In the course of the research and development for and the application of atomic energy and nuclear technologies, a number of radiation accidents have occurred. Some of these accidents have resulted in significant health effects and occasionally in fatal outcomes. The application of technologies that make use of radiation is increasingly widespread around the world. Millions of people have occupations related to the use of radiation, and hundreds of millions of individuals benefit from these uses. Facilities using intense radiation sources for energy production and for purposes such as radiotherapy, sterilization of products, preservation of foodstuffs and gamma radiography require special care in the design and operation of equipment to avoid radiation injury to workers or to the public. Experience has shown that such technology is generally used safely, but on occasion controls have been circumvented and serious radiation accidents have ensued. 2. Reviews of radiation exposures from accidents have been presented in previous UNSCEAR reports. The last report containing an exclusive chapter on exposures from accidents was the UNSCEAR 1993 Report [U6]. 3. This annex is aimed at providing a sound basis for conclusions regarding the number of significant radiation accidents that have occurred, the corresponding levels of radiation exposures and numbers of deaths and injuries, and the general trends for various practices. Its conclusions are to be seen in the context of the Committee's overall evaluations of the levels and effects of exposure to ionizing radiation. 4. Recent technologic advances have markedly enhanced the clinical applications of computed tomography (CT). While the benefits of CT exceed the harmful effects of radiation exposure in patients, increasing radiation doses to the population have raised a compelling case for reduction of radiation exposure from CT. Strategies for radiation dose reduction are difficult to devise, however, because of a lack of guidelines regarding CT examination and scanning techniques. Various methods and strategies based on individual patient attributes and CT technology have been explored for dose optimization. It is the purpose of this review article to outline basic principles of CT radiation exposure and emphasize the need for CT radiation dose optimization based on modification of scanning parameters and application of recent technologic innovations.

Radiation dose reduction in computed tomography: techniques and future perspective

The introduction of helical single-detector row computed tomography (CT) and, more recently, multi-detector row CT has greatly increased the clinical indications for CT. Correspondingly, CT examinations now account for greater than one-half of the radiation dose due to medical procedures in the population of North America. The level of CT radiation dose, especially in the pediatric population, is of concern to radiologists, medical physicists, government regulators, and the media. This review addresses this problem with particular reference to radiation dose in chest CT. Specifically it outlines the topics of measurement units used to quantify radiation exposure, factors affecting CT scanner dose efficiency, scanner settings that determine the administered radiation dose, and radiation dose reduction in chest CT. A table of reference dose values is provided. Given the wide variation documented in chest CT radiation exposure, the authors suggest that reference standards be promoted to minimize excessive CT radiation exposure. In addition, further research into the complex relationship between radiation exposure, image quality, and diagnostic accuracy should be encouraged in order to establish the minimum radiation dose necessary to provide adequate diagnostic information for standard clinical questions.

Achieving routine submillisievert CT scanning: report from the summit on management of radiation dose in CT.

PURPOSE: To validate a technique of computer-simulated dose reduction for conventional chest computed tomography (CT). MATERIALS AND METHODS: In 27 patients, CT scans were obtained at 200, 100, and 40 mAs at two levels. The raw data from the 200-mAs scan were modified on a computer workstation to simulate the increased noise present on 100- and 40-mAs scans. Real and simulated 100- and 40-mAs images were independently assessed in random order for overall image quality and radiologic findings by four subspecialty-trained chest radiologists who were blinded to the technique. The four observers were given paired real and simulated images. They were asked to identify the real image and note any difference in diagnostic quality. RESULTS: No difference was seen in overall image quality or radiologic findings between real and simulated images (P > .05).
