A 27-year-old male patient presented with respiratory symptoms, bilateral enlargement of the cervical lymph nodes and enlarged liver. In the imaging studies, bilateral enlargement of the hilar nodes was observed, together with pulmonary infiltrate. The patient was submitted to lung and liver biopsies, which revealed noncaseating granulomas. The clinical, radiological and histopathological findings were consistent with sarcoidosis and lymph node amyloidosis. The combination of sarcoidosis and amyloidosis has rarely been reported.

Resume Introduction La sarcoidose est une granulomatose frequente, d’evolution generalement favorable L’amylose AA a ete exceptionnellement decrite au cours de la sarcoidose Nous en rapportons deux nouvelles observations Observations Dans le premier cas, il s’agissait d’une femme, âgee de 40 ans, suivie pour une splenomegalie, et hospitalisee pour hepatite cholestatique Le diagnostic de sarcoidose avec atteinte pulmonaire, hepatique, splenique et ganglionnaire etait retenu sur des arguments cliniques, biologiques et histologiques Des depots amyloides de type AA au niveau des ganglions et de la rate etaient simultanement mis en evidence Dans le second cas, il s’agissait d’une femme âgee de 42 ans, hospitalisee pour adenopathies mediastinales et syndrome interstitiel pulmonaire, dont les biopsies montraient des granulomes epithelioides et gigantocellulaires sans necrose caseeuse, associes a des depots amyloides de type AA L’evolution de la sarcoidose et de l’amylose etait favorable dans les deux cas avec un recul respectif d’un et cinq ans et une corticotherapie initiale de 1 mg/kg par jour Conclusion L’amylose AA peut compliquer la sarcoidose La corticotherapie en association avec la colchicine est proposee pour traiter les deux affections

Localized amyloidosis of the lacrimal gland is a rare disease. We report a case of transthyretin-positive localized amyloidosis of the lacrimal gland in a 74-year-old man with left lacrimal gland swelling. Biopsy of the left lacrimal gland showed extensive deposition of nonstructural eosinophilic material in the secretory gland and ducts, which stained positive with direct fast scarlet. Immunostaining was negative for amyloid A and positive for both globulin light chain (kappa, lambda) and transthyretin. It is necessary to consider the possibility of senile systemic amyloidosis, even if localized amyloidosis of the lacrimal gland is suspected.

Lacrimal Gland Amyloidosis in an Elderly Patient.

Introduction: Sarcoidosis is a multisystem disease that commonly involves the lungs, but may also present with extrapulmonary manifestations. Genitourinary (GU) tract involvement has been traditionally thought to be rare, but that view may underestimate the true prevalence of the disease due to the often, silent presentation thereof. Methods: The literature pertaining to sarcoidosis from the general systemic point of view, etiology and therapy and with regard to specific organs was reviewed by identifying key words in a PubMed search. That material with special relevance to the Indian experience was emphasized. Results: There are a number of isolated case reports in the literature describing symptomatic and asymptomatic GU tract sarcoidosis. The world literature associated with the generalized syndrome was reviewed and summarized. Specific aspects of GU involvement are presented for each organ of the GU tract. Conclusions: It is critical for the practicing clinician to have a working knowledge of the clinical manifestations of this disease as it involves the GU tract, as well as to be able to distinguish it from tuberculosis and the various malignancies that affect this organ system.
