Nurses' Knowledge of Pain Assessment and Management: How much Progress Have We Made?

Aims and objectives. To determine whether a specific training course will enhance empathic skills in student nurses during a 3year degree course. The study considers levels of empathy in student nurses and assesses whether these can be increased. Background. Empathy is accepted as a critical component of supportive relationships. Many scholars have argued that empathy provides health professionals with the capacity to improve the health of patients, so it should ideally be taught to make health professionals more responsive to patient needs. Design. Cohort longitudinal study. Methods. Data were collected using the Italian version of the Balanced Emotional Empathy Scale (BEES). The study was designed to guarantee repeated measurements of levels of emotional empathy in the pre- and postintervention phases. This allowed us to evaluate the development and improvement of empathy and to measure the effects of extra training on the students’ empathy skills. Two groups took part in the study: the Intervention and the Control Groups. Students in the Intervention Group attended additional seminars and laboratories in small groups with tutors to learn and develop communicative and empathic abilities. Results. One hundred and three participants (76% women) completed the study. Data showed that the training course was effective, especially for women: BEES scores 31AE60 at pre-intervention phase and 42AE91 at the end of the academic course. Results regarding men were less clear, and the study discusses these unexpected, anomalous findings. Conclusion. According to literature, men and women have different empathic traits, and we found that they show disparate empathic tendencies. More women than men took advantage of the training course. In any case, our data show that specific training courses are effective. Relevance to clinical practice. As our study shows, empathy is a skill that may be taught. So it would be advisable to introduce these into the traditional nursing curriculum.

Developing empathy in nursing students: a cohort longitudinal study

Background and aim: Empathy is an essential element of good nursing care associated with increased patient satisfaction. Burnout represents chronic occupational stress which diminishes interest in work and reduces patient safety and satisfaction. The purpose of this study was to evaluate the correlation between empathy and burnout in nursing students and nurses. Method: This cross-sectional research was conducted in a sample of 298 nurses and 115 nursing students. Socio-demographic and career information was collected. Balanced Emotional Empathy Scale (BEES) and Maslach Burnout Inventory (MBI) were administered. Data were statistically analysed. Results: 63% of our sample answered questionnaires (54% of nurses and 84% of students). The BEES global mean score was slightly inferior to empathy cut-off of 32. In the student group, two BEES dimension scores were statistically significantly higher than nurses ( p =0.011 and p =0.007 respectively, t-test). Empathy was negatively related to age ( p =0.001, ANOVA). Emotional exhaustion (EE) scores of MBI reported statistically significantly lower levels for students ( p <0.0001, t-test). EE was negatively related to BEES mean total score in students (r=-0.307, p <0.002) and nurses (r=-0.245, p <0.002), personal accomplishment of MBI presented positive correlation with BEES mean total scores in students (r=0.319, p <0.002) and nurses (r=0.266, p <0.001, Pearson’s correlation). Female students showed superior empathy capacity in comparison to male students in all 5 dimensions of BEES ( p <0.001), whereas females nurses in only one dimension ( p <0.001). Conclusions: Our data suggest empathy declines with age and career. High levels of empathy can be protective against burnout development, which, when presents, reduces empathy.

Empathy and burnout: an analytic cross-sectional study among nurses and nursing students

Pain is a common phenomenon among emergency patients which may lead to chronic pain conditions and alteration of physiological function. However, it is widely reported that proper pain assessment and management, which is often accomplished by adequately trained nurses reduce the suffering of patients. Therefore, the aim of this study was to assess the emergency nurses´ knowledge, attitude and perceived barriers regarding pain management. A cross-sectional quantitative study design was applied to determine the nurses´ knowledge level, attitude and the perceived barriers related to pain management. Hundred twenty-six nurses from the emergency departments of seven referral hospitals of Eritrea participated in the study. Data were collected in August and September 2017. Both descriptive and inferential statistics were used to summarize and elaborate on the results. In general, the knowledge level and attitude of the emergency nurses was poor. The participants’ correct mean score was 49.5%. Nurses with Bachelor’s Degree had significantly higher knowledge and attitude level compared to the nurses at the Diploma and Certificate level of professional preparation (95% CI = 7.1–16.7 and 9.4–19.1; p <  0.001) respectively. Similarly, nurses who had previous training regarding pain scored significantly higher knowledge level compared to those without training (95% CI =1.82–8.99; p = 0.003). The highest perceived barriers to adequate pain management in emergency departments were measured to be overcrowding of the emergency department (2.57 ± 1.25), lack of protocols for pain assessment (2.45 ± 1.52), nursing workload (2.44 ± 1.29) and lack of pain assessment tools (2.43 ± 1.43). There was no significant difference in perceived barriers among nurses with different demographic characteristics. The emergency nurses’ knowledge and attitude regarding pain management were poor. Nurses with higher educational level and nurses with previous training scored significantly higher knowledge level. This indicates the need for nursing schools and the ministry of health to work together to educate nurses to a higher level of preparation for pain assessment and management.

