The association of body composition with the risk of gestational diabetes mellitus in Chinese pregnant women: A case-control study.

Structural differences in dietary fatty acids modify their rate of oxidation and effect on satiety, endpoints that may influence the development of obesity. This study tests the hypothesis that meals containing fat sources with elevated unsaturated fats will result in greater postprandial energy expenditure, fat oxidation, and satiety than meals containing fats with greater saturation. In a randomized, 5-way crossover design, healthy men and women (n = 23; age: 25.7 ± 6.6 years; BMI: 27.7 ± 3.8 kg/m2) consumed liquid meals containing 30 g of fat from heavy cream (HC), olive oil (OO), sunflower oil (SFO), flaxseed oil (FSO), and fish oil (FO). Energy expenditure and diet-induced thermogenesis (DIT) were determined by metabolic rate over a 240 min postprandial period. Serum concentrations of ghrelin, glucose, insulin, and triacylglycerol (TAG) were assessed. DIT induced by SFO was 5% lower than HC and FO (p = 0.04). Energy expenditure and substrate oxidation did not differ between fat sources. Postprandial TAG concentrations were significantly affected by fat source (p = 0.0001). Varying fat sources by the degree of saturation and PUFA type modified DIT but not satiety responses in normal to obese adult men and women.

Dietary Fat Chain Length, Saturation, and PUFA Source Acutely Affect Diet-Induced Thermogenesis but Not Satiety in Adults in a Randomized, Crossover Trial.

BACKGROUND
the evaluation of hydration status and body composition in patients with kidney disease is vital for proper management, since overhydration is associated with cardiovascular complications. Patients with chronic kidney disease (CKD) begin to show perceptible alterations in hydration during the intermediate stages of the disease; however, there is little information regarding the evaluation of blood volume status through bioelectrical impedance vector analysis (BIVA) in this population.


OBJECTIVE
to determine the association between hydration status measured with BIVA and biochemical and clinical parameters and mortality in patients with stage G3a, G3b and G4 CKD.


MATERIAL AND METHODS
A cross-sectional study was conducted with patients with stage G3a, G3b and G4 CKD who underwent bioelectrical impedance analysis (BIA). The following biochemical and clinical parameters were determined: serum and urinary albumin, hematocrit, serum electrolytes and creatinine, estimated glomerular filtration rate (eGFR, using the CKD-EPI formula), 24-hour urine output and blood pressure. The clinical and biochemical variables were associated with the components of the BIA. According to the resistance/height (R/H) and reactance/height (Xc/H) values, the BIVA results were individually plotted on reference ellipses to identify patients with abnormal hydration states. The patients were classified by group according to hydration status and CKD stage z-scores, and differences in clinical, biochemical and BIA parameters were identified. Mortality was determined by hydration status.


RESULTS
a total of 138 subjects, 69 men and 69 women, were studied. An association was found between the BIVA components (R/H, Xc/H and phase angle (PA) and serum albumin (albumin and R/H, r = -0.38, p = 0.001; Xc, r = 0.59, p = 0.000; PA, r = 0.58, p ≤ 0.0010). When the biochemical and clinical parameters were compared by hydration status, significant differences were found in eGFR (p = 0.01), serum calcium (p ≤ 0.001), serum albumin (p ≤ 0.001), hemoglobin (p = 0.04), hematocrit, (p = 0.04) and mean arterial pressure (p = 0.03). The patients were followed for a median of 65.5 months (IQR: 53.0 to 207.0), and 12 (8.6 %) patients with CKD died. The Kaplan-Meier curves showed that patients with overhydration had a significantly higher risk of death than patients with normal hydration.


CONCLUSIONS
there is an association between the hydration status evaluated by BIVA and clinical and biochemical variables. Patients with overhydration are significantly more likely to die than patients with normal hydration.

The use of non- and low-caloric sweetener(s) (NCS and LCS) as a means to prevent overweight and obesity is highly debated, as both NCS and LCS have been proposed to have a negative impact on energy homeostasis. This systematic review aimed to assess the impact of NCS and LCS on fasting and postprandial substrate oxidation, energy expenditure, and catecholamines, compared to caloric sweeteners or water, across different doses and types of NCS and LCS, acutely and in the longer-term. A total of 20 studies were eligible: 16 studies for substrate oxidation and energy expenditure and four studies for catecholamines. Most studies compared the acute effects of NCS or LCS with caloric sweeteners under non-isoenergetic conditions. These studies generally found higher fat oxidation and lower carbohydrate oxidation with NCS or LCS than with caloric sweeteners. Findings for energy expenditure were inconsistent. With the limited number of studies, no convincing pattern for the remaining outcomes and comparisons could be seen. In conclusion, drinks or meals with NCS or LCS resulted in higher fat and lower carbohydrate oxidation compared to caloric sweeteners. No other conclusions could be drawn due to insufficient or inconsistent results. Further studies in this research field are warranted.

