The integration of magnetic resonance imaging (MRI) for guidance in external beam radiotherapy has faced significant research and development efforts in recent years. The current availability of linear accelerators with an embedded MRI unit, providing volumetric imaging at excellent soft tissue contrast, is expected to provide novel possibilities in the implementation of image-guided adaptive radiotherapy (IGART) protocols. This study reviews open medical physics issues in MR-guided radiotherapy (MRgRT) implementation, with a focus on current approaches and on the potential for innovation in IGART.Daily imaging in MRgRT provides the ability to visualize the static anatomy, to capture internal tumor motion and to extract quantitative image features for treatment verification and monitoring. Those capabilities enable the use of treatment adaptation, with potential benefits in terms of personalized medicine. The use of online MRI requires dedicated efforts to perform accurate dose measurements and calculations, due to the presence of magnetic fields. Likewise, MRgRT requires dedicated quality assurance (QA) protocols for safe clinical implementation.Reaction to anatomical changes in MRgRT, as visualized on daily images, demands for treatment adaptation concepts, with stringent requirements in terms of fast and accurate validation before the treatment fraction can be delivered. This entails specific challenges in terms of treatment workflow optimization, QA, and verification of the expected delivered dose while the patient is in treatment position. Those challenges require specialized medical physics developments towards the aim of fully exploiting MRI capabilities. Conversely, the use of MRgRT allows for higher confidence in tumor targeting and organs-at-risk (OAR) sparing.The systematic use of MRgRT brings the possibility of leveraging IGART methods for the optimization of tumor targeting and quantitative treatment verification. Although several challenges exist, the intrinsic benefits of MRgRT will provide a deeper understanding of dose delivery effects on an individual basis, with the potential for further treatment personalization.

Medical physics challenges in clinical MR-guided radiotherapy

Over the last few years, magnetic resonance image-guided radiotherapy systems have been introduced into the clinic, allowing for daily online plan adaption. While quality assurance (QA) is similar to conventional radiotherapy systems, there is a need to introduce or modify measurement techniques. As yet, there is no consensus guidance on the QA equipment and test requirements for such systems. Therefore, this report provides an overview of QA equipment and techniques for mechanical, dosimetric, and imaging performance of such systems and recommendation of the QA procedures, particularly for a 1.5T MR-linac device. An overview of the system design and considerations for QA measurements, particularly the effect of the machine geometry and magnetic field on the radiation beam measurements is given. The effect of the magnetic field on measurement equipment and methods is reviewed to provide a foundation for interpreting measurement results and devising appropriate methods. And lastly, a consensus overview of recommended QA, appropriate methods, and tolerances is provided based on conventional QA protocols. The aim of this consensus work was to provide a foundation for QA protocols, comparative studies of system performance, and for future development of QA protocols and measurement methods.

Machine QA for the Elekta Unity system: A Report from the Elekta MR-linac consortium.

End-to-end empirical validation of dose accumulation in MRI-guided adaptive radiotherapy for prostate cancer using an anthropomorphic deformable pelvis phantom.

With the advent of MRI-guided radiotherapy, the suitability of commercially available radiation dose detectors needs to be assessed. The aim of this study was to investigate the effect of the magnetic field (B-field) on the response of the Gafchromic EBT-3 films. Moreover, as an independent study, we contribute to clarifying the inconsistency of the results of recent published studies, on the effect of B-field on the sensitivity of Gafchromic films. A 60Co beam was used to irradiate film samples in an electromagnet. An in-house PMMA phantom was designed to fit in the 5 cm gap between the two poles of the magnet. The phantom consists of two symmetrical plates where a film can be inserted. The absorbed dose rate of the 60Co beam for zero B-field was measured using alanine pellets in a Farmer-type holder. A 12-point response curve was created, representing [Formula: see text] as a function of dose, for each of five different B-field strengths (0 T to 2 T). This study finds that there is at most a small effect of the magnetic field on the response of EBT-3 film. In terms of netOD (red channel) the change in response varied from ‒0.0011 at 0.5 T to 0.0045 at 2.0 T, with a standard uncertainty of 0.0030. If uncorrected, this would lead to an error in film-measured dose, for the red channel, of 2.4% at 2 T, with a standard uncertainty on dose of 1.4%. Results are also presented for B-field strengths of 0.5 T, 1 T and 1.5 T, which are all zero within the measurement uncertainty. Comparison between other studies is also presented. Considering the small change on dose determined with EBT-3 when irradiated under the presence of B-field and taking into account the overall uncertainty in dosimetry using EBT-3 film achieved in this work, EBT-3 is assessed to be a suitable detector for relative and absolute dosimetry, with appropriate corrections, in MRI-guided radiotherapy. The results of the current work also elucidate the inconsistency on the reports from previous studies and demonstrate the necessity of similar investigations by independent teams, especially if the existing results may be in conflict.

