Recent developments for the delivery of proton and ion beam therapy have been significant, and a number of technological solutions now exist for the creation and utilisation of these particles for the treatment of cancer. In this paper we review the historical development of particle accelerators used for external beam radiotherapy and discuss the more recent progress towards more capable and cost-effective sources of particles.

Technologies for delivery of proton and ion beams for radiotherapy

This article is intended to present the different types of particle and radiation detectors available for applications in particle therapy. Several types of detectors and sensors exist for measurements of absorbed dose in reference and nonreference conditions and the ones in use for beam monitoring. Therefore, this manuscript focuses on the following applications: (a) primary methods for dosimetry in ion beams, (b) measurements of absorbed dose in realistic patient-specific scenarios with different dose delivery configurations, and (c) beam monitoring. Water and graphite calorimeters, Faraday cups, ionization based gas detectors are described first, followed by the description of the protocols for proton and ion dosimetry. Then films, scintillator and solid-state detectors are reviewed. Finally, several types of ionization chambers (large, pinpoint, arrays of chambers arranged in regular 1D or 2D pattern, parallel-plate configuration with large integral electrodes or with anode segmented in strips or pixels, multi-wire, and the multi-gap prototype) have been considered. New beam monitors to deal with a wide range of intensity and pulsed beams expected from new accelerators, different dose fractionation and advanced delivery techniques are presented. The existing detectors available for particle therapy have been described taking into account the different requirements for devices used in reference and nonreference conditions and the ones designed for beam monitoring.

Dose detectors, sensors, and their applications.

This paper explores the physics of vacuum rf breakdowns in high gradient mm-wave accelerating structures. We performed a series of experiments with 100 GHz and 200 GHz metallic accelerating structures, at the Facility for Advanced Accelerator Experimental Tests (FACET) at the SLAC National Accelerator Laboratory. This paper presents the experimental results of rf tests of 100 GHz travelling-wave accelerating structures, made of hard copper-silver alloy. The results are compared with pure hard copper structures. The rf fields were excited by the FACET ultra-relativistic electron beam. The accelerating structures have open geometries, 10 cm long, composed of two halves separated by a variable gap. The rf frequency of the fundamental accelerating mode depends on the gap size and can be changed from 90 GHz to 140 GHz. The measured frequency and pulse length are consistent with our simulations. When the beam travels off-axis, a deflecting field is induced in addition to the decelerating longitudinal field. We measured the deflecting forces by observing the displacement of the electron bunch and used this measurement to verify the expected accelerating gradient. We present the first quantitative measurement of rf breakdown rates in 100 GHz copper-silver accelerating structure, which was 10 −3 per pulse, with peak electric field of 0.42 GV/m, an accelerating gradient of 127 MV/m, at a pulse length of 2.3 ns. The goal of our studies is to understand the physics of gradient limitations in order to increase the energy reach of future accelerators.

High gradient tests of metallic mm-wave accelerating structures

This Habilitationsschrift (Habilitation thesis) is focused on my research activities on medical applications of particle physics and was written in 2013 to obtain the Venia Docendi (Habilitation) in experimental physics at the University of Bern. It is based on selected publications, which represented at that time my major scientific contributions as an experimental physicist to the field of particle accelerators and detectors applied to medical diagnostics and therapy. The thesis is structured in two parts. In Part I, Chapter 1 presents an introduction to accelerators and detectors applied to medicine, with particular focus on cancer hadrontherapy and on the production of radioactive isotopes. In Chapter 2, my publications on medical particle accelerators are introduced and put into their perspective. In particular, high frequency linear accelerators for hadrontherapy are discussed together with the new Bern cyclotron laboratory. Chapter 3 is dedicated to particle detectors with particular emphasis on three instruments I contributed to propose and develop: segmented ionization chambers for hadrontherapy, a proton radiography apparatus with nuclear emulsion films, and a beam monitor detector for ion beams based on doped silica fibres. Selected research and review papers are contained in Part II. For copyright reasons, they are only listed and not reprinted in this on-line version. Particle Accelerators and Detectors for medical Diagnostics and Therapy

This thesis describes the design a novel type of particle accelerator for charged particle therapy. The accelerator is called a non-scaling, Fixed Field Alternating Gradient (ns-FFAG) accelerator, and will accelerate both protons and carbon ions to energies required for clinical use. The work is undertaken as part of the PAMELA project. An existing design for a ns-FFAG is taken as a starting point and analysed in terms of its ability to suit the charged particle therapy application. It is found that this design is particularly sensitive to alignment errors and would be unable to accelerate protons and carbon ions at the proposed acceleration rate due to betatron resonance crossing phenomena. To overcome this issue, a new type of non-linear ns-FFAG is developed which avoids resonance crossing and meets the requirements provided by clinical considerations. Two accelerating rings are required, one for protons up to 250 MeV and fully stripped carbon ions to 68 MeV/u, the other to accelerate the carbon ions up to 400-430 MeV/u. Detailed studies are undertaken to show that this new type of accelerator is suitable for the application. An alignment accuracy of 50 micrometers will not have a detrimental effect on the beam and the dynamic aperture for most lattice configurations is found to be greater than 50 pi.mm.mrad normalised in both the horizontal and vertical plane. Verification of the simulation code used in the PAMELA lattice design is carried out using experimental results from EMMA, the world's first ns-FFAG for 10-20 MeV electrons built at Daresbury Laboratory, UK. Finally, it is shown that the described lattice can translate into realistic designs for the individual components of the accelerator. The integration of these components into the PAMELA facility is discussed.

Design of a non-scaling fixed field alternating gradient accelerator for charged particle therapy

We propose an innovative fast-cycling accelerator complex conceived and designed to exploit at best the properties of accelerated ion beams for hadrontherapy. A cyclinac is composed by a cyclotron, which can be used also for other valuable medical and research purposes, followed by a high gradient linear accelerator capable to produce ion beams optimized for the irradiation of solid tumours with the most modern techniques. The properties of cyclinacs together with design studies for protons and carbon ions are presented and the advantages in facing the challenges of hadrontherapy are discussed.
