviolet radiation (UVR) doses received by individuals are highly influenced by behavioural and environmental factors. This study aimed at quantifying hats' sun protection effectiveness in various exposure conditions, by predicting UVR exposure doses and their anatomical distributions. Methods A well-defined 3-dimensional head morphology and 4 hat styles (a cap, a helmet, a middle- and a wide-brimmed hat) were added to a previously published model. Midday (12:00-14:00) and daily (08:00-17:00) seasonal UVR doses were estimated at various facial skin zones, with and without hat wear, accounting for each UVR component. Protection effectiveness was calculated by the relative reduction in predicted UVR dose, expressed as a predictive protection factor (PPF). Results The unprotected entire face received 2.5 times higher UVR doses during a summer midday compared to a winter midday (3.3 vs 1.3 standard erythema dose [SED]) with highest doses received at the nose (6.1 SED). During a cloudless summer day, the lowest mean UVR dose is received by the entire face protected by a wide-brimmed hat (1.7 SED). No hat reached 100% protection at any facial skin zone (PPFmax : 76%). Hats' sun protection effectiveness varied highly with environmental conditions and was mainly limited by the high contribution of diffuse UVR, irrespective of hat style. Larger brim sizes afforded greater facial protection than smaller brim sizes except around midday when the sun position is high. Conclusion Consideration of diffuse and reflected UVR in sun educational messages could improve sun protection effectiveness.

Facial exposure to ultraviolet radiation: Predicted sun protection effectiveness of various hat styles

Hawai’i Island often receives extreme (UV Index ≥ 11) solar ultraviolet radiation (UVR). While the UV Index (UVI) has been measured since 1997 at Hawai’i’s high-altitude Mauna Loa Observatory (MLO), measurements where people live and recreate are rare. We measured UVI on the face of a rotating mannequin head with UVR sensors at its eyes, ears and cheeks while simultaneously measuring the UVI with a zenith-facing sensor at MLO and seven sites at or near sea level from 19 July to 14 August 2018. The mannequin sensors received higher UVR at midmorning and midafternoon than at noon. For example, at sea level the peak UVI at the left cheek was 5.2 at midmorning and 2.9 at noon, while the horizontal UVI at noon was 12.7. Our measurements were supplemented with wide-angle (190° and 360°) sky photographs and UV images of the mannequin head. Because the UVI applies to horizontal surfaces, people in tropical and temperate latitudes should be informed that their face may be more vulnerable to UVR at midmorning and midafternoon than at noon. Finally, our instruments provided opportunities to measure unexpected UVR-altering events, including rare biomass smoke over MLO and spectroscopic measurements of substantial UVR-absorbing sulfur dioxide in the eruption plume of the Kilauea volcano.
