The recent advancements in the field of laser-driven particle acceleration have made Laser-driven Ion Beam Therapy (L-IBT) an attractive alternative to the conventional particle therapy facilities. To bring this emerging technology to clinical application, we introduce the broad energy assorted depth dose deposition model which makes efficient use of the large energy spread and high dose-per-pulse of Laser Accelerated Protons (LAP) and is capable of delivering homogeneous doses to tumors. Furthermore, as a key component of L-IBT solution, we present a compact iso-centric gantry design with 360° rotation capability and an integrated shot-to-shot energy selection system for efficient transport of LAP with large energy spread to the patient. We show that gantry size could be reduced by a factor of 2–3 compared to conventional gantry systems by utilizing pulsed air-core magnets.

A compact solution for ion beam therapy with laser accelerated protons

The past few years have seen significant developments both of the technologies available for proton and other charged particle therapies, and of the number and spread of therapy centres. In this review we give an overview of these technology developments, and outline the principal challenges and opportunities we see as important in the next decade. Notable amongst these is the ever-increasing use of superconductivity both in particle sources and for treatment delivery, which is likely to greatly increase the accessibility of charged particle therapy treatments to hospital centres worldwide.

Current and future accelerator technologies for charged particle therapy

Particle therapy of moving targets is still a great challenge. The motion of organs situated in the thorax and abdomen strongly affects the precision of proton and carbon ion radiotherapy. The motion is responsible for not only the dislocation of the tumour but also the alterations in the internal density along the beam path, which influence the range of particle beams. Furthermore, in case of pencil beam scanning, there is an interference between the target movement and dynamic beam delivery. This review presents the strategies for tumour motion monitoring and moving target irradiation in the context of hadron therapy. Methods enabling the direct determination of tumour position (fluoroscopic imaging of implanted radio-opaque fiducial markers, electromagnetic detection of inserted transponders and ultrasonic tumour localization systems) are presented. Attention is also drawn to the techniques which use external surrogate motion for an indirect estimation of target displacement during irradiation. The role of respiratory-correlated CT [four-dimensional CT (4DCT)] in the determination of motion pattern prior to the particle treatment is also considered. An essential part of the article is the review of the main approaches to moving target irradiation in hadron therapy: gating, rescanning (repainting), gated rescanning and tumour tracking. The advantages, drawbacks and development trends of these methods are discussed. The new accelerators, called "cyclinacs", are presented, because their application to particle therapy will allow making a breakthrough in the 4D spot scanning treatment of moving organs.

Particle therapy of moving targets—the strategies for tumour motion monitoring and moving targets irradiation

Recent developments for the delivery of proton and ion beam therapy have been significant, and a number of technological solutions now exist for the creation and utilisation of these particles for the treatment of cancer. In this paper we review the historical development of particle accelerators used for external beam radiotherapy and discuss the more recent progress towards more capable and cost-effective sources of particles.

Technologies for delivery of proton and ion beams for radiotherapy

The type of the Electron String Ion Sources (ESIS) is considered to be the appropriate one to produce pulsed C4+ and C6+ ion beams for cancer therapy accelerators. In fact, the new test ESIS Krion-6T already now provides more than 1010 C4+ ions per pulse and about 5 × 109 C6+ ions per pulse. Such ion sources could be suitable to apply at synchrotrons. It has also been found that Krion-6T can provide more than 1011 C6+ ions per second at the 100 Hz repetition rate, and the repetition rate can be increased at the same or larger ion output per second. This makes ESIS applicable at cyclotrons as well. ESIS can be also a suitable type of ion source to produce the 11C radioactive ion beams. A specialized cryogenic cell was experimentally tested at the Krion-2M ESIS for pulse injection of gaseous species into the electron string. It has been shown in experiments with stable methane that the total conversion efficiency of methane molecules to C4+ ions reached 5%÷10%. For cancer therapy with simultaneous irradiation and precise dose control (positron emission tomography) by means of 11C, transporting to the tumor with the primary accelerated 11C4+ beam, this efficiency is preliminarily considered to be large enough to produce the 11C4+ beam from radioactive methane and to inject this beam into synchrotrons.

Electron string ion sources for carbon ion cancer therapy accelerators

Charged particle therapy, or so-called hadrontherapy, is developing very rapidly. There is large pressure on the scientific community to deliver dedicated accelerators, providing the best possible treatment modalities at the lowest cost. In this context, the Italian research Foundation TERA is developing fast-cycling accelerators, dubbed `cyclinacs'. These are a combination of a cyclotron (accelerating ions to a fixed initial energy) followed by a high gradient linac boosting the ions energy up to the maximum needed for medical therapy. The linac is powered by many independently controlled klystrons to vary the beam energy from one pulse to the next. This accelerator is best suited to treat moving organs with a 4D multipainting spot scanning technique. A dual proton/carbon ion cyclinac is here presented. It consists of an Electron Beam Ion Source, a superconducting isochronous cyclotron and a high-gradient linac. All these machines are pulsed at high repetition rate (100–400 Hz). The source should deliver both C6+ and H2+ ions in short pulses (1.5 μs flat-top) and with sufficient intensity (at least 108 fully stripped carbon ions per pulse at 300 Hz). The cyclotron accelerates the ions to 120 MeV/u. It features a compact design (with superconducting coils) and a low power consumption. The linac has a novel C-band high-gradient structure and accelerates the ions to variable energies up to 400 MeV/u. High RF frequencies lead to power consumptions which are much lower than the ones of synchrotrons for the same ion extraction energy. This work is part of a collaboration with the CLIC group, which is working at CERN on high-gradient electron-positron colliders.
