TL;DR: Inhalation of low levels of nitric oxide can rapidly and safely decrease pulmonary hypertension in mammals as mentioned in this paper, which can be achieved at therapeutic levels of 20 to 100 ppm and inhibition of reaction of Nitric oxide with oxygen to form toxic impurities such as nitrogen dioxide can provide effective inhalation therapy for pulmonary hypertension.

TL;DR: Specialized iNO machines can be developed to provide the drug intermittently in short bursts at high concentration dose, which would then provide both a preventative drug for those at high risk, as well as an effective treatment, without the health hazards associated with smoking.
