The report of the President's Council of Advisors on Science and Technology (PCAST) has placed CPS on the top of the priority list for federal research investment [6]. This article first reviews some of the challenges and promises of CPS, followed by an articulation of some specific challenges and promises that are more closely related to the sensor networks, ubiquitous and trustworthy computing conference.

Cyber-Physical Systems: A New Frontier

Information technology can improve the quality, efficiency, and cost of healthcare. In this survey, we examine the privacy requirements of mobile computing technologies that have the potential to transform healthcare. Such mHealth technology enables physicians to remotely monitor patients' health and enables individuals to manage their own health more easily. Despite these advantages, privacy is essential for any personal monitoring technology. Through an extensive survey of the literature, we develop a conceptual privacy framework for mHealth, itemize the privacy properties needed in mHealth systems, and discuss the technologies that could support privacy-sensitive mHealth systems. We end with a list of open research questions.

Privacy in mobile technology for personal healthcare

Research into ambient assisted living (AAL) strives to ease the daily lives of people with disabilities or chronic medical conditions. AAL systems typically consist of multitudes of sensors and embedded devices, generating large amounts of medical and ambient data. However, these biomedical sensors lack the processing power to perform key monitoring and data-aggregation tasks, necessitating data transmission and computation at central locations. The focus here is on the development of a scalable and context-aware framework and easing the flow between data collection and data processing. The resource-constrained nature of typical wearable body sensors is factored into our proposed model, with cloud computing features utilized to provide a real-time assisted-living service. With the myriad of distributed AAL systems at play, each with unique requirements and eccentricities, the challenge lies in the need to service these disparate systems with a middleware layer that is both coherent and flexible. There is significant complexity in the management of sensor data and the derivation of contextual information, as well as in the monitoring of user activities and in locating appropriate situational services. The proposed CoCaMAAL model seeks to address such issues and implement a service-oriented architecture (SOA) for unified context generation. This is done by efficiently aggregating raw sensor data and the timely selection of appropriate services using a context management system (CMS). With a unified model that includes patients, devices, and computational servers in a single virtual community, AAL services are enhanced. We have prototyped the proposed model and implemented some case studies to demonstrate its effectiveness.

CoCaMAAL: A cloud-oriented context-aware middleware in ambient assisted living

One of the challenges of the ageing population in many countries is the efficient delivery of health and care services, which is further complicated by the increase in neurological conditions among the elderly due to rising life expectancy. Personal care of the elderly is of concern to their relatives, in case they are alone in their homes and unforeseen circumstances occur, affecting their wellbeing. The alternative; i.e. care in nursing homes or hospitals is costly and increases further if specialized care is mobilized to patients’ place of residence. Enabling technologies for independent living by the elderly such as the ambient assisted living systems (AALS) are seen as essential to enhancing care in a cost-effective manner. In light of significant advances in telecommunication, computing and sensor miniaturization, as well as the ubiquity of mobile and connected devices embodying the concept of the Internet of Things (IoT), end-to-end solutions for ambient assisted living have become a reality. The premise of such applications is the continuous and most often real-time monitoring of the environment and occupant behavior using an event-driven intelligent system, thereby providing a facility for monitoring and assessment, and triggering assistance as and when needed. As a growing area of research, it is essential to investigate the approaches for developing AALS in literature to identify current practices and directions for future research. This paper is, therefore, aimed at a comprehensive and critical review of the frameworks and sensor systems used in various ambient assisted living systems, as well as their objectives and relationships with care and clinical systems. Findings from our work suggest that most frameworks focused on activity monitoring for assessing immediate risks, while the opportunities for integrating environmental factors for analytics and decision-making, in particular for the long-term care were often overlooked. The potential for wearable devices and sensors, as well as distributed storage and access (e.g. cloud) are yet to be fully appreciated. There is a distinct lack of strong supporting clinical evidence from the implemented technologies. Socio-cultural aspects such as divergence among groups, acceptability and usability of AALS were also overlooked. Future systems need to look into the issues of privacy and cyber security.

Progress in ambient assisted systems for independent living by the elderly

In this paper, we consider the challenge of preserving patient privacy in the context of mobile healthcare and home-care systems, that is, the use of mobile computing and communications technologies in the delivery of healthcare or the provision of at-home medical care and assisted living. This paper makes three primary contributions. First, we compare existing privacy frameworks, identifying key differences and shortcomings. Second, we identify a privacy framework for mobile healthcare and home-care systems. Third, we extract a set of privacy properties intended for use by those who design systems and applications for mobile healthcare and home-care systems, linking them back to the privacy principles. Finally, we list several important research questions that the community should address. We hope that the privacy framework in this paper can help to guide the researchers and developers in this community, and that the privacy properties provide a concrete foundation for privacy-sensitive systems and applications for mobile healthcare and home-care systems.

A privacy framework for mobile health and home-care systems

Advances in networking, sensors, and embedded devices have made it feasible to monitor and provide medical and other assistance to people in their homes. Aging populations will benefit from reduced costs and improved healthcare through assisted living based on these technologies. However, these systems challenge current state-of-the-art techniques for usability, reliability, and security. This is a particular challenge for open and extensible systems that combine software and hardware from many vendors and provide information to diverse clinicians. In this paper we present the I-Living architecture for assisted living that allows independent parties work together in a dependable, secure, and low-cost fashion with predictable properties. Our approach is based on an Assisted Living Service Provider (ALSP) who provides a server that collects and maintains encrypted assisted persons (APs)' records. Our ALSP can be a third party distinct from APs, communication providers, and clinicians; or it can be part of an ISP, hospital or similar enterprise. We have explored the architecture by developing a collection of applications and implementing them in a prototype system. Our system shows the feasibility and opportunity of an open approach to assisted living systems.
