Impact of maternal pre-pregnancy body mass index on maternal, fetal and neonatal adverse outcomes in the worldwide populations: A systematic review and meta-analysis.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is a common hereditary disorder in China. The existing prevalence and molecular epidemiology of G6PD deficiency in China were geographically limited. In this study, the spectrum of G6PD gene mutations was well characterized in a large and diverse population all over the country; and the correlation of genotype and enzyme activity phenotype was explored for the first time. The results showed that the overall prevalence of G6PD deficiency in China was 2.10% at the national level. The top six common mutations were c.1388 G>A, c.1376 G>T, c.95 A>G, c.392 G>T, c.871 G>A and c.1024 C>T, accounting for more than 90% of G6PD deficient alleles. Compound mutation patterns were frequently observed in females with severe deficiency. The distribution of G6PD activities depended on the type of mutation patterns and genders. Hemizygote, homozygote, and compound heterozygote were predominantly associated with severe G6PD deficiency, whereas heterozygotes with single mutation mainly presented moderate enzyme deficiency. A significant gap between G6PD activities in hemizygous and normal males was observed, and yet, the overall distribution of that in females carrying missense mutations was a continuum from G6PD severely deficient to normal. This is the first report of discussing the association between G6PD genetic variants in the Chinese and enzyme activity phenotypes.

Stillbirths in China: a nationwide survey.

COMMENT The prevalence of obesity in the USA has increased dramatically during the past 2 decades.1 In 1993, Hood and Dewan published the results of a trial comparing obstetric and anesthetic outcomes of 117 morbidly obese parturients with matched controls and found a higher initial epidural anesthesia failure rate, a higher CD rate, and an increased risk of obstetric complicationsy a triple whammy trifecta! That same study also found that morbidly obese patients had a longer first stage of labor (P<0.01), larger neonates (P<0.01), and were more likely to have a failed initial neuraxial technique for labor analgesia (P<0.01), and 50% of morbidly obese women required CD compared with 32% of controls (P<0.01).2 Tonidandel et al replicated the study to provide updated information on outcomes in the morbidly obese pregnant population. The medical records of pregnant patients weighing >136 kg (300 lb) were retrospectively reviewed and compared from the years 2011 to 2012 to matched controls weighing <113 kg (250 lb), consistent with the criteria used previously by Hood and Dewan.1 With respect to measured outcomes Tonidandel et al found: Comorbidities: Morbidly obese parturients were more likely to have chronic hypertension, hypertensive disorders of pregnancy, and diabetes compared with matched controls. Labor analgesia: Morbidly obese parturients were more likely to receive an epidural for labor analgesia, whereas matched controls were relatively more likely to receive a CSE for the initial attempt at pain control. Neuraxial anesthesia for either vaginal or CD took significantly longer to achieve based on procedure start to first dose times in the morbidly obese group (mean 14.1±8.5min, median 12min) compared with matched controls (mean 10.4±8.8min, median 8min) (P<0.001). The need for a replacement procedure for labor was 17%, which was significantly less than 20 years ago when 42% of catheters in morbidly obese women failed (P<0.01). Labor duration: The first stage of labor was significantly longer in duration in the morbidly obese group (mean 632±420 vs. 455±290min, P<0.001), whereas the second and third stages were similar in length. Delivery outcome: For patients who delivered vaginally, obstetrical use of forceps or vacuum was not significantly different between groups. Morbidly obese parturients were more likely to have a CD (50% vs. 32%, P<0.001). The failure rates of neuraxial anesthesia for CD were similar between groups. Morbidly obese parturients were more likely to have an existing catheter dosed and to receive a CSE technique for CD. Approximately half of the CDs for morbidly obese women were classified as urgent or emergent, confirming the need for early neuraxial placement in these laboring women. Primary indications for CD for morbidly obese and matched controls were generally similar and not statistically different overall. Operative duration did not differ between groups (obese 61.0±22.3 vs. controls 60.1±38.5min). Visual estimations of blood loss in the operating room or labor suite also did not differ between groups. General anesthesia: Compared with data from Hood and Dewan, morbidly obese women in the study were significantly less likely to receive a general anesthetic (24% vs. 3%, P<0.01). Length of hospitalization stay: Morbidly obese patients had significantly longer hospitalization stays in all categories, including admission to delivery times, delivery to discharge, and total length of stay. When analyzed separately by mode of delivery, morbidly obese women with vaginal deliveries had longer admission to delivery times. In contrast, for CD, morbidly obese women had longer hospitalization after delivery compared with matched controls. Increased risk and increased mortality still occur in the treatment of the morbidly obese parturient. As was found 20 years ago, morbidly obese parturients are at increased risk for antenatal comorbidities, failed labor analgesia, more epidural catheter redoses, longer first stage of labor and operative delivery, and increased length of hospitalization stay. However, the need for replacement labor epidural catheters and general anesthesia for CD were less commonly required than 20 years ago, and epidural catheters that were dosed for CD worked equally well in both groups. The common practice now of an early epidural catheter placement/replacement strategy in morbidly obese parturients explains why there are fewer general anesthetics compared with 20 years ago. Tonidandel et al have reaffirmed that in this present time increased risk still occurs in the morbidly obese parturient, which ultimately can help the obstetric anesthesiologist in the provision of better and safer obstetric anesthesia care.