Emergency nurses´ knowledge, attitude and perceived barriers regarding pain Management in Resource-Limited Settings: cross-sectional study.

Background and aims: The use of Tirosine Kinase Ihnibitors (TKIs) for the treatment of Chronic Myeloid Leukemia (CML) has definitely represented a turning point in the treatment of the onco-hematological diseases. Over the years, the interest of physicians, nurses, patients and caregivers has increasingly focused on the aspects of the humanization of care, the management of side effects and on the full and constant therapeutic adherence. The aim of the project was to define patient-oriented care processes, based on a proactive approach that can fully respond to the new health needs of CML patients. Methods: A nursing expert Working Group (WG) was established. WG reviewed literature about CML patients assistance and then it was conducted a survey on organizational models for the treatment of CML patients, adopted by Italian haematologic and transplant centers.  Finally, the main issues regarding CML patients care were identified and discussed on a multiprofessional basis. Results: Euriclea Project for care of CML patients with the description of a new and expanded nurse role was defined. The Nurse Case Manager or Nursing Clinical Experts were identified as key people for the management of the side effects of treatment, the promotion of the therapeutic adherence and the evaluation of efficacy and effectiveness of the process through the identification of specific indicators for structure, process and outcome. Discussion: The focal areas of the care process were identified so as to define a different approach to the CML patient, through a holistic view of care and the multidisciplinary interventions.

Clinical pathway for patients with Chronic Myeloid Leukaemia: The Euriclea Project.

Background: Despite the growing awareness on pain management, mild and severe pain is still common among hospitalized patients. Inadequate treatment has been mostly linked to health care workers' failure to assess patients’ pain and to intervene appropriately. Objective: To determine the pain management knowledge and attitudes of the nurses working in training and research hospital in Turkey and their relationship to demographic and educational factors. Methods: Descriptive and cross-sectional study design was used. The Nurses’ Knowledge and Attitudes Survey Regarding Pain (NKASRP) was used to determine the pain-related knowledge and attitude of the nurses. Results: A total of 246 nurses were included in the study. The percentage working at a surgical unit was 48.8% and 77.2% were staff nurses. Only 11.8% had taken a pain management course and 31.7% had read a book or journal on pain. The rate of correct responses to the NKASRP scale was 39.65% while the mean number of correct answers to all questions was 15.86±7.33 with a range of 0 to 37. A statistically significant difference was found regarding education level, working unit, whether a pain management course had been taken, whether a book or journal on pain had been read, and the evaluation of the nurse's efficacy regarding pain (p<0.05). Conclusion: We found that the nurses did not have adequate knowledge and the positive attitude for pain management although it is a very important part of patient care. We suggest that results be taken into account when the content of graduate and postgraduate training programs is determined and guidelines are prepared.

Vegetative state (VS) is one of the most severe outcomes of brain damage. The nurses who give care to VS patients face different and strenuous experiences during care in intensive care units (ICUs). The aim of this study was to determine the experiences of ICU nurses who give care to VS patients. This was a qualitative study with 24 nurses who work in ICUs in a hospital. Characteristics and results from a semi-structured interview were used as data. The nurses were interviewed individually, face-to-face. The evaluation of data was made by Colaizzi’s method, and three categories and seven themes were identified. The categories were: (i) professional approach, (ii) powers of care, and (iii) effects on the nurse. It was determined that the style of the nursing care for VS patients is based on nurses’ professional responsibility and the dictates of their conscience, despite the physical and psychological challenges the job entail. Moreover, the nurses question the necessity of the provision of care, which many of them believe is futile. In conclusion, the nurses’ experiences on the subject were consistent with the results from other related studies, and we recommend that care of VS patients would better be maintained in special care units other than ICUs. Aim:  To evaluate the empathic skills and the empathetic tendency of nursing students throughout their years of undergraduate education.

Background:  Empathy is a major component of the relationships between patients and nurses, and is an observable and teachable skill that nurses are claimed to possess.

Methods:  A cross-sectional study was designed to determine the differences in the empathic skills and tendency of nursing students in the successive years of undergraduate nursing education. A longitudinal study was also designed to evaluate the changes between the beginning and the end of the nursing education in the same group. All the registered students were asked to participate in the study. The total population was 466 nursing students of whom 438 participated (94%) in the cross-sectional study and 81 in the longitudinal study. The Empathic Communication Skills Scale (ECSS) and the Empathic Tendency Scale (ETS) were used to collect data.

Findings:  An increase in the ECSS and a decrease in the ETS were observed in both the cross-sectional and longitudinal studies. Comparing the years, fourth-year students have higher empathic skills level, whereas newly registered students have a higher empathic tendency score (P < 0.05).

Conclusions:  The findings have shown that empathic skills developed during undergraduate nursing education. However, empathetic tendency has shown a decline during these educational years. The decrease in empathetic tendency during undergraduate education should be taken into account, and educators and researchers should consider possible reasons for this outcome. Further research is indicated.
