Angiosarcoma is a rare and aggressive tumor of the thyroid gland, mainly seen in the Alpine regions. We present such a case with literature review. We present the case of a 60-year-old man with cough, dyspnea, and hemoptysis along with slow increase in the size of his long-standing goiter. Computed tomography of the neck showed a large thyroid mass and chest imaging revealed multiple pulmonary nodules. Fine needle aspiration cytology and tru-cut biopsy of the thyroid were notable for poorly differentiated malignant cells. Diagnosis of angiosarcoma of the thyroid was made after total thyroidectomy. Patient died of continued hemoptysis and respiratory failure 3 weeks after admission. We searched the literature for previous case reports using Pubmed and Ovid. Forty-seven reported cases were identified and our case was added to make a database of 48 cases. Demographic and tumor characteristics were analyzed. Angiosarcoma was found to be more common in females and at age of 60 or above. Results were consistent with previously reported series of 14 and 17 cases from Austria. This review provides information on various characteristics angiosarcoma of the thyroid which can be used as baseline data for future reference and research studies for this cancer.

The authors describe the case of a 46-year-old male presented with repeated massive gastrointestinal haemorrhage as the first clinical symptom of jejunal metastasis of a malignant tumour. Orchiectomy of the enlarged testis was subsequently carried out and testicular seminoma was found on histology. Metastases of seminoma to the liver and lung were detected. The source of gastrointestinal bleeding was identified by means of capsule endoscopy. Push enteroscopy was performed subsequently. At the present time, capsule endoscopy is a non-invasive method capable of visualizing a substantial part of small intestinal mucosa within a major part or even the whole small bowel. Push enteroscopy allows to see nearly all mucosal surface but only within the upper part of the small intestine, i.e. duodenum and the proximal part of the jejunum (up to 60 to 120 cm away from the Treitz ligament). As the tumour of our case was generalised when diagnosed and could not be treated either endoscopically or surgically, a cisplatinum based chemotherapy was started (bleomycin, etoposide, cisplatinum). Evident regression of the pulmonary and liver metastases was achieved according to control CT scan and there was no further gastrointestinal bleeding during the subsequent nine-month follow-up.

Primary thyroid angiosarcoma is a very rare and aggressive disease, originally reported in patients from the Swiss Alpine region, but is rare in other parts of the world. We describe a Japanese case of primary angiosarcoma of the thyroid in a 64-year-old man, who presented with a rapidly enlarging painful neck mass. Fine-needle aspiration biopsy was performed, and the results were interpreted as malignant. FDG-PET-CT demonstrated a necrotic lesion on the right side of the neck with high FDG uptake. The patient underwent a total thyroidectomy. Pathological findings revealed a proliferation of atypical cells lined vascular spaces. Immunohistochemistry staining (IHCS) was positive for factor VIII, and CD31, but not CD34, and negative for CK 5, CK 7, and CK 20. From these findings, it was diagnosed as an epithelioid angiosarcoma. Two weeks after the operation, bilateral bloody pleural effusion was observed, and he died 3 months after the surgery.

A case of rapidly progressing primary thyroid angiosarcoma in a Japanese man

Authors present a case of epithelioid haemangiosarcoma of the thyroid gland in a 54-year-old female with a history of eufunctional nodular goitre. The patient was treated by total thyroidectomy with extirpation of cervical lymph nodes and by subsequent chemotherapy. The tumour behaved highly aggressively with early generalization and unusual way of metastatic spread into the wall of the stomach and duodenum. The patient died 3 months after the surgery due to therapeutically unmanageable bleeding into the GIT. The clinico-pathological aspects of the case are discussed.
