Chronic health conditions have been on the rise across the world. But few health care systems, have proved able to cope with managing this dramatic increase in chronic conditions. This report from the World Health Organization (WHO) provides a comprehensive framework for the prevention framework for the prevention and management of Chronic health conditions around the world. Chronic health conditions were defined in this report as “health problems that persist across time and require some degree of health care management”. This new report addresses how policy makers can take effective and innovative action to tackle these conditions. The report addresses the following issues: Exploring chronic conditions, including how and why they are increasing, what is their economic impact on patients, families, communities, and governments, and how to reduce their prevalence. The deficits in current systems of health to successfully manage chronic conditions because the acute care model dominates health care systems. The acute care model increased health care expenditures without improving populations health status. A new framework for health care systems to improve care for chronic conditions. This framework is comprised of fundamental components within the patient, health care organization and community, and policy levels. These components were described as “building blocks” that can be used to re-design a health care system to manage long-term health problems more effectively. The report describes eight essential elements for improving and creating innovations in the care of chronic conditions. These elements include: Supporting a shift in attitudes, so that decision­ makers, health workers and patients are aware that a shift in resources from the acute, episodic model of care is needed to meet the different of chronic conditions. Building political commitment to the process of preventing and managing chronic conditions. Avoiding fragmentation of services, to ensure that the care for chronic conditions is integrated. To align and analyze policies which affect chronic conditions, such as agricultural regulations, labor laws and health education. To use new, team care models and evidence-based skills for managing chronic conditions, worth more efficient use of health care personnel. To re-oriented Health care for chronic conditions around the patient and family. To support patients within their communities, because communities can fill a crucial gap in health services that are not provided by organized health care. To emphasize prevention, as many of the complications of chronic conditions can be prevented or reduced. This report provides a well-formulated framework for decision-makers and health service planners to implement strategies for comprehensively managing chronic conditions.

The Meaning of Everyday Occupation

Due to low employment rates associated to chronic conditions in Europe, it is essential to foster effective integration and re-integration into work strategies. The objective of this systematic review is to summarize the evidence on the effectiveness of strategies for integration and re-integration to work for persons with chronic diseases or with musculoskeletal disorders, implemented in Europe in the past five years. A systematic search was conducted in MedLine, PsycINFO, CDR-HTA, CDR-DARE and Cochrane Systematic Reviews. Overall, 32 relevant publications were identified. Of these, 21 were considered eligible after a methodological assessment and included. Positive changes in employment status, return to work and sick leave outcomes were achieved with graded sickness-absence certificates, part-time sick leave, early ergonomic interventions for back pain, disability evaluation followed by information and advice, and with multidisciplinary, coordinated and tailored return to work interventions. Additionally, a positive association between the co-existence of active labour market policies to promote employment and passive support measures (e.g., pensions or benefits) and the probability of finding a job was observed. Research on the evaluation of the effectiveness of strategies targeting integration and re-integration into work for persons with chronic health conditions needs, however, to be improved and strengthened.

Effectiveness of Integration and Re-Integration into Work Strategies for Persons with Chronic Conditions: A Systematic Review of European Strategies

Background: Employees’ well-being at work after the return to work (RTW) is considered a key aspect of rehabilitation and maintenance of workability. This systematic review aimed at identifying the common psychosocial factors that predict the subjective and psychological well-being in RTW processes after having a long-standing health problem or disability. Objective: To evaluate the subjective and psychological well-being at work of employees with chronic or long-standing health problems or those returning to work after any cause of disability. Data source: Systematic review of articles published in English or Spanish using PsycINFO, PsycARTICLES, MEDLINE, Psychology, and Behavioral Sciences Collection, and Pubpsych. An additional study was identified by contacting expert academics in the field. The search equations used included terms such as Return to Work, Long-Standing Health Problems or Disability, Work Health Balance, and job satisfaction or subjective well-being. Eligibility criteria for the studies: Studies that included a measure of employees’ well-being at work following return to work were selected for the review. Evaluation of the studies and synthesis methods: The studies were selected using predefined fields which included quality criteria. Results: Of the 264 articles returned by the initial search, a total of 20 were finally selected. Results were organized around the three different theoretical approaches for understanding RTW and its antecedents and consequences: (a) RTW and autonomy at work have a positive effect on psychological well-being; (b) job demand is linked to less job satisfaction, whereas a higher level on the work–health balance is associated with job satisfaction and work engagement; (c) internal and external support is linked to job satisfaction in the case of a disease. Limitations: The evidence provided by the results is restricted by the limited availability of studies focusing on well-being at work following return to work. Moreover, the studies identified are of different kinds, thereby preventing comparisons. Conclusions and implications of the main findings: Employees’ subjective well-being after return to work has received very little attention to date. Given its importance in the current configuration of the labor market, it should be the object of more research.

Well-Being at Work after Return to Work (RTW): A Systematic Review.

Chronic health conditions represent the major share of the disease burden in Europe and have a significant impact on work. This study aims to: (1) identify factors that have a negative or positive impact on the work lives of persons with chronic health conditions; (2) explore the needs of these persons to maintain a job or return to work and (3) compare these results with respect to these persons' occupational status. An online survey was performed in seven European countries. Open-ended survey questions were analyzed using qualitative methods. In total, 487 participants with six chronic health conditions participated. The majority of participants named work-related aspects (such as career development, stress at the workplace, work structure and schedule as well as workload), support of others and attitudes of others as being the factors positively and negatively impact their work lives the most. Our study shed light on the importance of changing the attitudes of supervisors and co-workers to counteract stigmatization of persons with chronic health conditions in the workplace. In conclusion, this study provides a basis for developing new strategies of integration and reintegration at work for persons with chronic health conditions in European countries.
