The method for obtaining composite hydrogel tubular products with the increased strength on the basis of copolymers of 2-hydroxyethylmethacrylate with polyvinylpyrrolidone has been developed. The method consists in the formation of hydrogel tubes with a subsequent precipitation from the solution into their outer surfa with polyvinylpyrrolidone. The obtained composite hydrogel tubes are characterized by the sufficient strength, resilience, elasticity, as well as the ability to withstand an internal pressure within 24–43 kPa (180–320 mm Hg).

Formation Features of Tubular Products on the Basis of Composite Hydrogels

A nonoperative approach to venous stasis ulceration of the lower extremity, consisting of initial bedrest, ulcer cleansing, dressing changes, and ambulatory elastic compression stocking therapy, has been maintained for over 15 years. All patients had class III, severe chronic venous insufficiency. One hundred five of 113 patients (93%) experienced complete ulcer healing in a mean of 5.3 months. One hundred two patients were compliant with elastic compression stockings, and 11 patients were noncompliant. Complete ulcer healing occurred in 99 of 102 patients (97%) who were compliant versus six of 11 patients (55%) who were noncompliant (p less than 0.0001). The influence of noncompliance, previous venous ulceration, previous venous surgery, previous known deep venous thrombosis, peripheral arterial insufficiency (ankle brachial systolic blood pressure index less than or equal to 0.60), pretreatment ulcer duration, ulcer size, age, sex, diabetes, smoking, and photoplethysmography venous refill time on ulcer healing was determined by logistic regression analysis. Only noncompliance with elastic compression stockings (p less than 0.0001) and a pretreatment ulcer duration of more than 9 months (p = 0.02) significantly decreased initial ulcer healing. Posthealing follow-up was available in 73 patients for a mean of 30 months. Fifty-eight patients (79%) continued to be compliant with stockings; 15 patients were noncompliant. Total ulcer recurrence in patients who were compliant was 16%. Five-year lifetable recurrence was 29%. All patients who were noncompliant had recurrent ulceration by 36 months. Previous ulceration, previous venous surgery, and peripheral arterial insufficiency had no effect on ulcer recurrence (p greater than 0.05).

Introdução: a diabetes mellitus se caracteriza por ser um problema de grande desordem mundial, com capacidade de morbidades agudas e crônicas. Uma das principais desordens são as lesões ulcerosas nas extremidades, como nos pés, e quando há uma má evolução do quadro do paciente, pode resultar em amputação do membro afetado. A ozonioterapia é uma opção de tratamento alternativo para esses tipos de lesões devido a sua atuação no estresse oxidativo, o que resulta numa boa recuperação do indivíduo, com subsequente angiogênese e cicatrização das lesões, podendo ser usado de forma adjuvante ao tratamento convencional, já que tem ação antimicrobiana. Objetivo: Verificar a partir da literatura disponível qual a influência da ozonioterapia como adjuvante no tratamento de feridas ulcerosas de pé diabético. Metodologia: Revisão integrativa de literatura a partir dos descritores “Ozônio”, “Terapias complementares”, “Pé Diabético” e “Diabetes Mellitus”, por meio da base de dados PubMed e BSV, resultando em 17 artigos para análise completa e selecionando 11 com maior relevância. Resultados: A ozonioterapia pode ser usada como tratamento adjuvante no tratamento de úlceras do pé diabético, como foi concluído em 8 (72,7%) estudos analisados, porém 3 (27,2%) concluíram que não havia estudos suficientes para um desfecho sobre o assunto, além de alertarem para que as terapias adjuvantes só sejam usadas quando os tratamentos convencionais não forem eficazes. Apesar das evidências positivas observadas em relação ao uso do ozônio de modo adjuvante no tratamento de úlceras do pé diabético, no Brasil, o Conselho Federal de Medicina (CFM) estabelece a ozonioterapia apenas como procedimento experimental. Conclusão: A grande maioria dos artigos analisados nesta revisão de literatura indicam que a ozonioterapia é eficiente no tratamento das lesões ulceradas em pacientes diabéticos, porém a quantidade de estudos realizados nessas áreas ainda é reduzida, fazendo-se necessária uma gama maior e mais diversificada de estudos referentes a essa prática alternativa.

Influência da ozonioterapia como adjuvante no tratamento de lesões ulcerosas em pacientes portadores de diabetes mellitus

BACKGROUND: Wearing elastic compression stockings is of considerable significance for patients with chronic venous disease (CVD), since their physiological effect is to improve venous hemodynamic parameters, making them a good treatment option that can impact on patients' quality of life. OBJECTIVE: To assess quality of life in patients with chronic venous disease who do or do not wear elastic stockings. METHODS: This was a cross-sectional observational study assessing a sample of 50 CVD patients of both sexes, divided into two groups, one who wear elastic stockings and another who do not. Primary variables were the domains of the SF-36 (Medical Outcomes Study Short-form 36) and AVVQ (Aberdeen Varicose Veins Questionnaire) and secondary variables were: age; sex; physical activity; educational level; profession; wearing elastic stockings (frequency, pressure and type); elevation of lower limbs; itching and CEAP classification. The SF-36 and AVVQ quality of life questionnaires were administered to patients. Statistical significance was set at p<0.05. RESULTS: Wearing elastic stockings proved beneficial for the quality of life of people with chronic venous disease. For the AVVQ disease-specific questionnaire the greatest improvements were in overall score (p=0.0028) and the extent of varicosity domain (p=0.000). The SF-36 domains role emotional (p=0.017) and functional capacity (p=0.000) both improved. CONCLUSIONS: Wearing elastic stockings is an effective treatment for CVD that improves disease-specific quality of life and also leads to improvements in general quality of life.
