Retrognathic mandibles that must be, moved both anteriorly and inferiorly in order to correct any abnormality can be sectioned on an arc through the rami. An arcing osteotomy was described which allows the mandibular body to be moved inferiorly and anteriorly to the new occlusal relationship and to maintain maximum and exact bone contact. The final occlusal position determines the configuration of the osteotomy to be used. The technique involves the use of presurgical occlusal adjustment and individually cast metal arch bars for stabilizing the postoperative occlusion.

Publisher Summary This chapter discusses the different aspects of speech and orthognathic surgery. Orthognathic surgery is the surgical correction of hard tissue abnormalities of the mandible and maxilla. Orthodontic treatment may help correct abnormalities in occlusion by moving teeth, but extreme symmetrical and asymmetrical abnormalities of the mandible and maxilla are uncorrectable by routine orthodontic treatment alone. Orthognathic surgery may correct a protruded or retruded maxilla, a prognathic jaw, or a micrognathic jaw. The surgical corrections perform both a cosmetic and functional purpose. A majority of the surgical corrections are sought by the patient for cosmetic reasons. Surgically induced changes in the length of the mandible provide a unique test of the importance of extrinsic laryngeal control of frequency of voice, as moving the mandible moves the supralaryngeal muscles and the hyoid/larynx complex itself. During the period of intermaxillary fixation verbal communication, breathing, eating, coughing, and other vegetative functions are profoundly altered. Some of the most notable speech changes, although quite variable from subject to subject, nevertheless typify the problems in communication found by the orthognathic surgery patient.

A surgical procedure involving the advancement of the mandible for correction of retrognathia with simultaneous retraction of the mandibular alveolus is described. Indications for treatment and reports of clinical cases are Included. A mandibular subapical osteotomy was used simultaneously with a mandibular sagittal split to correct mandibular alveolar protrusion and extrusion and mandibular retrusion in three patients. A good esthetic and functional result was obtained. Indications for treatment and a description of the combined surgical procedure are given.