Causes and Timing of Death in Extremely Premature Infants From 2000 Through 2011

Hereditary anemia has various manifestations, such as sickle cell disease (SCD), Fanconi anemia, glucose-6-phosphate dehydrogenase deficiency (G6PDD), and thalassemia. The available management strategies for these disorders are still unsatisfactory and do not eliminate the main causes. As genetic aberrations are the main causes of all forms of hereditary anemia, the optimal approach involves repairing the defective gene, possibly through the transplantation of normal hematopoietic stem cells (HSCs) from a normal matching donor or through gene therapy approaches (either in vivo or ex vivo) to correct the patient's HSCs. To clearly illustrate the importance of cell and gene therapy in hereditary anemia, this paper provides a review of the genetic aberration, epidemiology, clinical features, current management, and cell and gene therapy endeavors related to SCD, thalassemia, Fanconi anemia, and G6PDD. Moreover, we expound the future research direction of HSC derivation from induced pluripotent stem cells (iPSCs), strategies to edit HSCs, gene therapy risk mitigation, and their clinical perspectives. In conclusion, gene-corrected hematopoietic stem cell transplantation has promising outcomes for SCD, Fanconi anemia, and thalassemia, and it may overcome the limitation of the source of allogenic bone marrow transplantation.

Objective. In this meta-analysis, we aim to investigate the neonatal mortality in hospitalized Chinese population in the recent 20 years in China, which may provide basis for decreasing the neonatal mortality. Methods. The merged mortality was estimated based on the random effect model, and subgroup analysis was performed for the gender, publication year, gestational age, and birth weight. Sensitivity analysis was utilized to evaluate the effects of research type and research quality on the effects. Results. The neonatal mortality in eastern China was lower than that of the central and western China (2.3% versus 2.9; 2.3% versus 26.%). The mortality in neonates with a gestational age of 28-32 weeks (0.6%) was significantly higher than that of 42 weeks (0.1%). The mortality in those with a body weight of 1.0-2.5 kg (0.3%) was significantly higher than that of 2.5-4.0 kg (0.2%) and >4.0 kg (0.0%). Sensitivity analysis revealed that the findings of meta-analysis were stable. The major causes for neonatal death included asphyxia, respiratory distress syndrome, and infection. Conclusions. The hospitalized neonatal mortality showed a tendency of decrease, which differed based on gender, region, gestational age, and birth weight.

Neonatal Mortality in Hospitalized Chinese Population: A Meta-Analysis

To date, the genetic risk factors for neonatal hyperbilirubinemia remain unknown in Southeastern China. This case-control study aimed to identify the genetic risk factors for neonatal hyperbilirubinemia in Fujian, Southeastern China. A total of 286 hyperbilirubinemic newborns were enrolled as a case group, and 250 randomly selected newborns without jaundice or with a bilirubin level that was lower than the threshold required for phototherapy served as controls. The serum levels of total bilirubin, unconjugated bilirubin, and direct bilirubin were measured, and the common genetic loci in UGT1A1, OATP1B1, and HO-1 genes were genotyped. Higher incidence of ABO incompatibility and G6PD deficiency was detected in the case group compared to the control group (P < 0.01). There were significant differences in the frequencies of rs4148323 and rs1805173 genotypes between the case and control groups (P < 0.05). At the rs4148323 locus, the frequencies of GA heterozygotes and AA mutant homozygotes were higher in the case group than in the control group (P < 0.05), and at the rs1805173 locus, the frequencies of LS, MS, and SS genotypes were higher in the case group than in the control group (P < 0.05). A higher frequency of rs4148323 A allele and rs1805173 S allele was detected in the case group compared to the control group (P = 0). Additionally, multivariate logistic regression analysis identified that the mutant genotype of rs4148323 in the UGT1A1 gene, ABO incompatibility, G6PD deficiency, and SS genotype at rs1805173 locus of the HO-1 gene were genetic risk factors of neonatal hyperbilirubinemia. Our data demonstrate that G211 mutation in the UGT1A1 gene, ABO incompatibility, G6PD deficiency, and the SS genotype of the repeats in the promoter region of the HO-1 gene are risk factors for neonatal hyperbilirubinemia in Fujian, Southeastern China.
