TL;DR: The specificity of the outcome of PTL-DLBCL is a high risk of extranodal relapse, particularly CNS relapse and contralateral testis, even in cases with localized disease at diagnosis, so management of the patients should be carefully discussed in terms of adjuvant therapy after orchidectomy.
