Ten-year follow-up of helium ion therapy for uveal melanoma

The antitumor activity of the inorganic complex cis -diammine-dichloroplatinum(II) (cisplatin) led to the development of other types of non-organic cytostatic drugs. Numerous platinum other platinum and non-platinum metal compounds were shown to be effective against animal model tumors as well as tumors in man. However, the introduction of novel transition metal agents in clinical treatment is exceptionally slow. So far, Ru(II) and Ru(III) complexes have shown very promising properties while the Ru(III) compound, [ImH][ trans -Cl 4 (Me 2 SO)(Im)Ru(III)] (Im=imidazole, NAMI-A), is the first ruthenium compound that successfully entered phase I clinical trials. Rhodium belongs to the same group as platinum and ruthenium. However, rhodium compounds, analogues to the corresponding platinum and ruthenium compounds that possess significant antitumor properties, were found to be less effective as anticancer agents mainly due to their toxic effects. Dimeric μ-Acetato dimers of Rh(II) as well as monomeric square planar Rh(I) and octahedral Rh(III) complexes have shown interesting antitumor properties.

Changing Indications in Penetrating Keratoplasty: A Systematic Review of 34 Years of Global Reporting

Many Doppler imaging studies have been performed in recent years in a large number of ocular disorders because of improvements in the Doppler equipment used for detecting and measuring the low blood-flow velocities that are a requisite for the quantitative evaluation of blood flow in the orbital vessels. The ophthalmic artery, central retinal artery and vein, posterior ciliary arteries, and the superior ophthalmic vein can be easily identified using color Doppler sonography. The changes in local blood flow in these vessels assessed by spectral analysis pulsed Doppler sonography have been used to characterize and to obtain new insights into different nontumoral vascular disorders including carotid artery stenosis, central retinal vein occlusion, giant cell arteritis, glaucoma, diabetes, fistulas, and tumoral processes of the eye and orbit. Our experience has confirmed the important role of Doppler sonography in the assessment of subclinical changes in the vascular bed, in the understanding of different processes, for following up after specific treatments, and for determining the long-term prognosis of these various conditions.

In 75% of the patients with malignant melanoma of the choroid useful vision can be retained by beta-radiation with ruthenium-106/rhodium. Tumors with a prominence of more than 6 mm should be treated only for very special reasons. There is a statistically proven correlation between the initial tumor volume and the occurrence of residual tissue volume. Histopathological investigations have shown that residuals of the tumor do not permit any conclusions concerning the dignity or vitality of tumor cells. The risk to life is not increased by the conservative procedure, and it may even be reduced.

The present paper reports on follow-up examinations of 246 patients after Ruthenium106 treatment of choroidal melanomas (follow-up time 1.8 to 13 years). In January 1988, 212 patients were alive and free of metastases; 15 had died, 12 of them of melanoma metastases, and 17 could not be followed up. In a retrospective analysis of factors influencing tumor-related mortality, the rate of regression of irradiated tumors was found to be particularly significant. The mean time taken to reduce tumor volume by half was 4.4 months in patients who died of metastases, as against 6.6 months in a matched group of survivors. Using discriminance analysis, the difference was found to be highly significant (P = 0.0058). In the total patient population, a significant correlation was found between pretreatment tumor volume and the probability of metastases (P = 0.058). It may be concluded, though with the qualification that the number of tumor-related deaths was small, that the tumor regression rate is an important factor in the prognosis for patients with Ruthenium-treated malignant choroidal melanomas.

The ultrasonographic findings in 39 cases of malignant melanoma of the choroid has been correlated with histopathological results. Tumors with hemorrhages and/or necrosis showed a lower reflectivity than those with a higher amount of connection tissue. B-scan ultrasonography was found to be reliable for the measurement of the extent of the tumor in all three dimensions.

Between 1961 and 1968 140 goniotomies were performed at the Department of Ophthalmology, University of Hamburg. The condition of 72 eyes was evaluated and 41 eyes were re-examined. The average intraocular pressure was 33 mmHg preoperativly and only 20 mmHg 13 to 20 years after the goniotomy procedure. Patients from this group with buphthalmus had an even lower pressure of only 17.6 mmHg. Pressure regulation was achieved in 85% of the evaluated eyes by one or more operations, in conjunction with drug therapy in some instances. In 18 of the 41 eyes a satisfactory result was achieved after a single goniotomy procedure. In 15 of the 41 eyes visual acuity was 0.3 or better with normal pressure. The risk of postoperative amblyopia is mentioned.

During the period from 1980 until the beginning of 1986, 1,219 corneal buttons removed in the course of penetrating keratoplasty were histologically examined. In 16.5% (201 cases) a re-keratoplasty was necessary. Indications for a primary corneal transplant (n = 1,018) were keratitis-induced corneal changes (35.0%), dystrophies (30.5%), surgical traumas (17.2%), nonsurgical traumas (11.4%), congenital corneal clouding (3.1%) and degenerations (2.8%). Over the six-and-a-half-year period the incidence of pseudophakic bullous keratopathy was 6.9%, with an increase from 1.6% in 1980 to 12.4% in 1986. Besides an unspecific endothelial cell atrophy, fibroblast-like cell structures, possible macrophages, were found in some specimens by histologic examination on corneal buttons as well as on explanted intraocular lenses.
