Aims Recent decades have seen very large declines in coronary heart disease (CHD) mortality across most of Europe, partly due to declines in risk factors such as smoking. Cardiovascular diseases (predominantly CHD and stroke), remain, however, the main cause of death in most European countries, and many risk factors for CHD, particularly obesity, have been increasing substantially over the same period. It is hypothesized that observed reductions in CHD mortality have occurred largely within older age groups, and that rates in younger groups may be plateauing or increasing as the gains from reduced smoking rates are increasingly cancelled out by increasing rates of obesity and diabetes. The aim of this study was to examine sex-specific trends in CHD mortality between 1980 and 2009 in the European Union (EU) and compare trends between adult age groups. Methods Sex-specific data from the WHO global mortality database were analysed using the joinpoint software to examine trends and significant changes in trends in age-standardized mortality rates. Specific age groups analysed were: under 45, 45–54, 55–64, and 65 years and over. The number and location of significant joinpoints for each country by sex and age group was determined (maximum of 3) using a log-linear model, and the annual percentage change within each segment calculated. Average annual percentage change overall (1980–2009) and separately for each decade were calculated with respect to the underlying joinpoint model. Results Recent CHD rates are now less than half what they were in the early 1980s in many countries, in younger adult age groups as well as in the population overall. Trends in mortality rates vary markedly between EU countries, but less so between age groups and sexes within countries. Fifteen countries showed evidence of a recent plateauing of trends in at least one age group for men, as did 12 countries for women. This did not, however, appear to be any more common in younger age groups compared with older adults. There was little evidence to support the hypothesis that mortality rates have recently begun to plateau in younger age groups in the EU as a whole, although such plateaus and even a small number of increases in CHD mortality in younger subpopulations were observed in a minority of countries. Conclusion There is limited evidence to support the hypothesis that CHD mortality rates in younger age groups in the member states of the EU have been more likely to plateau than in older age groups. There are, however, substantial and persistent inequalities between countries. It remains vitally important for the whole EU to monitor and work towards reducing preventable risk factors for CHD and other chronic conditions to promote wellbeing and equity across the region.

Materials and methods 
A cohort of survivors of myocardial infarction established in Tours in 2009 was followed over 6 years The general practitioner of the patients included in the first 1-year follow-up study was contacted to collect data on treatments, cardiovascular risk factors and lifestyle between January and June 2015 Data were described and compared with the recommendations and predictors of achieving all the recommended targets were determined


Results 
A total of 97 patients (52%) among the 185 patients who underwent a coronary angioplasty for MI were included at baseline Treatment was adapted by the general practitioner on the advice on the cardiologist for 75% (73/97) patients, by both of them for 12% (12/97) and by the general practitioner alone for 7% (7/97) patients Among the 97 initial patients, 62 were included in the final analysis at 6 years Fatal events rate was 5% (5/97) at 1 year and 12% (11/91) at 6 years Non-fatal events rate was 44% (43/97) at 6 months, 19% (17/91) at 1 year and 29% (18/62) at 6 years Six years after the myocardial infarction, 6 (10%) patients achieved the recommended targets in terms of prescriptions of treatment, risk factors and lifestyle targets Exposure to a cardiac rehabilitation program after a myocardial infarction was associated with long-term achievement of optimal therapeutic objectives (OR = 731 [95% CI 174; 4488], p<0002)


Conclusion 
Our data show suboptimal long-term adherence to secondary prevention treatment in this high-risk population of survivors of myocardial infarction, which seems to be improved in patients exposed to a cardiac rehabilitation program

Utilization of influenza vaccination among chronically ill individuals in Germany: A nationwide claims-based analysis.

The purpose of this study was to investigate health care for patients with rheumatoid arthritis (RA) before and after admission to nursing homes. Data of a German health insurance fund from persons with diagnostic codes of RA, aged ≥65 years, admitted to a nursing home between 2010 and 2014 and continuously insured 1 year before and after admission were used. The proportion of patients with ≥1 rheumatologist visit and ≥1 prescription of biologic or conventional synthetic disease-modifying antirheumatic drugs (bDMARDs or csDMARDs), glucocorticoids and non-steroidal anti-inflammatory drugs (NSAIDs) in the year before and after admission were calculated. Predictors of rheumatologic care after admission were analyzed by multivariable logistic regression. Of 75,697 nursing home residents, 2485 (3.3%) had RA (90.5% female, mean age 83.8). Treatment by rheumatologists and prescription of antirheumatic drugs decreased significantly in the year after admission (rheumatologic visits: 17.6 to 9.1%, bDMARDs: 2.1 to 1.5%, csDMARDs: 22.5 to 16.5%, glucocorticoids: 46.5 to 43.1%, NSAIDs: 47.4 to 38.5%). 60.2% of patients in rheumatologic care received csDMARDs compared with 14.5% without rheumatologic care. Rheumatologic care before admission to a nursing home strongly predicted rheumatologic care thereafter (OR 33.8, 95%-CI 23.2–49.2). Younger age and lower care level (reflecting need of help) were also associated with a higher chance of rheumatologic care. Rheumatologic care is already infrequent in old patients with RA and further decreases after admission to a nursing home. Patients without rheumatologic care are at high risk of insufficient treatment for their RA. Admission to a nursing home further increases this risk.

