Statement of problemAlthough the intraoral scanning of edentulous ridges is feasible, clinical evidence that the resulting denture retention is equivalent to that achieved with conventional impressions is lacking.PurposeThe purpose of this clinical study was to determine the retention of complete denture bases fabricated from digital intraoral scans versus conventional impressions by using border molding and posterior palatal seal compression.Material and methodsTwenty volunteers with an edentulous maxilla were recruited. An intraoral scan of the maxilla and a conventionally border-molded impression with a custom tray were made. The conventional impression was poured; the definitive cast was scanned. Three-dimensionally (3D) printed (PB1) and milled bases (MB1) were fabricated based on the scan of the definitive cast. Based on the intraoral scan, a 3D-printed (PB2) and a milled base (MB2) were fabricated. On each base, a platform with a hook consisting of a central notch orienting the force against the post dam (PD) and 2 lateral notches orienting the forces against the left (LT) and right (RT) tuberosities was set in the center of the outer surface of the base. A traction dynamometer was inserted in the hook and oriented into the corresponding notch by applying force until dislodgement. All bases were subsequently stored in artificial saliva for 2 weeks and scanned. Retention testing was repeated by using the same procedure. To evaluate trueness and to visualize the differences on a color map, the scan of the definitive cast and the intraoral scans were matched and compared in 3 dimensions. The Wilcoxon tests were used to compare the retention of the different bases (95% confidence interval, α=.05).ResultsNineteen participants with a mean ±standard deviation age of 64.1 ±14.7 years completed the 4 study sessions. The retention of printed bases (PD: 16.08 ±15.28 N; LT: 14.98 ±14.72 N; RT: 11.28 ±9.57 N) and milled bases (PD:14.52 ±17.07 N; RT: 11.99 ±12.10 N; LT: 13.55 ±15.53 N) fabricated from conventional impressions presented significantly higher retentive forces than those printed (PD: 6.21 ±4.72 N; RT:5.12 ±2.78 N; LT: 4.45 ±2.77 N) and milled (PD: 6.58 ±4.92 N; RT: 4.65 ±2.63 N; LT: 5.02 ±3.58 N) from the intraoral scans (P<.05). The differences were significant in all directions of dislodgement, as well as after storage in artificial saliva for 2 weeks. Comparison of the 3D distances between the intraoral scan and the definitive cast revealed a mean deviation of 0.45 ±0.11 mm.ConclusionsConventional impressions of the edentulous maxilla, including the clinical steps of border molding and posterior palatal seal compression, provide better retention than digital intraoral scans with both milled and 3D-printed denture bases.

It is a worldwide challenge in child health as treatment outcomes for metastatic and recurrent disease still pose a major concern for both basic and clinical scientists. The treatment strategies for rhabdomyosarcoma include multi-agent chemotherapies after surgical resection with or without ionization radiotherapy. In this comprehensive review, we first provide a detailed clinical understanding of rhabdomyosarcoma including its classification and subtypes, diagnosis, and treatment strategies. Later, we focus on chemotherapy strategies for this childhood sarcoma and discuss the impact of three mechanisms that are involved in the chemotherapy response including apoptosis, macro-autophagy, and the unfolded protein response. Finally, we discuss in vivo mouse and zebrafish models and in vitro three-dimensional bioengineering models of rhabdomyosarcoma to screen future therapeutic approaches and promote muscle regeneration. 

Rhabdomyosarcoma: Current Therapy, Challenges, and Future Approaches to Treatment Strategies

Simple Summary Rhabdomyosarcoma (RMS) is a rare pediatric sarcoma affecting skeletal muscle in children and young adults. It is responsible for 3% of all childhood malignant tumors and is the third most prevalent pediatric extracranial solid tumor. Despite advances in diagnostic and treatment methods and clinical trials to improve pediatric RMS survival rates, children with high-risk RMS and recurrent disease have 5-year survival rates of less than 30% and 17%, respectively. The cure rate remains low and the current RMS therapies continue to pose potential life-threatening toxicities, which can lead to lifelong morbidity. The treatment strategies for RMS include multi-agent chemotherapies after surgical resection with or without radiotherapy. Here, we focus on chemotherapy strategies and discuss the impact of apoptosis, autophagy, and the UPR that are involved in the chemotherapy response. Then, to screen future therapeutic approaches and promote muscle regeneration, we discuss in vivo mouse and zebrafish models and in vitro three-dimensional bioengineering models. This study aimed to evaluate the efficiency of a digital workflow by comparing the accuracy of prosthetic teeth positioning between virtual standard-size digitally constructed and conventional dental laboratory-fabricated prostheses.Twenty-five computed tomography datasets with a dentate upper jaw were selected after applying inclusion criteria to 100 random datasets obtained from the institutional library, and partially edentulous maxillae were constructed virtually. Digital datasets of temporary prostheses were fabricated on these virtually constructed edentulous maxillae in two ways: one dataset comprised prostheses that were fabricated conventionally using prosthetic teeth and wax in the dental laboratory and then scanned using a model scanner, whereas the other dataset was designed virtually using standardized virtual dental arches. The digital datasets of both prostheses were compared for differences at six dental-based measurement points with the original patient dentition.Overall, the conventional design pathway was more accurate than the digital one (conventional 2.915 ± 1.388 mm, digital 3.609 ± 2.052 mm, P < 0.001). However, when all six measurement points were evaluated individually, only three points showed significant differences in the tooth positions. Compared with the original dentition, the deviations were less in the anterior teeth region than in the molar region, fulfilling the esthetic expectations of the patients. Standardized virtual dental arches were practically adequate because virtual reconstruction of every edentulous case using these virtual arches was possible without any additional modifications.It is possible to fabricate clinically acceptable temporary prostheses using a comprehensive digital workflow based on standardized digital dental arches.

Comparison of conventional and digital workflow for dental rehabilitation with a novel patient-specific framework implant system: an experimental dataset evaluation

A flipper is an interim removable partial denture used for immediate esthetic restoration and space maintenance for a limited period before definitive treatment. Traditional methods for fabricating flippers are labor-intensive and manual. With the advent of technological advancement in dentistry, fully digitalized fabrication including computer designing and three-dimensional (3D) printing can be used. This study aimed to demonstrate the complete digital workflow for fabricating flippers and compare the features of different digital methods. In a partially edentulous patient, three flippers were fabricated using different digital protocols: individual printing (tooth and base parts) and combination, i.e., one-body printing and gingival layering and one-body printing and gingival coloring. Each flipper was tried into the oral cavity, and the adaptation of flippers was evaluated using the triple-scan technique. This study confirms that fully digital fabrication of a flipper is an effective method compared with traditional methods and shows various digital workflows available in 3D printing. Thus, an appropriate fabrication method must be selected for each situation.

Fully Digitalized Workflow of Flipper Fabrication: Different Three-Dimensional Printing Strategies and Characteristics

Fabricating an immediate complete denture can be very challenging in some clinical situations. This clinical report describes a digital workflow to fabricate a printed maxillary immediate complete denture for a patient with a severely compromised maxilla. Digital data obtained by using an intraoral scanner was utilized to reconstruct the three-dimensional (3D) image of the jaws at the desirable vertical dimension of occlusion. After performing the virtual teeth extraction and alveoloplasty, the denture base and teeth were designed. The resulting data were exported to a 3D printer for denture fabrication and the 3D printed (additively manufactured) denture was successfully inserted immediately after the surgery. After initial healing and confirmation of good retention and function, a new printed denture was fabricated by digitally duplicating the relined denture maintaining the same teeth positions but adjusting the base to a new intraoral scan of the healed ridge.