The effect of magnetic field strength on the response of Gafchromic EBT-3 film

Anatomical changes during proton therapy require rapid treatment plan adaption to mitigate the associated dosimetric impact. This in turn requires a highly efficient workflow that minimizes the time between imaging and delivery. At the Paul Scherrer Institute, we have developed an online adaptive workflow, which is specifically designed for treatments in the skull-base/cranium, with the focus set on simplicity and minimizing changes to the conventional workflow. The dosimetric and timing performance of this daily adaptive proton therapy (DAPT) workflow has been experimentally investigated using an in-house developed DAPT software and specifically developed anthropomorphic phantom. After a standard treatment preparation, which includes the generation of a template plan, the treatment can then be adapted each day, based on daily imaging acquired on an in-room CT. The template structures are then rigidly propagated to this CT and the daily plan is fully re-optimized using the same field arrangement, DVH constraints and optimization settings of the template plan. After a dedicated plan QA, the daily plan is delivered. To minimize the time between imaging and delivery, clinically integrated software for efficient execution of all online adaption steps, as well as tools for comprehensive and automated QA checks, have been developed. Film measurements of an end-to-end validation of a multi-fraction DAPT treatment showed high agreement to the calculated doses. Gamma pass rates with a 3%/3 mm criteria were >92% when comparing the measured dose to the template plan. Additionally, a gamma pass rate >99% was found comparing measurements to the Monte Carlo dose of the daily plans reconstructed from the logfile, accumulated over the delivered fractions. With this, we experimentally demonstrate that the described adaptive workflow can be delivered accurately in a timescale similar to a standard delivery.

Experimental validation of daily adaptive proton therapy

In the evolving field of adaptive MR guided radiotherapy, the need for dedicated procedures for acceptance and quality assurance is increasing. Research has been devoted to MR compatible dosimeters and phantoms, but to date no end-to-end test has been presented that covers an MRgRT workflow. Such an end-to-end test should comprise each step of the workflow and include all associated uncertainties. The purpose of this study was to investigate the usability of an anthropomorphic deformable and multimodal pelvis (ADAM-pelvis) phantom in combination with film dosimetry for end-to-end testing of an MRgRT adaptive workflow. The ADAM-pelvis phantom included surrogates for muscle tissue, adipose and bone, as well as deformable silicone organs mimicking a prostate patient. At the interfaces of the critical structures (bladder and rectum), small pieces of GafChromic EBT3 films were placed to measure delivered dose. Pre-treatment MR imaging of the phantom was used to delineate the prostate, rectum and bladder and to generate a treatment plan to deliver 2 Gy to the prostate. Electron density (ED) map from CT imaging was used for dose calculation after deformable image registration (DIR) to the pre-treatment MR scan. At each fraction, bladder- and rectum filling was varied and a new adapted plan was generated. Dose calculation was performed using both a DIR-based ED map and a CT-based ED map after acquisition of a new CT scan of the phantom at each fraction. All dose calculations were performed taking into account the magnetic field. A good agreement between measured and calculated dose was found using both, the CT-derived and the DIR-based ED map (2.0% and 2.8% dose difference, respectively). The gamma index pass-rate (3%/2 mm) varied from 96.4% to 100%.The ADAM-pelvis phantom was suitable for end-to-end testing in MR-guided radiotherapy and a very good agreement with the calculated dose was achieved.

A film-based end-to-end test for MR-guided online adaptive radiotherapy

The 0.35 T 60Co MRIdian system (ViewRay Inc., Mountain View) has been in clinical use in our institution since May 2016. For quality assurance (QA) of dose delivery and end-to-end testing for this machine, a reliable dosimeter is required. However, it is possible that a magnetic field may cause perturbations to dosimetry measurements. For static magnetic fields, there is conflicting information in the literature concerning EBT film behaviour, while for real-time MR imaging such information is not available at all. The purpose of this study was to investigate the suitability of EBT3 GafChromic film for MRIdian QA, both with and without real-time MR imaging. EBT3 film sheets were irradiated in water using the MRIdian and a conventional linear accelerator (Linac) for reference. Dose calibration measurements were first performed up to 8 Gy for both machines. The MRIdian measurements were performed with and without real-time MR imaging. Second, film sheets were irradiated at seven different angles with respect to the B0-field. Optical density and dose values were analysed for the three colour channels. In both the film dose-response and B0-field orientation measurements, the mean dose values were within the 1% uncertainty range of prescribed dose values for the red and green channels, for both machines. There were no dose deviations detected between the MRIdian and Linac film measurements, nor for different B0-field orientations. In addition, the film dose-response measurements during real-time imaging were within 1.5% of the reference Linac measurements. EBT3 GafChromic film can be used for absolute dosimetry during real-time MR imaging independent of its orientation in the B0-field. This makes it a suitable dosimeter for patient-specific QA measurements and end-to-end testing of 0.35 T MRI-radiotherapy devices.