Jahrlich veroffentlicht das Robert Koch-Institut (RKI) die Zahl der HIV-Erstdiagnosen und die Gesamtzahl von Menschen mit HIV-Diagnosen in Deutschland. Eine Bestimmung mithilfe von Sekundardaten erfolgte bislang nur mit Arzneimitteldaten. Auf Basis von Leistungsdaten wird die Anzahl von erstmals ambulant versorgten HIV-Patienten unter gesetzlich Krankenversicherten bestimmt sowie die Gesamtzahl von GKV-Patienten mit HIV in der ambulanten Versorgung. HIV-Testraten fur Deutschland werden untersucht. Die Leistungsdaten der gesetzlich Krankenversicherten aus dem ambulanten kollektivvertraglichen Sektor (kurz: ambulante Versorgung) liegen fur den Beobachtungszeitraum 2009 bis 2018 vor. Fur die Jahre 2011 bis 2017 wurden Jahreskohorten auf Basis von Patientenentitaten gebildet und hinsichtlich der HIV-Diagnose, der HIV-spezifischen Betreuungsleistungen und HIV-Testraten untersucht. Jahrlich werden etwa 6000 Patienten mit HIV erstmals ambulant versorgt. Die Gesamtzahl der ambulant versorgten Patienten mit HIV betrug im Jahr 2011 etwa 59.300 (0,106 %) und stieg zum Jahr 2017 auf etwa 80.800 (0,141 %) an. Der durchschnittliche Anstieg der Gesamtzahl um etwa 3600 Patienten pro Jahr lag damit deutlich unter der geschatzten Zahl jahrlich neu versorgter Patienten. Die Ergebnisse konnen erste Anhaltspunkte zur ambulanten Versorgung von Patienten mit HIV geben. Die vorliegenden Sekundardaten geben die Moglichkeit, eine weitere epidemiologische Datenquelle zur bevolkerungsbezogenen Darstellung der administrativen Pravalenz von HIV zu erschliesen. Es besteht Forschungsbedarf zu Patienten, welche erstmals ambulante Versorgung in Anspruch nehmen, um die Uberreprasentativitat zu klaren.

[Prevalence of patients with HIV receiving outpatient medical care in Germany-an analysis based on secondary data].

Eine der wichtigsten und wirksamsten Praventionsmasnahmen zum Schutz vor saisonaler Influenza sind Impfungen. Die standige Impfkommission (STIKO) empfiehlt die Influenzaimpfung fur besondere Personengruppen, wie Patienten in beschutzenden Einrichtungen sowie in Pflege- und Altenheimen (bPA-Einrichtungen). Die Europaische Kommission (EK) fordert eine Influenzaimpfquote von 75 % in der alteren Bevolkerung. In Deutschland liegen bisher nur vereinzelte Ergebnisse aus Befragungen zu Influenzaimpfquoten fur Patienten in bPA-Einrichtungen vor. Ziel der Studie war es, auf Basis ambulanter Abrechnungsdaten erstmals einen bundesweiten Uberblick uber die Influenzaimpfquoten bei in bPA-Einrichtungen versorgten Patienten zu geben. Es erfolgte die deskriptive Auswertung der ambulanten vertragsarztlichen Abrechnungsdaten fur 6 Studienkohorten in den Jahren 2010 bis 2016. Die Impfquote der Studienkohorten variiert uber die Saisons hinweg zwischen 56,6 % und 60,3 %. Uber den Gesamtzeitraum wurde ein Durchschnitt von 58,5 % erreicht. Es zeigten sich deutliche regionale Unterschiede in den Influenzaimpfquoten von bis zu 20 Prozentpunkten (PP) sowie wenn zuvor ein Hausarzt konsultiert wurde (bis zu 35 PP). Die Ergebnisse geben erste Einblicke zu Impfquoten in bPA-Einrichtungen und enthalten Hinweise auf Zusammenhange zwischen strukturellen Versorgungsunterschieden und der Inanspruchnahme von Influenzaimpfungen. Die Ergebnisse konnen dazu beitragen, Gruppen mit besonders niedrigen Influenzaimpfquoten zu identifizieren. Sie erganzen die Datenbasis fur gesundheitspolitische Masnahmen zur Erhohung der Impfquoten. Fur die untersuchte Gruppe wurden die Empfehlungen der EK zu einer Influenzaimpfquote von 75 % bisher nicht erreicht.

Influenza vaccination rates of patients with statutory ambulatory medical care in protective facilities and in nursing and special care homes

Gemas der Pflegestatistik im Jahr 2014 wurden 2,6 Mio. pflegebedurftige Menschen in Deutschland versorgt; davon 1,86 Mio. zu Hause und rund 764.000 vollstationar im Heim. In den Berichterstattungen zur Zahl von Pflegebedurftigen wird jedoch lediglich von positiv begutachteten Pflegebedurftigen nach § 14 SGB XI berichtet. Fur Deutschland gibt es bisher keine bundesweiten Abschatzungen zu Patienten in beschutzenden Wohnheimen bzw. Einrichtungen bzw. Pflege- oder Altenheimen mit Pflegepersonal (bPA-Einrichtungen) auf Basis von Routinedaten. Diese Lucke wollen wir schliesen. Die vorliegende Auswertung basiert auf bundesweiten vertragsarztlichen Abrechnungsdaten des Jahres 2014. Anhand von Leistungsdaten erfolgte eine Abschatzung der Anzahl an Patienten in bPA-Einrichtungen. Die Darstellung der Morbiditat und die vertragsarztliche Versorgung erfolgten im Rahmen von deskriptiven Analysen. Es wurden 525.863 Patienten in bPA-Einrichtungen ambulant vertragsarztlich versorgt, davon 173.233 Patienten ausschlieslich in bPA-Einrichtungen und 352.630 Patienten sowohl in bPA-Einrichtungen als auch in Praxen durch Vertragsarzte. Knapp zwei Drittel aller Patienten in bPA-Einrichtungen werden nach wie vor in ambulanten Praxen auserhalb der bPA-Einrichtung versorgt. Bundesweite Abrechnungsdaten konnen zur Beurteilung der pflegerischen und vertragsarztlichen Versorgung von Patienten in bPA-Einrichtungen einen wesentlichen Beitrag leisten.
