Hypertension immediately after coronary surgery is a problem in about one third of the patients so treated. This report discusses the possible causes of postoperative hypertension and describes several means of controlling the complication.

Durability of Mitral Valve Bioprostheses: A Meta-Analysis of Long-Term Follow-up Studies

Entre 1974 y 2006 se realizaron 1.535 recambios mitrales aislados (mortalidad hospitalaria, 5%). Un total de 369 pacientes requirieron una segunda intervencion (mortalidad hospitalaria, 8,1%) y 80, una tercera (59,8 ± 11,4 anos). Las causas de la tercera intervencion fueron deterioro estructural (67,5%), dehiscencia periprotesica (20%) y endocarditis (6,3%). La mortalidad hospitalaria fue 15 (18,8%) pacientes. Tras un seguimiento medio de 17,8 anos, 21 pacientes precisaron nueva intervencion (cuarta intervencion) y la curva actuarial libre de reoperacion fue del 40,1% ± 13,8% a 20 anos. La mortalidad tardia fue del 58,5% (supervivencia a 18 anos, 15,4% ± 5,4%). La indicacion de una reintervencion reiterativa mitral debe evaluarse de forma individualizada, dado el alto riesgo quirurgico asociado.

All patients (285) undergoing mitral valve replacement (MVR) with a Carpentier-Edwards (C-E) bioprosthesis +/- coronary bypass grafts (CABG) were reviewed (109 men and 176 women with a median age of 70 years). Overall, the 5-year survival rate was 58.9%, 62.7% for MVR (199 patients) and 50.1% for MVR+CABG (86 patients). Late survival was adversely affected by the operative time variables of NYHA class IV, older (> or = 70 years) age, low (> or = 56%) ejection fraction (EF), and the additional performance of associated procedures+CABG with MVR (P < or = 0.001). The 5-year freedom from stroke rate was 89.2%, 89.1% for MVR and 90.2% for MVR +/- CABG. Advanced heart class was the only significant variable associated with a greater risk of late stroke (P < or = 0.01). Neither chronic preoperative atrial fibrillation nor operative obliteration of the left atrial appendage increased or decreased the late risk of stroke in patients following MVR. Hazard function for stroke occurring in the first postoperative year (first 48 h excluded to discount intraoperative events) demonstrated the highest rate within the first month (40%), rapidly diminishing thereafter. This pattern was reproduced in the 12-year hazard function in that the rate of stroke occurrence was greatest in the first year (6.7%) following implantation. The mean stroke rate over 12 years was 2.5%. Strokes following MVR +/- CABG are more likely to occur in older and more compromised patients, and the higher early rate is not reflected in the mean rate. A more aggressive approach to early anticoagulation with IV heparin, Coumadin, and possibly antiplatelet therapy is advocated to reduce this complication rate.

The risk of stroke in the early postoperative period following mitral valve replacement.

458 patients with a Carpentier-Edwards porcine bioprosthesis (aortic (Ao): 169, mitral (Mi): 289) operated between January 1975 and December 1981, were studied during the first trimester of 1987. Forty seven patients underwent an associated operation. The total follow-up was 3,001 patient-years with a maximum follow-up of 11.4 years and a mean follow-up of 6.5 years. Only 5.6% of patients were lost to follow-up. The patients were aged between 20 and 80 years. The actuarial 9-year survival rate was 69.2 +/- 6.3% for aortic prostheses and 79.6 +/- 3.9% for mitral prostheses. The principal cause of valve failure, appearing with a considerable frequency after 5 years, was primary tissue degeneration which alone represented 67.8% of the causes of valve failure. The rate of absence of valve failure, for all causes combined, was 77.8 +/- 5.9% for the aortic position and 74.9 +/- 4.9% for the mitral position. The actuarial rate of absence of primary tissue degeneration at 9 years was 79.7 +/- 4.1% for aortic prostheses and 75.2 +/- 4.4% for mitral prostheses. The frequency of tissue degeneration decreased with increasing age, representing 2.9%, 1.9% and 1.5% patient-years respectively for the age-groups: 20 to 39 years, 40 to 59 years and 60 to 80 years. However, this difference was not statistically significant. Tissue degeneration was the principal cause for reoperation (n = 59) with an operative mortality of 7.8%.

The anti-hypertensive properties of sodium nitroprusside have been tested in 20 patients undergoing coronary arterial surgery. Were measured the arterial pressure, heart rate, mean right atrial pressure and mean left atrial pressure. The cardiac output, systemic vascular resistance and left ventricular stroke work index were deduced. A dose of 0.8 to 3 micrograms . kg-1 . min-1 sodium nitroprusside was given at the start of surgery, and immediately afterwards. The results showed a decrease of the systemic vascular resistance, a significant drop of arterial pressure, and a significant increase of heart rate as well as a tendency for the cardiac output to fall, probably because of insufficient vascular filing. When the mean left atrial pressure was kept at 14.8 +/- 3 mmHg (1.97 +/- 0.40 kPa), and left ventricular stroke work index fell, whilst cardiac output increased. Sodium nitroprusside seemed to be useful in coronary arterial surgery if used with care.
