The 10 Elements of Agroecology: enabling transitions towards sustainable agriculture and food systems through visual narratives

There has been growing policy interest in social justice issues related to both health and food. We sought to understand the state of knowledge on relationships between health equity--i.e. health inequalities that are socially produced--and food systems, where the concepts of 'food security' and 'food sovereignty' are prominent. We undertook exploratory scoping and mapping stages of a 'meta-narrative synthesis' on pathways from global food systems to health equity outcomes. The review was oriented by a conceptual framework delineating eight pathways to health (in)equity through the food system: 1--Multi-Scalar Environmental, Social Context; 2--Occupational Exposures; 3--Environmental Change; 4--Traditional Livelihoods, Cultural Continuity; 5--Intake of Contaminants; 6--Nutrition; 7--Social Determinants of Health and 8--Political, Economic and Regulatory context. The terms 'food security' and 'food sovereignty' were, respectively, paired with a series of health equity-related terms. Combinations of health equity and food security (1414 citations) greatly outnumbered pairings with food sovereignty (18 citations). Prominent crosscutting themes that were observed included climate change, biotechnology, gender, racialization, indigeneity, poverty, citizenship and HIV as well as institutional barriers to reducing health inequities in the food system. The literature indicates that food sovereignty-based approaches to health in specific contexts, such as advancing healthy school food systems, promoting soil fertility, gender equity and nutrition, and addressing structural racism, can complement the longer-term socio-political restructuring processes that health equity requires. Our conceptual model offers a useful starting point for identifying interventions with strong potential to promote health equity. A research agenda to explore project-based interventions in the food system along these pathways can support the identification of ways to strengthen both food sovereignty and health equity.

Food sovereignty, food security and health equity: a meta-narrative mapping exercise

The global discourse on food sovereignty suggests several mechanisms for improving food security and agricultural livelihoods, including redistributive land reform and restructuring of markets to improve food distribution and access. In Brazil, the Fome Zero (Zero Hunger) social welfare programme has created innovative links between public nutrition and food security programmes and rural development initiatives through mediated market support for the family farm sector. We report on a participatory assessment of the experience of land reform beneficiaries in seven municipalities in Mato Grosso, Brazil, who were contracted to produce food for the Programa de Aquisicao de Alimentos (Food Procurement Programme, PAA) and the Programa Nacional de Alimentacao Escolar (National School Feeding Programme, PNAE) under the umbrella of Fome Zero. This analysis offers insight into the opportunities and challenges related to participation in mediated ‘farm-to-institution’ food procurement programmes, and assesses their influence on key food sovereignty principles, including agro-ecological transition, increased market stability and farmer autonomy.

Reflexive governance for food security: The example of school feeding in Brazil

ObjectiveThe present paper analyses the advances and challenges of the school feeding programme in Brazil (PNAE), as part of the Brazilian experience building up an integrated food and nutrition security national system. It explores the role of policy and regulatory frameworks in constructing quality service delivery and intersectoral integration.DesignReview of PNAE and federal government technical documents and studies, legislation, minutes of meetings and official documents of the National Council of Food and Nutrition Security from 2003 to 2011.SettingFood insecurity has decreased significantly in Brazil in the last decade, indicating that appropriate choices were made in terms of public policies and institutional arrangements, which other countries can learn from.SubjectsBrazil food and nutrition security system; school feeding; school food.ResultsBrazil's integrated food and nutrition security policy approach promoted intersectorality in the food system, articulating actions to guarantee access to healthy food and to strengthen family farming. The quality of school meals has progressively improved; in particular, the availability of fruits and vegetables increased. However, national standards regarding menu composition have not yet been met. Regulations were an important factor, along with the policy approach linking food production, nutrition, health and education. Challenges are related to conflict of interests and to farmers’ insufficient capacity to meet supply requirements and comply with technical procedures.ConclusionsLocal food production, school meals and nutrition education can be linked through integrated programmes and policies, improving access to healthier foods. Government leadership, strong legislation, civil society participation and intersectoral decision making are determinant.
