Since the majority of patients with gastric cancer are diagnosed at advanced stages, they are not suitable for surgery and present with locally advanced or metastatic disease. Recent advances in immunotherapy have elicited a considerable amount of attention as viable therapeutic options for several cancer types. This work presents a summary of the currently ongoing clinical trials and critically addresses the efficacy of a large spectrum of immunotherapy approaches in the general population for gastric cancer as well as in relation to tumor genetic profiling.

Das Magenkarzinom zahlt weltweit zu den haufigsten malignen Erkrankungen und ist bei einer 5‑Jahres-Uberlebensrate von 5–30 % mit einer schlechten Prognose vergesellschaftet. Durch Etablierung der molekularen Typisierung und Unterteilung in 4 Subtypen konnte die genomische Heterogenitat dieser Tumoren bewiesen werden. Die Bestimmung von pradiktiven Markern, wie HER2 (Human epithelial growth factor Receptor 2) und PD-L1 (programmed death-ligand 1), eroffnen neue Therapieoptionen. Mikrosatelliteninstabilitat (MSI) ist bei lokalisierten Magenkarzinomen ein prognostischer Pradiktor und konnte zukunftig auch fur das therapeutische Vorgehen wichtig werden. Perioperativ hat sich das FLOT-Regime (5-Fluoruracil/Leucovorin/Oxaliplatin/Docetaxel) als Standardtherapie des kurativ resektablen Magenkarzinoms etabliert und es wurde in die aktuellen Leitlinien aufgenommen. Bei HER2-positiven Karzinomen ist in der Erstlinie die Kombinationstherapie aus Chemotherapie und Trastuzumab Standard. In der Erstlinientherapie der HER2-negativen Tumoren gelten Platin-Fluoropyrimidin-haltige Chemotherapiedubletten als etabliert. Eine Zweit- und Drittlinientherapie verbessert bei ausgewahlten Patienten mit gutem Performancestatus die Lebensqualitat und Symptomkontrolle. Der VEGFR-2-Antikoper (Vascular endothelial growth factor Receptor 2) Ramucirumab ist in der zweiten Therapielinie wirksam. Als palliative Drittlinie wurde das orale Chemotherapeutikum Trifluridin/Tipiracil zugelassen. Die Kombinationstherapie Checkpointinhibitor/Chemotherapie wird voraussichtlich bei Patienten mit hoher PD-L1-Expression zu einem Paradigmenwechsel in der Erstlinientherapie fuhren. Das neue Antikorper-Chemotherapie-Konjugat Trastuzumab-Deruxtecan hat das Potenzial fur eine effektive Therapieoption bei vorbehandelten Patienten mit HER2-positivem Magen-GEJ-Karzinomen. Neue molekulare Zielstrukturen wie Claudin-18.2-Inhibitoren geben Hoffnung auf erweiterte Therapieoptionen.