Effect of Non- and Low-Caloric Sweeteners on Substrate Oxidation, Energy Expenditure, and Catecholamines in Humans—A Systematic Review

The modern diet is closely linked to the consumption of processed foods, causing an increase in the intake of salt, simple sugars, phosphorus and added potassium. This excess intake is associated with an increased risk of obesity, diabetes, hypertension and chronic kidney disease (CKD). CKD, which according to data from the ENRICA study affects 15% of the population, magnifies its impact due to the higher prevalence of diabetes and hypertension and due to limitations in the management of sodium and phosphorus. The intake of these products far exceeds the established recommendations, assuming 72% of total sodium, 25-35% of phosphorus, 12-18% of potassium and exceeding 10% of the caloric intake in simple sugars. Measures are necessary to reduce their contribution through nutritional advice, labeling review, education campaigns on healthy habits, fees and institutional actions that involve food safety agencies, industry, distribution and scientific societies.

Validity of bioelectrical impedance vector anlysis (BIVA) to assess total body water (TBW) in women before, during pregnancy and post partum (PP)

The macronutrient composition of the diet may play a more important role in maintaining a healthy body weight and preventing obesity than previously thought. The primary goal of this research was to determine the extent to which the simple addition of a small serving of a sugar-sweetened beverage (SSB) to meals with different macronutrient compositions impacts appetite, energy metabolism and substrate oxidation. Appetite, energy metabolism and substrate oxidation were measured in 27 healthy weight adults (age = 23 ± 5 y; BMI = 23 ± 2 kg/m2) on two occasions in a room calorimeter after consuming a SSB or a non-nutritive-sweetened beverage (NNSB) with a standard (15%E) or high- (30%E) protein meal. Meal carbohydrate (CHO) content was adjusted to maintain equivalent calories for both study visits. All meals were composed of the same foods and provided 17 g of fat and 500 non-beverage calories. Study visits were separated by at least 1 week and menstruating females were studied during the luteal phase (Days 15–20). The effects of sex, protein level and beverage type and their interactions on satiety, appetite for foods with specific taste profiles, diet-induced thermogenesis (DIT) and rates of substrate oxidation were assessed using a 3-way Repeated Measures Analysis of Variance. Increasing dietary protein decreased hunger and increased satiety. Males were hungrier and less satisfied with the meals than females. Increasing dietary protein also decreased the desire to eat something savory, salty and fatty and the males had a greater appetite for food with these taste profiles. Interestingly, there was no effect of sex, dietary protein or beverage type on the desire to eat something sweet. The inclusion of a SSB markedly suppressed DIT (2.42% ± 5.91%) and fat oxidation (9.87 ± 11.09 g). Appetite sensations, food preferences, energy expenditure and substrate oxidation are significantly altered in response to changes in meal macronutrient composition produced by modifications in the protein content of a meal and consumption of a SSB. Most notably, consumption of a SSB during a meal markedly reduces energy efficiency and fat oxidation independent of macronutrient composition. ClinicalTrials.gov: NCT02211599

Postprandial energy metabolism and substrate oxidation in response to the inclusion of a sugar- or non-nutritive sweetened beverage with meals differing in protein content

Background There is conflicting evidence regarding the role of angiotensin-converting enzyme inhibitors and physical function. While some studies show improvements in muscle strength and physical function, others show no significant difference or decreased performance. This ambiguity could be due to differential effects of angiotensin-converting enzyme inhibitor subtypes which can be categorized as centrally or peripherally-acting based upon their ability to cross the blood-brain barrier. Objective The objective of this study is to compare physical performance measures among angiotensin-converting enzyme inhibitor subtype users. Methods Design: Cross-sectional Setting: Ambulatory Participants: Performed in 364 participants in the Health and Retirement Study cohort who were ≥ 65 years (median age (IQR) 74.00 (69–80) years. Measurements: Average difference in hand grip (kg), gait speed(m/s) and peak expiratory flow (L/min). Results Compared to participants on a peripherally-acting angiotensin-converting enzyme inhibitor (113 (31%)), those on a centrally-acting agent (251(69%)) had stronger grip strength 28.9 ±1.0 vs 26.3±1.0, p=.011 and higher peak expiratory flow rates 316.8±130.4 vs. 280.0±118.5, p=.011 in unadjusted analysis. After multiple adjustments the difference in PEF remained statistically significant (Estimate(CI) 26.5, 95% CI 2.24, 50.5, p = 0.032). Conclusion Our results suggest that in older adults the use of centrally-acting angiotensin-converting enzyme inhibitors compared to a peripherally acting angiotensin-converting enzyme inhibitors was associated with better lung function in older individuals.
